Volumetric Breast Density Estimation From Three-Dimensional Reconstructed Digital Breast Tomosynthesis Images Using Deep Learning.

PURPOSE: Breast density is a widely established independent breast cancer risk factor. With the increasing utilization of digital breast tomosynthesis (DBT) in breast cancer screening, there is an opportunity to estimate volumetric breast density (VBD) routinely. However, current available methods extrapolate VBD from two-dimensional (2D) images acquired using DBT and/or depend on the existence of raw DBT data, which is rarely archived by clinical centers because of storage constraints. METHODS: We retrospectively analyzed 1,080 nonactionable three-dimensional (3D) reconstructed DBT screening…

Effect of Mammographic Screening Modality on Breast Density Assessment: Digital Mammography versus Digital Breast Tomosynthesis.

Background Breast Imaging Reporting and Data System (BI-RADS) breast density categories assigned by interpreting radiologists often influence decisions surrounding supplemental breast cancer screening and risk assessment. The landscape of mammographic screening continuously evolves, and different mammographic screening modalities may result in different perception of density, reflected in different assignment of BI-RADS density categories. Purpose To investigate the effect of screening mammography modality on BI-RADS breast density assessments. Materials and Methods Data were retrospectively a…

Between-Race Differences in Supplemental Breast Cancer Screening Before and After Breast Density Notification Law.

Guidelines recommend supplemental breast cancer screening for women at increased breast cancer risk; however, the passage of breast density notification laws may lead to supplemental screening that is incongruent with women's risk. We examined supplemental screening (ie, MRI, ultrasound, or tomosynthesis within 6 months of screening mammogram) among a sample of 2,764 African American (AA) and 691 European American (EA) women with negative or benign screening mammograms for whom we had data from both before and after implementation of breast density notification laws in the state of Michigan. Breast cancer and bone mineral density: the Marburg Breast Cancer and Osteoporosis Trial (MABOT II).

OBJECTIVES: The current case-control study is the first to examine the relationship between bone mineral density (BMD) measured by dual-energy X-ray absorptiometry (DXA) and quantitative ultrasonometry (QUS) in pre- as well as postmenopausal women with breast cancer compared to healthy matched controls. METHODS: Among 1422 women (premenopausal, n = 238, postmenopausal, n = 1184), BMD and QUS were measured. In total, 541 of the women had an incident diagnosis of breast cancer (122 premenopausal, 419 postmenopausal) without prior breast cancer treatment. Because of significant intergroup differ

Digital mammographic density and breast cancer risk: a case-control study of six alternative density assessment methods.

INTRODUCTION: Mammographic density is a strong breast cancer risk factor and a major determinant of screening sensitivity. However, there is currently no validated estimation method for full-field digital mammography (FFDM). METHODS: The performance of three area-based approaches (BI-RADS, the semi-automated Cumulus, and the fully-automated ImageJ-based approach) and three fully-automated volumetric methods (Volpara, Quantra and single energy x-ray absorptiometry (SXA)) were assessed in 3168 FFDM images from 414 cases and 685 controls. Linear regression models were used to assess associations

Evaluating federated learning approaches for mammography under breast density heterogeneity.

PURPOSE: Breast density is a key factor that influences mammography interpretation and is a major source of heterogeneity in multicenter datasets. Such heterogeneity poses challenges for collaborative machine learning across institutions, particularly in federated learning (FL). We aim to evaluate the impact of breast density-induced heterogeneity on FL for mammography image classification and to assess the robustness of common FL algorithms in realistic clinical settings. APPROACH: We conducted experiments on mammography datasets under two scenarios: (1) a strongly heterogeneous setting wher…

Breast density classification using frequency-based features in microwave imaging.

Breast cancer remains one of the leading causes of death among women worldwide. One major challenge in early and accurate detection is breast density. High breast density not only obscures tumors on current imaging modalities, making them harder to identify, but also significantly increases the likelihood of diagnostic errors, both by medical professionals and automated detection systems. As a result, accurately classifying the breast density is crucial, and can lead to better, more tailored screening approaches and reduce the chances of error. This is especially critical for younger women, wh…

Steroid hormone metabolites and mammographic breast density in premenopausal women.

