Detection of Blastomyces dermatitidis Antigen in Urine Using a Commercially Available Quantitative Enzyme Immunoassay.

Laboratory diagnosis of blastomycosis relies on a combination of methods, including antigen detection. We assessed the performance of analyte-specific reagents from Gotham Biotech (Portland, ME) for quantitative detection of Blastomyces dermatitidis galactomannan (GM) in urine using an enzyme immunoassay (EIA) compared to the Blastomyces quantitative EIA from MiraVista Diagnostics (Indianapolis, IN). Residual urine from 232 unique patients previously tested by the MiraVista assay was evaluated using the Gotham EIA, which showed 97.4% (74/76), 100% (156/156), and 99.1% (230/232) positive, negat…

Discordant influence of Blastomyces dermatitidis yeast-phase-specific gene BYS1 on morphogenesis and virulence.

Blastomyces dermatitidis is a thermally induced dimorphic fungus capable of causing lung and systemic infections in immunocompetent animal hosts. With the publication of genomic sequences from three different strains of B. dermatitidis and the development of RNA interference as a gene-silencing tool, it has become possible to easily ascertain the virulence and morphological effects of knocking down the expression of candidate genes of interest. BYS1 (Blastomyces yeast-phase-specific 1), first identified by Burg and Smith, is expressed at high levels in yeast cells and is undetectable in mold. …

The WI-1 adhesin blocks phagocyte TNF-alpha production, imparting pathogenicity on Blastomyces dermatitidis.

The WI-1 adhesin is indispensable for pathogenicity of Blastomyces dermatitidis and is thought to promote pulmonary infection by fixing yeast to lung tissue and cells. Recent findings suggest that WI-1 confers pathogenicity by mechanisms in addition to adherence. Here, we investigated whether WI-1 modulates host immunity by altering production of pro-inflammatory cytokines. Production of TNF-alpha in lung alveolar fluids of mice infected with B. dermatitidis was severalfold higher for WI-1 knockout yeast compared with wild-type yeast, and in vitro coculture of unseparated lung cells with these…

Attempted isolation of Blastomyces dermatitidis from the nares of dogs: Northern Wisconsin, USA.

The nasal cavities of domestic animals may concentrate and allow isolation of environmentally acquired fungal pathogens. We obtained two swabs each from the nares of 110 asymptomatic, physically normal dogs from a veterinary practice in Eagle River, WI, USA, an area highly endemic for blastomycosis. Four of the tested dogs had past histories of blastomycosis. Samples were placed on yeast extract phosphate (Smith's) media at 20 degrees C but growth of Blastomyces dermatitidis was not observed on any of the 220 cultures. One dog developed cytologically confirmed B. dermatitidis one month followi…

Small subunit ribosomal DNA sequence shows Paracoccidioides brasiliensis closely related to Blastomyces dermatitidis.

The similarities of paracoccidioidomycosis and blastomycosis are highly suggestive of a close relation of the two etiological agents. Wherea … maximum likelihood analyses and, finally, the Kishino-Hasegawa test revealed that P. brasiliensis, Blastomyces dermatitidis, and Emmonsia parva are more closely related than Histoplasma capsulatum and B. dermatitidis, whose teleomorphic forms belong to one genus, Ajellomyces. In accordance with the work of other investigators who have used internal transcrib …

Western immunoblot analysis and serologic characterization of Blastomyces dermatitidis yeast form extracellular antigens.

A major 98-kDa extracellular protein antigen of Blastomyces dermatitidis was shown in Western blot (immunoblot) analysis to be highly reactive with serum antibodies from patients with blastomycosis. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis analysis of yeast form B. dermatitidis culture filtrates and cell extracts demonstrated over 50 proteins, only 24 of which were immunoreactive. Of these, a 98-kDa protein was found to be the most specific and was isolated. This protein was found in both broth culture filtrates and extracts of yeast forms. Western blot tests with this antigen…

Intracellular and extracellular defenses of human phagocytes against Blastomyces dermatitidis conidia and yeasts.

The lesions of blastomycosis are characterized by both suppuration and granuloma formation, but the relative roles of human neutrophils, monocytes, and macrophages against Blastomyces dermatitidis are poorly defined. Our studies reveal that B. dermatitidis yeasts are generally too large to be ingested by polymorphonuclear neutrophils (PMNs), and are killed predominantly by external PMN attachment and degranulation, whereas conidia are first ingested, then killed. PMN function is maximal in the presence of serum, divalent cations, and complement, and killing is more efficient for conidia (appro…

Mortality associated with Blastomyces dermatitidis infection: A systematic review of the literature and meta-analysis.

