Brain death and transcranial Doppler: experience in 130 cases of brain dead patients.

BACKGROUND AND PURPOSE: Diagnosis of brain death requires confirmation of the clinical diagnosis by appropriate tests, generally electroencephalography (EEG) and angiography. The diagnostic limitations or logistical problems inherent to these tests indicate the need to develop other more appropriate methods. The results obtained with transcranial Doppler (TCD) led us to conduct this prospective study of TCD recordings in brain dead patients. METHODS: 130 patients, aged 2-88 years were diagnosed as brain dead between July 1987 and June 1993. Clinical criteria were confirmed in all cases by EEG…

Distinct goal-directed perfusion signatures in donation after brain death versus circulatory death heart transplantation.

BACKGROUND: Continuous oxygen extraction ratio (O2ER) monitoring during cardiopulmonary bypass is an emerging strategy to support goal-directed perfusion (GDP) during heart transplantation (HT). Because donation after circulatory death (DCD) introduces additional warm ischemia, we examined whether post-reperfusion O2ER dynamics differ from donation after brain death (DBD). METHODS: Adult HT at a single center (10/2021-06/2025) with continuous perfusion data were retrospectively reviewed. O2ER was calculated as oxygen consumption/oxygen delivery index (DO2i). The primary endpoint was O2ER afte…

Donation After Brain Death: Are There Any Organizational Difficulties?

OBJECTIVES: In the past 3 years, nearly 90 % of organ transplants performed in our country have been from living donors. Low rates of organ donation and family consent in cases of brain death reduce the source of deceased donors. Here, we reported the challenges of determination of brain death and donor care processes in our hospital and offered our recommendations to strengthen these services. MATERIALS AND METHODS: We retrospectively analyzed adult patients (n = 5845 )treated in adult intensive care units of Manisa City Hospital from June 1, 2024, through November 30, 2025. Demographic info…

Guillain Barré Syndrome Mimicking Brain Death: A Case Report.

Here, we present a case of Guillain-Barré syndrome (GBS) that mimicked brain death. A 66-year-old lady with a medical history of breast cancer (now receiving hormone therapy), hypertension, and hypothyroidism, presented to the emergency department. The patient was admitted to the neuro ICU with absent brainstem and spinal cord responses, concerning for possible brain death. Further evaluation, however, identified the uncommon GBS with respiratory failure. This case emphasizes the need to recognize this severe manifestation of GBS since misinterpreting this as brain death might result in the di…

Methanol Poisoning Leading To Brain Death: a Case Report.

INTRODUCTION: The COVID-19 pandemic has put increased stress on medical systems, infrastructure, and the public in expected and unexpected w … mergent management of methanol poisoning. EEG and brain perfusion imaging were performed to confirm brain death, given concerns about the cranial nerve exam after methanol poisoning. CONCLUSIONS: While rare, methanol toxicity remains a potentially fatal poisoning in the United States and worldwide. When healthcare and public resources are strained, healthcar …

Toxicologic Confounders of Brain Death Determination: A Narrative Review.

The aim of this narrative review is to describe the toxicologic confounders of brain death currently reported in the literature to offer guidance for physicians assessing brain death after a toxic exposure. We established an a priori definition of a "brain death mimic" as an unresponsive, intubated patient missing some, but not all brainstem reflexes. We completed a review of the literature utilizing MEDLINE and EMBASE to find case reports of patients of all ages in English, French, and Spanish meeting the criteria and hand searched the references of the results. We recorded xenobiotic dose, d…

Brain Death Secondary to Rocky Mountain Spotted Fever Encephalitis.

A two-year-old female presented with acutely altered mental status following eight days of fever and rash. She had been camping at an Indiana campground 11 days prior to the onset of illness and was evaluated twice for her fever and rash prior to admission. Laboratory evaluation on admission revealed thrombocytopenia, hyponatremia, and elevated transaminases. The patient developed diffuse cerebral edema, and despite intensive care, the edema led to brain death from Rocky Mountain spotted fever (RMSF). We present this case to highlight the importance of considering RMSF and other tick-borne ill…

Addressing Religious or Cultural Opposition to Brain Death Diagnosis.

In 1968, the neurologic or "brain death" standard for declaration was introduced as an accepted standard for declaration of death in hospitals where heartbeat and breathing are being sustained by technology, but functions of the brain, including the brain stem, have ceased. For many people, this accepted ethical, legal, and medical definition of death by neurologic standards can seem to blur the line between life and death as the heart is still beating, the lungs are still moving air albeit by mechanical ventilation, and the body is still warm. As experts in end-of-life care, hospice and palli…

Direct peritoneal resuscitation reduces intestinal permeability after brain death.

