Effects of resistance training during bed rest on the viscoelastic properties of tendon structures in the lower limb.

The purpose of this study was to investigate the effects of resistance training on the tendon properties in knee extensors during 20 days of bed rest. Sixteen men were assigned to the resistance training group (BR-Tr) or the non-training, control group (BR-Con). Leg-press exercises were performed as five sets of 10 repetitions at 90% of maximum load daily for 20 days during bed rest. Before and after bed rest, the elongation of the tendon structures of the vastus lateralis muscle during isometric knee extension was determined using ultrasonography, while subjects performed ramp isometric contr…

Effect of lower body negative pressure against orthostatic intolerance induced by 21 days head-down tilt bed rest.

BACKGROUND: Exposure to actual or simulated weightlessness is known to induce orthostatic intolerance in humans. Many different methods have been suggested to counteract orthostatic hypotension. The repetitive or prolonged application of lower body negative pressure (LBNP) has shown beneficial effects to counter orthostatic intolerance, but devoting so much time to countermeasures is not compatible with space mission objectives or costs. The purpose of the present study was to assess the effects of brief LBNP sessions against orthostatic intolerance during a 21-d head-down tilt (HDT) bed rest.…

Regional changes in cerebral blood flow between the upright and supine posture and over 3 days of bed rest.

A reduction in cerebral blood flow (CBF) has been observed during spaceflight and bed rest. We aimed to examine the magnitude and regional heterogeneity of the decrease in CBF during bed rest compared to posture changes on Earth. Seventeen participants (age, 29 ± 9 years, 7 females) were studied in the upright and supine posture and over 3 days of bed rest. We assessed blood flow via duplex ultrasonography in the internal carotid (ICA) and vertebral arteries (VA), and via transcranial Doppler of the middle cerebral artery (MCAv). Mean arterial pressure (MAP) and end-tidal CO2 ( E T C O 2 )…

Alterations in blood pressure dependent activation of leg muscles during standing following bed rest mimic those observed with ageing.

INTRODUCTION: Baroreflex-mediated activation of lower leg muscles (i.e., muscle-pump baroreflex) has been suggested to play a significant ro … mined in young adult male participants (35 ± 2 years) before and after 60 days of 6° head-down tilt bed rest (HDBR). By analyzing the interactions between systolic blood pressure (SBP) and heartbeat-by-heartbeat electromyogram impulse (EMGimp) during standing, the interactions between SBP and EMGimp including muscle-pump baroreflex were characterized by fr …

Bed rest duration and development of cerebrospinal fluid leaks after intradural spinal surgery: a meta-analysis of comparative studies.

INTRODUCTION: Postoperative cerebrospinal fluid (CSF) leaks and related complications are a major concern after intradural spinal surgeries. The role of prolonged bed rest in reducing the incidence of these complications has been debated. This meta-analysis aimed to evaluate whether early versus late mobilization affects the incidence of CSF leak-related complications (CLRC) after intradural spinal surgery. EVIDENCE ACQUISITION: Following PRISMA guidelines, we conducted a systematic review and meta-analysis of comparative studies on early ambulation (EA) versus prolonged bed rest (PBR) in pat…

Effect of bed rest duration after untethering surgery for thickened/fatty filum terminale: A systematic review and meta-analysis.

Transection of the filum terminale is a common surgical procedure in paediatric patients and post-operative cerebrospinal fluid (CSF) leak/pseudomeningocoele is its most common surgical complication. Alongside intraoperative methods to achieve watertight dural closure, the recommendation of post-operative bed rest is common practice. The duration of bed rest varies considerably, with little evidence to support the efficacy of prolonged (> 24 h) durations. This work sought to evaluate existing literature to determine the effect of the duration of bed rest on the incidence of CSF complications a…

Effects of Posture and Walking on Tibial Vascular Hemodynamics Before and After 14 Days of Head-Down Bed Rest.

Human skeletal hemodynamics remain understudied. Neither assessments in weight-bearing bones during walking nor following periods of immobility exist, despite knowledge of altered nutrient-artery characteristics after short-duration unloading in rodents. We studied 12 older adults (8 females, aged 59 ± 3 years) who participated in ambulatory near-infrared spectroscopy (NIRS) assessments of tibial hemodynamics before (PRE) and after (POST) 14 days of head-down bed rest (HDBR), with most performing daily resistance and aerobic exercise countermeasures during HDBR. Continual simultaneous NIRS rec…

Risk factors for venous thromboembolism induced by prolonged bed rest during interstitial brachytherapy for gynecological cancer: a retrospective study.

BACKGROUND: Screening and management of venous thromboembolism (VTE) after surgery is important in preventing sublethal VTE. However, the risk factors for VTE during interstitial brachytherapy (ISBT) remain unknown, and appropriate screening and management strategies are yet to be established. Therefore, this study aimed to evaluate the risk factors for VTE resulting from requisite bed rest during ISBT for gynecologic cancers. METHODS: We retrospectively analyzed 47 patients. For patients without definitive preceding radiotherapy, whole pelvic irradiation (30-50 Gy) followed by ISBT of 12-30 …

[Short-term bed rest as a cause of deep vein thrombosis in a patient with a huge abdominal tumor].

