Efficacy of blood flow restriction training in chronic ankle conditions: a systematic review and meta-analysis.

BACKGROUND: Chronic ankle conditions often lead to persistent functional limitations. Blood flow restriction (BFR) training is a potential adjunct to rehabilitation, but its specific efficacy for chronic ankle conditions remains to be synthesized. OBJECTIVE: To systematically evaluate the effects of BFR-assisted rehabilitation on primary outcomes (dynamic balance and patient-reported ankle stability) and secondary outcomes (ankle range of motion and muscle strength) in individuals with chronic ankle conditions (including chronic ankle instability, chronic ligamentous injury, and tendinopathy)

Treatment of hypertension with angiotensin converting enzyme inhibitors.

Angiotensin converting enzyme inhibitors are effective therapy in hypertension. They are particularly useful in severe drug resistant or accelerated hypertension, in renal hypertension and in hypertensive heart failure. Although their exact mode of action has not been determined it is a consequence of the inhibition of angiotensin converting enzyme. They offer distinct advantages over conventional drugs in the treatment of high blood pressure particularly as they have no central or autonomic side effects and as a consequence the patients feel well. There is no postural effect on blood pressure

Flow rate modulates focused ultrasound-mediated vascular delivery of microRNA.

Gene therapy targeting ischemic heart disease is a promising therapeutic avenue, but it is mostly restricted to viral-based delivery approaches which are limited due to off-target immunological responses. Focused ultrasound presents a non-viral, image-guided technique in which circulating intravascular microbubble contrast agents can reversibly enhance vascular permeability and gene penetration. Here, we explore the influence of flow rate on the microbubble-assisted delivery of miR-126, a potent pro-angiogenic biologic, using a custom acoustically coupled pressurized mesenteric artery model. 12. Vascular pain: Ischemic pain in the extremities and Raynaud's syndrome.

INTRODUCTION: Peripheral artery diseases (PAD) and Raynaud's syndrome are associated with substantial morbidity. PAD, through the restriction of blood flow to the extremities, may lead to critical limb ischemia with symptoms of pain at rest which may eventually progress to severe limb ischemia with gangrene. This serious and painful clinical condition requires extensive medical care, is limb-threatening and, in case of delayed or unsuccessful treatment, is associated with a high mortality rate. In Raynaud's syndrome, the blood supply to certain parts of the body, usually the fingers and toes a

A case report of gastroschisis: Prenatal diagnosis in primary care settings matters.

Gastroschisis may lead to serious congenital disability, as prolonged exposure to the amniotic fluid could compromise the mesenteric blood flow and irritate the foetal intestinal wall. The consequences of gastroschisis include spontaneous preterm birth, growth restriction and foetal demise. Awareness of this disease is essential, as a high mortality rate is linked to inadequate initial therapy. Early detection of the disease, mainly by primary care physicians as the initial point of contact, is crucial to improve the course of the condition through early monitoring and referral to tertiary car

Cardioprotective and anti-inflammatory properties of blood flow restriction training in older or high-risk cardiovascular disease population: systematic review and meta-analysis.

INTRODUCTION: Blood flow restriction (BFR) training combined with aerobic exercise (AE) is proposed to benefit elderly and high-risk cardiovascular populations due to its lower joint and cardiovascular strain. However, its safety remains unclear. This meta-analysis evaluates the efficacy and safety of BFR training in improving cardiovascular health and inflammatory markers in elderly and high-risk individuals. EVIDENCE ACQUISITION: Following PRISMA guidelines, six databases were systematically searched for studies involving BFR training in elderly (>60 years) or high-risk cardiovascular popul

Hepatic veno-occlusive disease in children after hematopoietic stem cell transplantation.

Hepatic veno-occlusive disease (VOD), or sinusoidal obstruction syndrome, is a potentially fatal complication, which occurs in 7-27% of children undergoing hematopoietic stem cell transplantation. In this article, we review commonly accepted diagnostic criteria, atypical diagnostic features as well as preventative and treatment measures associated with VOD. Reversal of portal venous flow by Doppler ultrasound is often a late finding; many patients with anicteric VOD may never develop hyperbilirubinemia, and yet perish from severe VOD. Transjugular liver biopsy is usually not available and/or i

Association of obliterative extracranial vascular disease with carotid sinus hyperaesthesia: a possible mechanism of transitory cerebral ischaemia.

In extensive studies of cerebral circulation it has been shown that obliterative extracranial vascular disease is associated with a particular regulatory pattern of cerebral circulation. In stenosis of the vertebral artery, of the internal carotid or in the subclavian steal syndrome, cerebral blood flow and O2 consumption was adequate under resting conditions and it is essentially the high cerebrovascular resistance which points to some derangement. The haemodynamic reserves of the brain being restricted, a sudden fall in heart rate may produce a significant decrease in the cerebral blood flow

Sildenafil citrate increases fetal weight in a mouse model of fetal growth restriction with a normal vascular phenotype.

Fetal growth restriction (FGR) is defined as the inability of a fetus to achieve its genetic growth potential and is associated with a significantly increased risk of morbidity and mortality. Clinically, FGR is diagnosed as a fetus falling below the 5(th) centile of customised growth charts. Sildenafil citrate (SC, Viagra™), a potent and selective phosphodiesterase-5 inhibitor, corrects ex vivo placental vascular dysfunction in FGR, demonstrating potential as a therapy for this condition. However, many FGR cases present without an abnormal vascular phenotype, as assessed by Doppler measures of

Blood Flow Restriction Training Reduces Hemodynamic Load and Improves Cardiovascular Biomarkers in Runners with Exercise-Induced Hypertension.

