Forty-one infants with acute viral bronchiolitis were hospitalized in our paediatric intensive care unit during the seven year period from 1980 to 1987. In 14 out of 27 evaluated patients, Respiratory Syncitial Virus (RSV) was detected in the nasal secretions. Twenty-three children required only supportive care and monitoring. Eighteen infants had to be ventilated because of respiratory failure. The major indication for mechanical ventilation was an arterial or capillary pCO2 of more than 64 mmHg; other criteria were repeated apnoea, respiratory acidosis, and clinical deterioration. In all cas…

Among 173 consecutive open lung biopsies, nine gave a histopathological diagnosis of bronchiolitis. Seven of these patients had some connective tissue disorder (CTD), six of whom are presented in this report; two had classical and one possible rheumatoid arthritis (RA), one ankylosing spondylitis, one scleroderma, and one developed classical RA four years after biopsy. Four of the patients were smokers, most suffered from breathlessness and cough. In terms of lung function three patients had obstruction, one both restriction and obstruction and three a decreased diffusion capacity. For control…

Our experience concerns 36 infants ventilated for bronchiolitis over the last five years. Among the infants admitted in the pediatric department during the same period 11% have been ventilated. Their age ranged between 1.5 and 49.5 weeks (mean 9.2 weeks) and their weight between 2,700 and 8,000 g (mean 3,841 g). Fourteen were premature born infants. 16 patients have been intubated for apneas, 13 for clinical deterioration and 10 for hypercarbia. The duration of ventilation ranged between 1 and 18 days (mean 6.5 days). No death occurred. Neither neurological nor respiratory sequelae could be re…

A patient in whom a severe systemic illness was characterised by weight loss, arthralgia and breathlessness was exposed to her pet bird. This pragmatic approach reproduced the features of her illness. The changes that occurred in her pulmonary physiology and histology differed from those seen in extrinsic allergic alveolitis in many important respects.

Bronchiolitis obliterans is an anatomo-clinical entity which may, in young pediatric patients, follow severe viral infections, especially due to measles virus and adenoviruses. After a transient remission, the disease progresses to chronic respiratory failure with dyspneic paroxysms. Diagnosis is based on different investigations which confirm the absence of proximal bronchiolar obstruction and the presence of bronchiolar obliteration. Deficient pulmonary perfusion due to ventilatory defects are demonstrated by pulmonary scintigraphy and even better by digital subtraction pulmonary angiography…

Seasonal respiratory syncytial virus (RSV) lower respiratory infections (RSV-LRI) and bronchiolitis in children remain a high burden to medical facilities. Studies evaluating the practical approach in outpatient settings are scarce. We conducted a survey to provide an insight into management in pediatric offices in Germany. Invitations for participation in an online survey were sent to 623 providers of pediatric primary care via email. Recipients were randomly chosen from national databanks based on zip codes. The response rate was 16,2%, n=92 (14,7%) questionnaires were fully answered and ana…

BACKGROUND: Respiratory syncytial virus (RSV) is a prevalent cause of lower respiratory tract infections. It may be associated with hepatocellular involvement, as indicated by increased liver enzymes aspartate aminotransferase and alanine transaminase (ALT).
AIM: To evaluate the rate of increased liver enzyme levels in children with acute bronchiolitis and correlate them with clinical, laboratory, and radiological variables.
METHODS: The study was a retrospective review of the medical records of children who presented with acute bronchiolitis when admitted to the Pediatric Department, Salman…

Acute viral bronchiolitis causes significant mortality in the developing world, is the number one cause of infant hospitalisation in the developed world, and is associated with the later development of chronic lung diseases such as asthma. A vaccine against respiratory syncytial virus (RSV), the leading cause of viral bronchiolitis in infancy, remains elusive, and hence new therapeutic modalities are needed to limit disease severity. However, much remains unknown about the underlying pathogenic mechanisms. Neutrophilic inflammation is the predominant phenotype observed in infants with both mil…

Bronchiolitis is a lower respiratory tract illness caused by viral infection in children 2 years of age and younger, frequently associated with wheezing on physical examination. It is a common cause of hospitalization, particularly in patients with risk factors for more serious disease. The diagnosis can be made based on clinical signs and symptoms alone, and care is generally supportive with a focus on safely doing less for symptomatic children. Bronchodilators, systemic steroids, and other therapies have been shown to have no significant effect on hospitalization rates, length of stay, or sy…

As the bronchioles have a strategic position between the airways and the alveolar structures, they are at a site where disorders of many origins may develop, including infections, inflammatory and/or fibrosing processes of immune, occupational, environmental, tumoral, and iatrogenic origin, which may result in predominant bronchiolitis and/or organizing pneumonia. This etiologic variety results in many distinct entities and syndromes, common or rare, with new or renewed faces such as bronchiolocentric interstitial pneumonia or organizing pneumonia primed by radiation to the breast.

BACKGROUND: Bronchiolitis is the leading cause of hospitalisation among infants in high-income countries. Acute viral bronchiolitis is associated with airway obstruction and turbulent gas flow. Heliox, a mixture of oxygen and the inert gas helium, may improve gas flow through high-resistance airways and decrease the work of breathing. In this review, we selected trials that objectively assessed the effect of the addition of heliox to standard medical care for acute bronchiolitis.
OBJECTIVES: To assess heliox inhalation therapy in addition to standard medical care for acute bronchiolitis in in…

Follicular bronchiolitis (FB) is a benign progressive lung disease. It is characterized with lymphoplasmocellular infiltration and hyperplastic follicles in the peribronchial areas in the small airways. Follicular bronchiolitis should be considered in cases where chronic cough, recurrent upper respiratory tract infections and progressive dyspnea are observed in children. The diagnosis should be supported by lung biopsy. A 8-year old female patient presented to our hospital with complaints including continuing cough and wheezing. Bilateral extensive rales and rhonchi in the lungs were heard on …

BACKGROUND: Bronchiolitis obliterans is a chronic obstructive lung disease that follows a severe insult to the lower respiratory tract. Severity and outcome are different depending on the geographic area. The aim of this study was to communicate the results of our sample.
METHODS: A retrospective study was performed in a sample of boys and girls with a medical diagnosis of bronchiolitis obliterans (BO) between December 1994 and February 2011. The diagnosis was made by high-resolution computed tomography (HRCT) scan and lung function. Lung biopsy was performed in one case. Pulmonary function t…

Bronchiolitis obliterans (BO) is an infrequent chronic obstructive pulmonary disease that follows an insult to lower respiratory tract. BO following a severe lung infectious disease is the most common form reported in children. It implies a chronic necrotizing and ultimately fibrosing process affecting the small airways which results in progressive obliteration with resultant obstructive lung disease. The characteristic symptom-complex includes dyspnea, chronic cough, sputum production and wheezing. Although histopathology remains the gold standard for diagnosis, computed tomographic scan is o…

We present a case of benign transient hyperphosphatasemia in a 4-month-old infant with acute bronchiolitis and pneumonia. During hospitalization the infant had an increased catalytic activity of alkaline phosphatase (ALP): day 2, 5074 U/L; day 3, 5622-U/L; and day 8, 3129 U/L. The x-ray, leukocytosis, and C-reactive protein findings pointed to bacterial etiology of the respiratory disorder. Electrophoretic separation revealed an atypical isoenzyme profile: fast anodal, near-cathodal and bone fractions. ALP levels normalized within 54 days, and control electrophoresis indicated normal liver, pl…
