Forearm fractures in children. Single bone fixation with elastic stable intramedullary nailing in 20 cases.

We present our experience with elastic stable intramedullary nailing (ESIN) used in the single bone fixation of both bones forearm fractures in children. From May 2002 to July 2004, 20 children (14 boys and 6 girls), median age of 10 years (range 6-15 years) were treated with ESIN for 16 closed and 4 grade I open forearm fractures. All patients were reviewed clinically at a median follow-up of 20 months (range 6-30 months). All fractures were radiologically united at a median of 6.7 weeks (6-9 weeks). The median operating time was 35min (range 25-60min). The median hospital stay was 2 days (ra

In situ and split-thickness grafting for nail bed defects with bone exposure: A retrospective case series.

Nail bed defects accompanied by bone exposure are challenging to treat because they rarely heal spontaneously and frequently require flap coverage or digit amputation. This study evaluated the postoperative function and appearance of nail bed defects repaired using in situ or split-thickness nail bed grafting. From March 2019 to December 2023, 28 (32 fingers) patients with nail bed defects and bone exposure underwent in situ or split-thickness autologous nail bed grafting. The clinical outcomes were quantitatively evaluated across five domains: nail plate appearance, proximal nail fold adheren

Elastic Stable Intramedullary Nail Fixation for Pediatric Forearm Bone Fractures: Outcomes and Complications Rate.

BACKGROUND: Forearm shaft fractures are common among children. Elastic stable intramedullary nailing (ESIN) is generally considered the gold standard operative procedure. This study aimed to assess functional outcomes, axial alignment, bone healing, and complication rates in pediatric population. METHODS: This retrospective cohort study reviewed pediatric forearm fracture cases from January 2016 to December 2020 at Mouwasat Dammam Hospital, Saudi Arabia. The study included children aged 16 years and younger who were treated with elastic intramedullary nailing. Both clinical and radiological o

Comparative Outcomes of Intramedullary Nailing Versus Plate Fixation in the Management of Both-Bone Forearm Fractures.

BACKGROUND: Both-bone forearm fractures are common orthopedic injuries requiring surgical intervention to restore function and stability. Intramedullary nailing (IMN) and plate fixation (PF) are two widely used techniques for managing these fractures. MATERIALS AND METHODS: A total of 100 patients aged 18-50 years with diaphyseal fractures of the radius and ulna were included in this prospective study. Patients were randomly assigned to two groups: Group A (IMN, n = 50) and Group B (PF, n = 50). Surgical procedures were performed following standard protocols. Postoperative evaluations include

Return to Work Following Intramedullary Nailing of Lower-Limb Long-Bone Fractures in South Africa.

BACKGROUND: Injuries are one of the leading causes of global death and disability and commonly have substantial economic implications. The economic impact of injuries is particularly pronounced in low- and middle-income countries, where 90% of injuries occur. In this study, we aimed to assess return-to-work rates of individuals who sustained a lower-limb long-bone fracture in South Africa and to identify factors that influence the ability to return to employment. METHODS: This prospective cohort study was conducted across 2 tertiary trauma centers in Cape Town, South Africa. Adults who receiv

Micro-Structural and Biomechanical Evaluation of Bioresorbable and Conventional Bone Cements for Augmentation of the Proximal Femoral Nail.

Osteoporotic proximal femur fractures are on the rise due to demographic change. The most dominant surgical treatment option for per/subtrochanteric fractures is cephalomedullary nailing. As it has been shown to increase primary stability, cement augmentation has become increasingly popular in the treatment of osteoporotic per/subtrochanteric femur fractures. The ultimate goal is to achieve stable osteosynthesis, allowing for rapid full weight-bearing to reduce possible postoperative complications. In recent years, bioresorbable bone cements have been developed and are now mainly used to fill

Does the distal nail position impact the rates of misalignment and bone union in distal tibial fractures?

PURPOSE: This retrospective study aimed to assess the impact of distal nail placement on misalignment and healing rates in distal tibial fractures. METHODS: We reviewed all patients with distal tibial fracture treated with intramedullary nailing between 2015 and 2021, and a minimum follow-up of 12 months. Distal nail positioning was determined according to the Triantafillou zones. We related these positioning zones to misalignments (alignment ≥ 3°) and too bony union disorders (delayed union, non-union). RESULTS: Out of the 62 patients included, 56 (90.3%) show bone union without additional

Pitfalls of toxicological investigations in hair, bones, and nails in extensively decomposed bodies: illustration with two cases.

It is difficult to carry out toxicological investigations in biological samples collected from extensively decomposed bodies and to interpret obtained results as several pitfalls should be considered: redistribution phenomena, degradation of xenobiotics during the postmortem period, contamination by putrefaction fluids, and external contamination. This work aims to present two cases in order to illustrate and discuss these difficulties in this tricky situation. Case#1: the body of a 30-year-old woman was found in a wooded area (1 month after she has been reported missing by her family): hair a

The Use of a Free Fibular Strut as a "Biological Intramedullary Nail" for the Treatment of Complex Nonunion of Long Bones.