Exercise-induced hypertension (EIH) is associated with elevated risks of arrhythmia, coronary artery disease, arterial stiffness, and sudden cardiac death. Blood flow restriction (BFR) training has been proposed as a non-pharmacological intervention. This study aimed to evaluate whether changes in cardiovascular biomarkers accompanied hemodynamic improvements following BFR training in middle-aged runners with EIH. This secondary analysis included 28 male runners with EIH (mean age: 57.4 ± 6.5 years) from a prior cohort applying the same BFR training protocol. Participants exhibited a maximal s

Negative pressure ventilation as haemodynamic rescue following surgery for congenital heart disease.

A low cardiac output state is an important cause of morbidity and mortality following repair of tetralogy of Fallot (ToF). This is often refractory to conventional measures. The cardiac output of these patients is highly dependent on diastolic pulmonary arterial flow which is enhanced during spontaneous respiration, but much reduced by intermittent positive pressure ventilation (IPPV). We report the successful use of negative pressure ventilation (NPV) as haemodynamic therapy in three children with a low output secondary to restrictive right ventricular (RV) physiology following ToF repair. A new national treatment guideline from Danish Society of Anaesthesiology and Intensive Care Medicine (DASAIM) outlines the anaesthetic management of pre-eclampsia and eclampsia. Key principles involve blood pressure control, seizure treatment and prophylaxis, and restricted fluid therapy. Treatment algorithms, flow charts and checklists are provided. Anaesthetic recommendations for vaginal delivery and caesarean section are provided. Close monitoring, especially for patients with severe disease, organ dysfunction, or failure, is crucial, requiring a multidisciplinary approach, as argued in th

AMPK activation in pregnant human myometrial arteries from high-altitude and intrauterine growth-restricted pregnancies.

High-altitude (>2,500 m) residence increases the incidence of intrauterine growth restriction (IUGR) due, in part, to reduced uterine artery blood flow and impaired myometrial artery (MA) vasodilator response. A role for the AMP-activated protein kinase (AMPK) pathway in protecting against hypoxia-associated IUGR is suggested by genomic and transcriptomic studies in humans and functional studies in mice. AMPK is a hypoxia-sensitive metabolic sensor with vasodilatory properties. Here we hypothesized that AMPK-dependent vasodilation was increased in MAs from high versus low-altitude (<1,700 m) C

Low-load blood flow restriction training and ischemia modulate expression of Na+,K+-ATPase and FXYDs in human skeletal muscle.

Anterior cruciate ligament (ACL) rupture leads to muscle deconditioning and downregulation of Na+,K+-ATPase (NKA). Low-load blood flow restriction (LL-BFR) training was shown to improve muscle function after ACL injury, but its effects on NKA are unknown. We analyzed the expression of NKA and its FXYD regulators in knee muscles from ACL-injured subjects undergoing LL-BFR, low-load training with sham blood flow restriction (LL-Sham), or no training (Control). In addition, we dissected effects of ischemia components by subjecting cultured human myotubes to glucose deprivation and/or hypoxia. The

Reduction of early vein graft thrombosis by tissue plasminogen activator gene transfer.

Vein grafts are used to bypass coronary arterial stenosis, but many grafts thrombose soon after surgery. A model was developed in the pig to allow continuous measurement of blood flow and production of flow-restricting thrombi (cyclic flow reductions; CFRs). Saphenous vein lumen was exposed to adenovirus ex vivo, to over-express human tissue plasminogen activator (h-tPA), with beta-galactosidase adenovirus as a control. The vein segments were engrafted into carotid arteries and examined 0, 1 or 3 days later (4-7 animals/group). Untransduced grafts examined on the day of surgery developed repea

Effect of sprint interval cycling with blood flow restriction on capillary blood lactate, cognitive function and perceptual response: a randomized controlled trial.

Regular participation in physical activity can slow age-related functional and cognitive decline but many have cited a lack of time and energy as key barriers to exercise adherence. Hence, exercises with lower time commitment may appeal to sedentary individuals or working adults who perform insufficient physical activity. Sprint interval exercise with blood flow restriction (BFR-SIE) is a time-efficient exercise modality that may augment cognitive gains through enhanced metabolic stimulation and cerebrovascular alterations. Therefore, we aimed to investigate the effect of BFR-SIE on capillary

Placental MRI and its application to fetal intervention.

OBJECTIVE: Magnetic resonance imaging (MRI) of placental invasion has been part of clinical practice for many years. The possibility of being better able to assess placental vascularization and function using MRI has multiple potential applications. This review summarises up-to-date research on placental function using different MRI modalities. METHOD: We discuss how combinations of these MRI techniques have much to contribute to fetal conditions amenable for therapy such as singletons at high risk for fetal growth restriction (FGR) and monochorionic twin pregnancies for planning surgery and

The effects of blood flow restriction training on early muscle strength and mid-term knee function following anterior cruciate ligament reconstruction: a systematic review and meta-analysis.

OBJECTIVE: Early restoration of muscle strength and knee joint function after anterior cruciate ligament reconstruction (ACLR) is a critical goal in the rehabilitation process. Blood flow restriction training (BFRT), a low-load training method, has gained attention in musculoskeletal rehabilitation in recent years, but its specific effects in ACLR rehabilitation remain unclear. METHODS: Relevant literature up to December 20, 2024, was searched in the PubMed, Embase, Cochrane, and Web of Science databases, and study selection was performed according to PRISMA guidelines.