A 43-year-old female with a huge myoma uteri was scheduled for a simple total hysterectomy. However, there was massive penetration of the tumor into the retroperitoneum, and her first operation failed because we had no advance information on its posterior aspect. Angiography was done to obtain this information. She rested on bed until the day after the first operation and angiography. MRI, performed again due to early growing tumor, revealed deep vein thrombi in the left iliac vein to the left femoral vein which the last MRI had not shown. Therefore, just before the second operation a temporar

The effects of hospitalization for bed rest on duration of gestation, fetal growth and neonatal morbidity in triplet pregnancy.

Nineteen women attending a special multiple pregnancy antenatal clinic with a triplet pregnancy were randomly allocated to either bed rest in hospital from 24 weeks gestation onwards until delivery, or to continue conventional outpatient management. Conclusions are limited by the trial size, but the study suggests that routine hospitalization for bed rest decreases the incidence of preterm delivery and light-for-gestational age infants and reduces the need for intensive neonatal care. Although still compatible with change variation, the observations, if confirmed in a larger randomized study, …

Alterations in Muscle Contractile Properties, Structure, and Function during 10-d Bed Rest, Postrecovery, and after COVID-19 Lockdown.

PURPOSE: Tensiomyography detects early hallmarks of bed-rest-induced atrophy. We aimed to identify early tensiomyography alterations in six muscles during a 10-d bed rest (BR10), followed by a 30-d recovery period (R+30), and to compare these changes with those observed after the 50-d COVID-19 lockdown in 10 healthy males (22.9 ± 5.0 yr). METHODS: Tensiomyography and muscle thickness (assessed by ultrasound imaging) were assessed at baseline, during bed rest, at BR10, R+30, and after the COVID-19 lockdown. Additional assessments included vertical jump performance (force plate), body compositi…

Daily use of countermeasures to prevent headward fluid shifts throughout 30 days of strict head-down tilt bed rest.

Strict 6° head-down tilt bed rest (HDTBR) induces a chronic headward fluid shift similar to weightlessness that may underlie the development of spaceflight-associated neuro-ocular syndrome. We tested the efficacy of two headward fluid shift countermeasures, using either 25 mmHg lower body negative pressure (LBNP, 2 × 3 h daily, n = 12) or use of venoconstrictive thigh cuffs (VTC) following 1 h of moderate aerobic cycle exercise (Ex + CUFF, 6 h daily for 6 of 7 days, n = 12) across 30 days of HDTBR. The study also included a seated posture group (Upright, 2 × 3 h daily, n = 11), which was condu…

Five days of bed rest in young and old adults: Retainment of skeletal muscle mass with neuromuscular electrical stimulation.

The consequences of short-term disuse are well known, but effective countermeasures remain elusive. This study investigated the effects of neuromuscular electrical stimulation (NMES) during 5 days of bed rest on retaining lower limb muscle mass and muscle function in healthy young and old participants. One leg received NMES of the quadriceps muscle (3 × 30min/day) (NMES), and the other served as a control (CON). Isometric quadriceps strength (MVC), rate of force development (RFD), lower limb lean mass, and muscle thickness were assessed pre-and post-intervention. Muscle thickness remained unal…

Acetazolamide, Short Bed Rest, and Subfascial Off-Suction Drainage in Preventing Persistent Spinal Fluid Leaks from Incidental Dural Tears.

BACKGROUND: Postoperative cerebrospinal fluid (CSF) leak might occur after the repair of dural tears. Acetazolamide lowers intradural pressure by decreasing CSF production. The aim of this study was to investigate the incidence of persistent CSF leak from incidental dural tears after using acetazolamide, short bed rest, and subfascial off-suction drainage. METHODS: The medical records at the Ain Shams University hospital were examined retrospectively for this descriptive single-arm cohort study. Adult patients who underwent lumbar degenerative spine surgery from January 2011 through January 2…

Prolonged bed rest impairs rapid CPI-17 phosphorylation and contraction in rat mesenteric resistance arteries to cause orthostatic hypotension.

Prolonged bed rest (PBR) causes orthostatic hypotension (OH). Rapid constriction of splanchnic resistance arteries in response to a sudden increase in sympathetic tone contributes to the recovery of orthostatic arterial pressure upon standing. However, the molecular mechanism of PBR-induced dysfunction in arterial constriction is not fully understood. Previously, we showed that CPI-17, a regulatory protein for myosin phosphatase, mediates α1A-adrenergic receptor-induced rapid contraction of small mesenteric arteries. Here, we tested whether PBR associated with OH affects the α1-adrenergic rece…