Published case fatality in blastomycosis patients ranges between 4% and 78%. This study aimed to assess mortality associated with blastomycosis and identify its associated risk factors. We conducted a systematic review of publications related to Blastomyces dermatitidis available in PubMed and Scopus databases. Studies that reported data on blastomycosis mortality and that were published from inception through February 2018 were assessed and included in the analysis. Using the R meta package, a random-effect model meta-analysis was used to calculate pooled and stratified estimates of case-fata…

Classic pyomyositis of the extremities as an unusual manifestation of Blastomyces dermatitidis: a report of two cases.

Pyomyositis is an infection of skeletal muscle that, by definition, arises intramuscularly rather than secondarily from adjacent infection. It is usually associated with bacterial infection, particularly Staphylcococcus aureus. Fungi are rare causes, and Blastomyces dermatitidis has not been reported previously. In this case series, we report two cases of pyomyositis caused by B. dermatitidis. Cases were prospectively identified through routine clinical care at a single academic referral hospital. Two patients with complaints of muscle pain and subacute cough were treated at our hospital in 20…

Differential requirements of T cell subsets for CD40 costimulation in immunity to Blastomyces dermatitidis.

Cell-mediated immunity and production of type 1 cytokines are the main defenses against pathogenic fungi. Ligation of CD40 by CD40L on T cells is critical for the induction of these immune responses in vivo. We explored the role of CD40/CD40L interactions in vaccine immunity to Blastomyces dermatitidis by immunizing CD40(-/-) and CD40L(-/-) mice and analyzing their resistance to reinfection in a murine pulmonary model. In the absence of CD40 or CD40L, CD4(+) cells failed to get primed or produce type 1 cytokine and impaired the generation of CD8(+) T1 cells. The CD8(+) T cell defect was not du…

CD28 is required for optimal induction, but not maintenance, of vaccine-induced immunity to Blastomyces dermatitidis.

Cellular immunity mediated by T lymphocytes, in particular CD4+ and CD8+ type 1 cells, is the main defense against pathogenic fungi. Here, CD28-deficient (CD28-/-) mice were used to study the role of costimulation for the generation and maintenance of T-cell-mediated, type 1 cytokine-dependent mechanisms of vaccine immunity to Blastomyces dermatitidis infection. Disruption of CD28 costimulation reduced the number of type 1 CD4 and CD8 cells generated and impaired resistance to infection. Type 1 T-cell subsets generated in vaccinated CD28-/- mice were durable and protected mice for at least 3 m…

Role of glucan and surface protein BAD1 in complement activation by Blastomyces dermatitidis yeast.

Our previous studies showed that Blastomyces dermatitidis yeast activates the human complement system, leading to deposition of opsonic complement fragments onto the yeast surface. This report examines the influence of altered surface expression of glucan or BAD1 protein (formerly WI-1) on the yeast's ability to activate and bind C3. Compared to the wild type, a glucan-deficient mutant yeast delayed initiation of C3 deposition and reduced C3-binding capacity by 50%. Linkage of baker's-yeast beta-glucan to the glucan-deficient yeast restored initial C3 deposition kinetics to the wild-type level…

The in vitro isolation of Blastomyces dermatitidis from a woodpile in north central Wisconsin, USA.

Environmental isolations of the dimorphic fungus, Blastomyces dermatitidis, the aetiologic agent of blastomycosis are rare, and have usually involved animal inoculation techniques. We report the in vitro isolation of B. dermatitidis from a woodpile in November 1997, from a private property in a highly endemic area of north central Wisconsin, USA. The woodpile was 73 m from the Wisconsin River and 5 m from a kennel which had housed nine dogs over the past 14 years, four of which had been diagnosed with blastomycosis. One of 19 samples from the property yielded B. dermatitidis after 37 degrees C…

Characteristics of the pulmonary cellular immune response to two strains of Blastomyces dermatitidis in the mouse.

In order to assess the cellular responses in the lung in murine pulmonary blastomycosis, serial lung lavages were performed in normal BALB/cByJ mice that had received an intranasal inoculation with 1 of 2 strains of Blastomyces dermatitidis of opposite virulence. The virulent strain, ATCC 26199, induced an increasing number of neutrophils recovered from the lung lavages. An early peak of incoming neutrophils was seen at 1 to 2 days, followed by a more rapid accumulation of intraalveolar neutrophils, until by the ninth day after infection, 80% of the cells recovered from the mice were neutrophi…
