In recent years technologic advances have led to the understanding of the molecular genetics of many blood group antigens. This review highlights the molecular basis of selected antigens and describes implications for transfusion medicine. Viral infections transmitted by the donor organ or by blood and blood products are severe complications in heart transplantation. The use of blood and blood saving management is evaluated in 57 consecutive orthotopic heart transplantations. Indication was cardiomyopathy in 63%, coronary artery disease in 30%, valve disease in 5%, congenital in 5%, arrhythmia in 5% and primary cardiac malignancy in 2%. Exchange of blood units between transfusion services and blood depots, using EDP systems requires standardisation of unit identification and bar-codes. The ISBT Working Party on Data Processing defined recommendations for blood-unit identification by Codabar. Our group (German Society of Transfusion Medicine and Immunohematology, Section Data Processing and Standardization) extended and adapted these standards to the German blood bank settings. This paper describes the basic definitions and standards. A complete outline will be published in the next future.

According to the European Hypertension Guidelines regarding office blood pressure measurements (OBPMs), the mean between second/third or third/fourth OBPM should be taken if the first two readings differ by ≤10 or >10 mmHg, respectively. A man aged 42 years died from a hemorrhage resulting from stab wounds in his neck, chest and abdomen. The MN blood group and PGM1 phenotype of the victim's heart blood were inconsistent with those of the blood stains around the victim. The victim was transfused with 4000 ml of blood during the 9 hours before his death. Because PGM1 phenotypes obtained from blood stains at the scene of the crime and from the weapon corresponded with those from the victim's dental pulp, peripheral nerve and bone marrow, the victim's blood groups and isoenzymic phenotypes could be determined accurately.