BACKGROUND: Steroid hormones influence breast morphology and cellular proliferation and are associated with breast carcinogenesis. However, their associations with mammographic breast density (MBD) are less studied, particularly in premenopausal women. We, therefore, investigated the associations of steroid hormone metabolites with MBD in premenopausal women. METHODS: Our study included 700 premenopausal women scheduled for screening mammograms. We analyzed 54 steroid hormone metabolites (Metabolon®) and assessed volumetric measures of MBD including volumetric percent density (VPD), dense vol…

Australian Women's Responses to Breast Density Information: A Content Analysis.

UNLABELLED: Breast density (BD) is an independent risk factor for breast cancer and reduces mammographic sensitivity. This study explored women's responses and intentions if notified that they had dense breasts. METHODS: Content analysis was used to assess responses from a written questionnaire undertaken in conjunction with focus groups on BD involving 78 Australian women aged 40-74. RESULTS: Half the women reported that they would feel a little anxious if notified they had dense breasts, while 29.5% would not feel anxious. The most common theme (29.5%) related to anxiety was the psychosoci…

Understanding Clinical Mammographic Breast Density Assessment: a Deep Learning Perspective.

Mammographic breast density has been established as an independent risk marker for developing breast cancer. Breast density assessment is a routine clinical need in breast cancer screening and current standard is using the Breast Imaging and Reporting Data System (BI-RADS) criteria including four qualitative categories (i.e., fatty, scattered density, heterogeneously dense, or extremely dense). In each mammogram examination, a breast is typically imaged with two different views, i.e., the mediolateral oblique (MLO) view and cranial caudal (CC) view. The BI-RADS-based breast density assessment …

Vision 20/20: Mammographic breast density and its clinical applications.

Breast density is a strong predictor of the failure of mammography screening to detect breast cancer and is a strong predictor of the risk of developing breast cancer. The many imaging options that are now available for imaging dense breasts show great promise, but there is still the question of determining which women are "dense" and what imaging modality is suitable for individual women. To date, mammographic breast density has been classified according to the Breast Imaging-Reporting and Data System (BI-RADS) categories from visual assessment, but this is known to be very subjective. Despit…

Enhancement of mammographic density measures in breast cancer risk prediction.

BACKGROUND: Mammographic density is a strong risk factor for breast cancer. METHODS: We present a novel approach to enhance area density measures that takes advantage of the relative density of the pectoral muscle that appears in lateral mammographic views. We hypothesized that the grey scale of film mammograms is normalized to volume breast density but not pectoral density and thus pectoral density becomes an independent marker of volumetric density. RESULTS: From analysis of data from a Swedish case-control study (1,286 breast cancer cases and 1,391 control subjects, ages 50-75 years), we …

Statistical evaluation of a fully automated mammographic breast density algorithm.

Visual assessments of mammographic breast density by radiologists are used in clinical practice; however, these assessments have shown weaker associations with breast cancer risk than area-based, quantitative methods. The purpose of this study is to present a statistical evaluation of a fully automated, area-based mammographic density measurement algorithm. Five radiologists estimated density in 5% increments for 138 "For Presentation" single MLO views; the median of the radiologists' estimates was used as the reference standard. Agreement amongst radiologists was excellent, ICC = 0.884, 95% C…

Acculturation and breast density in foreign-born, U.S. Chinese women.

The role of acculturation in the breast cancer risk increase among U.S. Chinese women is unclear. We examined the association between acculturation and breast density in a sample of foreign-born, U.S. Chinese women and examined factors that may explain such an association. Between January 2002 and May 2003, 212 Chinese women were recruited from Philadelphia region screening programs. Cranial-caudal mammographic images were classified into one of four categories ranging from "entirely fatty" to "extremely dense." Questionnaires assessed information on sociodemographic, cultural, reproductive, a…

Assessing breast density using the chemical-shift encoding-based proton density fat fraction in 3-T MRI.

OBJECTIVES: There is a clinical need for a non-ionizing, quantitative assessment of breast density, as one of the strongest independent risk factors for breast cancer. This study aims to establish proton density fat fraction (PDFF) as a quantitative biomarker for fat tissue concentration in breast MRI and correlate mean breast PDFF to mammography. METHODS: In this retrospective study, 193 women were routinely subjected to 3-T MRI using a six-echo chemical shift encoding-based water-fat sequence. Water-fat separation was based on a signal model accounting for a single T2* decay and a pre-calib…