BACKGROUND: Nonunion of long-bone fractures is difficult to treat, especially when the bones are osteoporotic or there is a large bone gap as a result of repeated failure of the metallic nails or implants. In such cases, the use of an autologous intramedullary fibular strut graft may be a viable treatment option. METHODS: Twenty-two patients with a complex nonunion of the shaft of the femur, humerus, or tibia were managed with a free autologous fibular strut graft for intramedullary fixation with use of closed or open methods. All patients had evidence of moderate to severe local osteoporosis

Outcomes of Distal Femur Nonunions Treated With a Combined Nail/Plate Construct and Autogenous Bone Grafting.

In this study, we sought to retrospectively evaluate union and infection rates after treatment of distal femur nonunions using a combined nail/plate construct with autogenous bone grafting obtained from the ipsilateral femur using a reamer irrigator aspirator system. Ten (10) patients treated at a Level I trauma center for nonunion of a femoral fracture using a combined nail/plate construct from 2004 to 2014 were included in the study. Union rate and postoperative infection rates were recorded. Mean interval from index surgery to nonunion repair was 12 months (range 4-36 months). Follow-up at

A new intramedullary sustained dynamic compression nail for the treatment of long bone fractures: a biomechanical study.

OBJECTIVES: We developed a new intramedullary interlocking nail (sustained dynamic compressive nail; SDCN) which can sustain a continuous dynamic compressive force on the fracture edges to overcome implant failure, screw loosening and nonunion complications encountered in the traditionally used static intramedullary nailing (SIMN). MATERIALS AND METHODS: Six pairs of composite femurs were fixed with SIMN and SDCN. The axial compression, bending, torsional stiffness, and strain values were analyzed. RESULTS: The mean axial compression stiffness values were 427.1 N/mm for the SDCN and 217.8 N/

Reconstruction of segmental bone defects due to chronic osteomyelitis with use of an external fixator and an intramedullary nail.

BACKGROUND: Callus distraction over an intramedullary nail is a rarely used technique for the reconstruction of intercalary defects of the femur and tibia after radical débridement of chronic osteomyelitic foci. The aim of this study was to summarize our experience with distraction osteogenesis performed with an external fixator combined with an intramedullary nail for the treatment of bone defects and limb-shortening resulting from radical débridement of chronic osteomyelitis. METHODS: Thirteen patients who ranged in age from eighteen to sixty-three years underwent radical débridement to tre

[Comparative study of stability following the nailing of fractures of the femur shaft. An experimental study with cadaver bones].

Intramedullary osteosynthesis is preferred for shaft fractures of the long bones of the lower extremities because it generally results in early weight-bearing stability, allowing full function of the leg and rapid rehabilitation. Only transverse and short oblique fractures near the middle of the medullary cavity can be sufficiently stabilised using intramedullary nails alone. Additional stabilising aids such as cerclages or interlocking nails must be used for all other types of shaft fracture. There are no previous studies of stability that make a comparison between conventional intramedullary

Evolution of the concept of an extensible nail accommodating to normal longitudinal bone growth: clinical considerations and implications.

Based on animal experiments, we observe that the central portion of an epiphysis can be subjected to acute trauma in a low order of its cross-sectional area without interfering with growth. Moreover, the presence of a metallic device of small diameter placed across the center of the distal femoral epiphysis of a growing dog does not interfere with growth. A metal pin with a T-shaped end can be placed across the distal femoral epiphysis of a growing dog with the T-shaped end fixed in the epiphysis without interfering with growth. As growth occurs, migration of the spin or elongation of the femu

Elastic stable intramedullary nailing is the best treatment of unicameral bone cysts of the long bones in children?: Prospective long-term follow-up study.

Since Virchow first described the unicameral bone cyst (UBC) in 1876, several methods for management were proposed. Currently performed surgical procedures include curettage combined with bone grafting, allografting with freeze-dried crushed cortical bone, use of homologous cancellous bone chips, the application of high-porosity hydroxyapatite, and cryosurgery. Decompression with multiple drill holes and intralesional injections of either steroids or bone marrow have also been used to treat UBC, with variable success rates. Elastic stable intramedullary nailing for the treatment of UBC in long

The use of flexible nails in the treatment of paediatric long bone fractures: Experience at a level one paediatric trauma centre, a cohort study.

INTRODUCTION: Fractures occur in children at an incidence only surpassed by women >85 years and account for 25 % of paediatric injuries. Over the last three decades, there has been a trend towards operative management of children's fractures including utilisation of flexible nails as popularised by the Nancy group in the 1980s. Between 5 and 11 % of paediatric forearm fractures are now fixed in this manner with complication rates of 12-42 %. This study shares the experience of a paediatric level one major trauma centre using this technique in managing long bone fractures in children. METHODS
