Early childhood language delay: the otolaryngologist's perspective.

Many etiologic factors have been implicated in delayed language development in the preschool child including: low birth weight, perinatal insults to the CNS, and chronic/recurrent middle ear disease with conductive hearing impairment during the first years of life. The results of an in-depth evaluation of 24 children, who manifested preschool language delay significant enough to require remedial intervention, are reported. A suggested early identification model employing High Risk Factors and a Developmental Screening Questionnaire for use by otolaryngologists, pediatricians and audiologists i…

Hearing Aid Service Models, Technology, and Patient Outcomes: A Randomized Clinical Trial.

IMPORTANCE: The poor affordability of hearing aids (HAs) limits their adoption. To justify higher costs, HAs fitted by audiologists (AUD service model) and high-end HAs should deliver better outcomes than over-the-counter (OTC) service models and low-end HAs. OBJECTIVE: To determine the effect of HA service models (AUD, OTC, and a hybrid OTC+ model) and technology levels (high end and low end) on patient outcomes. DESIGN, SETTING, AND PARTICIPANTS: This randomized clinical trial was conducted at the University of Iowa and Vanderbilt University Medical Center in research laboratories from Feb…

Auditory and Vestibular Side Effects of FDA-Approved Drugs for Diabetes.

As of September 10, 2019, there were 75 medications (oral/injectable/inhalants) approved by the U.S. Food and Drug Administration for diabet … side effects (one each). More than 50% of the drugs will have some effect on a balance assessment. Audiologists who evaluate a patient with diabetes (PWD) are presented with symptoms that could be related to their diabetes medication. Although the manufacturer might report a side effect as rare or as a percentage from their clinical trials subjects, not all P …

Diagnosis of hearing loss using automated audiometry in an asynchronous telehealth model: A pilot accuracy study.

Introduction Standard criteria exist for diagnosing different types of hearing loss, yet audiologists interpret audiograms manually. This pilot study examined the feasibility of standardised interpretations of audiometry in a telehealth model of care. The aim of this study was to examine diagnostic accuracy of automated audiometry in adults with hearing loss in an asynchronous telehealth model using pre-defined diagnostic protocols. Materials and methods We recruited 42 study participants from a public audiology and otolaryngology clinic in Perth, Western Australia. Manual audiometry was perfo…

Screening measurements and procedures. Exemplified by an identification audiometry program.

To summarize, an effective procedure in audiometric screening may be using skilled testers to screen 1, 2, and 4 kHz at 20 dBHL (with oto immittance measures) on all students up to and including grade 3, 9th and 12th year high school students, and high-risk groups including those repeating a grade, having a history of recurrent middle ear disease, having speech and/or language problems, known to participate in high noise activities, for example, guns, dirt bikes, or model planes, and those failing previous screenings. Testing is done in a quiet, nondistracting environment, with each person tes

Availability of resources for paediatric hearing care in a South African province.

BACKGROUND: Unavailability of healthcare resources can lead to poor patient outcomes. The latter is true for infants with hearing loss and require early hearing detection and intervention (EHDI). AIM: To determine the availability and distribution of resources for EHDI in state hospitals in the Eastern Cape (EC) province, South Africa. SETTING: Sixteen state hospitals (nine district, four regional and three tertiary hospitals). METHODS: Descriptive cross-sectional survey completed between July 2022 and October 2022. RESULTS: Thirteen hospitals had audiologists (n = 4) or speech therapists …

The knowledge, experience, and awareness of professionals working with older adults on the auditory-cognitive interactions.

INTRODUCTION: Hearing loss is a commonly occurring condition with dementia. Research already presented a theoretical framework for the auditory-cognitive interactions, though it is still unclear if and how professionals beyond audiologists act upon this interactions in clinical practice. METHODS: An online 64-item questionnaire was developed and evaluated respondents' work setting as well as their knowledge, experience, and awareness regarding hearing loss, cognitive decline, and the auditory-cognitive link. The questionnaire was sent to Dutch professionals working with older adults. Responde…

INTRODUCTION: Increasingly person-centered care (PCC) is being recognized as an important aspect of speech-language pathology and audiology … 91 practitioners (39.6% speech-language pathologists) completed the online survey, with nine (44.4% audiologists) participating in the focus group discussions. A high preference toward PCC was noted, with a total mean mPPOS score of 4.6 (0.6 SD). Quantile regression analysis revealed four predictors (age, home language, sector, and personality trait openness) …

[Hearing aid provision in children].

Achieving high-quality hearing aid fitting in children is an interdisciplinary task and a challenge for all involved specialist disciplines. According to quality agreements (standards), comparative hearing aid fitting is mandatory prior to prescription. Furthermore, RECD measurements and, in older children, in-situ measurements should be applied on a regular basis for finetuning and adjustment of the hearing aid fitting. ENT physicians, phoniatricians and pediatric audiologists, pediatric acousticians, teachers of the hearing impaired, and special educational needs teachers should work togethe…

Management of Children with Severe, Severe-profound, and Profound Sensorineural Hearing Loss.

Management of children with severe, severe-profound, and profound sensorineural hearing loss is best achieved using a family-centered approach by a team of health professionals, including audiologists, speech pathologists, otolaryngologists, pediatricians, genetic counselors, and early intervention programs. Early diagnosis and intervention offers the best chance for speech and language acquisition. Although hearing aids can provide some of the needed information, they are often not sufficient for spoken language development and a cochlear implant is needed. This must be combined with a strong…

Activation region overlap visualization for image-guided cochlear implant programming.

Objective. The cochlear implant is a neural prosthesis designed to directly stimulate auditory nerve fibers to induce the sensation of hearing in those experiencing severe-to-profound hearing loss. After surgical implantation, audiologists program the implant's external processor with settings intended to produce optimal hearing outcomes. The likelihood of achieving optimal outcomes increases when audiologists have access to tools that objectively present information related to the patient's own anatomy and surgical outcomes. This includes visualizations like the one presented here, termed the…