BACKGROUND: The profound inflammatory response associated with brain death is frequently cited as the reason organs procured from brain dead donors are associated with worse graft function. The intestine releases inflammatory mediators in other types of shock, but its role is brain death has not been well-studied. Direct peritoneal resuscitation (DPR) improves visceral organ blood flow and reduces inflammation after hemorrhagic shock. We hypothesized that use of DPR would maintain intestinal integrity and reduce circulating inflammatory mediators after brain death. METHODS: Brain death was in…

Diagnosis of brain death: confirmatory tests after clinical test.

BACKGROUND: The brain death confirmation tests occupy a different position in each country's diagnostic criteria (or guideline); the choices of tests are also different. China brain death criteria include clinical judgment and confirmation tests. This study aimed to confirm the preferred confirmatory test and complementary confirmatory tests. METHODS: We did a clinical brain death determination on deep coma patients, and then divided them into brain death group and non-brain death group. According to the Chinese standards for determining brain death, both the groups accepted confirmatory test…

An unexpected ending: brain death following acute severe asthma.

A 56-year-old woman presented to our hospital with acute severe asthma. As a consequence of severe refractory airflow limitation, moderate hypercapnia ensued for several days. Unexpectedly, the patient died as a result of brain stem herniation, in the absence of hypoxaemia, arterial hypotension or an intracranial mass lesion. We discuss the mechanisms that may have precipitated severe intracranial hypertension resulting in brain death, and the possible methods to detect and avoid such a devastating consequence.

Hypoxia ischemia-mediated cell death in neonatal rat brain.

The examination of Bcl-2-associated X protein (Bax) protein's role in the activation of cognate nuclear, mitochondrial and ER cell death signaling cascades and the resulting effects on cell death phenotype in the brain after neonatal hypoxia-ischemia (HI) requires an understanding of neonatal HI insult and progression, as well as, its dysfunctional outcomes. In addition, knowledge of key concepts of oxidative stress, a major injurious component of HI, and the different cell death phenotypes (i.e. apoptosis and necrosis) will aid the design of appropriate useful experimental paradigms. Here we

Variability in hospital-based brain death guidelines in Canada.

PURPOSE: Variability has been reported in the practices to determine death by neurological criteria for adults and children. The objective of this study was to determine if this variability exists in the Canadian context. METHODS: A cross-sectional survey of the Canadian intensive care units (ICUs) involved in the care of potential organ donors, and Canadian organ procurement organizations (OPOs) was undertaken. We contacted the medical directors of these units and asked them to provide their guidelines for the neurological determination of death (NDD). A framework, which identifies key diagn

Sudden death resulting from epidermoid cyst of the brain.

Epidermoid cysts of the brain are rare tumor-like lesions, most often of maldevelopmental origin. They are benign in nature, causing symptoms depending on their localization. Surgical resection leads to excellent results. A case is reported of a 68-year-old-man who died suddenly and unexpectedly. Postmortem examination revealed signs of central dysregulation and a bifrontal epidermoid cyst. To the authors' knowledge, death resulting from epidermoid cyst of the brain has not been reported so far.

Myocardial performance after brain death: studies in isolated hearts.

OBJECTIVES AND METHODS: Brain death related hemodynamic instability and/or cardiac dysfunction is frequently described in the potential organ donor which may lead to exclusion of the heart from transplantation. The underlying mechanisms are controversely discussed. Therefore, in the present study, potential brain death associated cardiodepressant factors were evaluated separately in cross-circulated canine heart models. Brain death was induced by inflation of a subdural balloon catheter. Loading conditions and coronary perfusion pressure were kept identical in all cross-circulated hearts throu…

Miller Fisher syndrome, a brainstem encephalitis, mimics brain death.

Miller Fisher syndrome is a rare focal encephalitis of the brainstem characterized by cranial nerve palsy, ataxia, and areflexia. While this condition usually is mild and self-limiting, as this case documents, findings may fulfill many of the criteria in the clinical examination for brain death. Unlike brain death, however, it is not associated with irreparable structural damage of the brain or, as is mandatory, does it fulfill a very important prerequisite of brain death, the exclusion of potentially reversible causes of coma. Treatment of Miller Fisher syndrome is supportive.
