The course and outcome of bulimia nervosa.

Although knowledge of the long-term course and outcome of bulimia nervosa is critical to the effective management of the bulimic patient, research in this area is still in its infancy. Comparisons between existing outcome studies are complicated by differences in research design and methods, a lingering confusion over the definition of terms such as recovery and relapse, and the possibility of multiple forms of bulimia nervosa with varying outcomes. Nonetheless, these studies agree that bulimia nervosa is a chronic disorder, characterized by high rates of relapse and persistent subclinical sym…

The genetic epidemiology of bulimia nervosa.

OBJECTIVE: The authors seek to clarify, from both an epidemiologic and genetic perspective, the major risk factors for bulimia nervosa and to understand the relationship between narrowly defined bulimia and bulimia-like syndromes. METHOD: Personal structured psychiatric interviews were conducted with 2,163 female twins from a population-based register. Psychiatric disorders were assessed using DSM-III-R criteria. RESULTS: Lifetime prevalence and risk for narrowly defined bulimia were 2.8% and 4.2%, respectively. Including bulimia-like syndromes increased these estimates to 5.7% and 8.0%, res…

Drugs in the treatment of bulimia nervosa.

The neuro-transmitter serotonin seems to be important in the treatment of disturbed eating behaviour. In Anorexia Nervosa (AN) a serotonin antagonist has been proposed, whereas in Bulimia Nervosa (BN) serotonin agonists have been used with success, e.g. fenfluramine. A new generation of antidepressants has been introduced. that selectively have a serotonergic effect. The previous tricyclic and particularly the tetracyclic antidepressants had a noradrenergic effect as well. Fluoxetine belongs to the new generation. A total of 30 females with BN were treated with fluoxetine in an open study. Cli…

[Nutritional status and immunocompetence in bulimia].

Due to the fact that malnutrition gives rise to a decrease in immunocompetence and that this parameter is used in turn to assess the patient's nutritional state, certain aspects of cell-mediated immunity were studied in a group of female patients suffering from atypical malnutrition (bulimia nervosa) and compared with those of healthy young female controls in order to establish their nutritional condition. Several anthropometric and hematological parameters were determined in the patient and healthy groups: weight, height, body mass index (BMI), WBC, blood lymphocyte count, lymphocyte subsets:…

Studies of the epidemiology of bulimia nervosa.

Research on the epidemiology of bulimia nervosa has focused largely on the prevalence of the disorder. As methods have improved, consensus has increased regarding the prevalence rate among adolescent and young adult women--about 1%. However, the accuracy of this figure and its clinical significance must be questioned. In this synthesis of the epidemiological work to date, the authors review the literature from a clinical and research perspective. They recommend a shift in emphasis away from studies of the distribution of the disorder toward studies of the determinants of the whole spectrum of …

Fluoxetine as a treatment for bulimia nervosa.

The results of a small study using fluoxetine in the treatment of bulimia nervosa are presented. Ten subjects were treated on an open basis with fluoxetine 60-80 mg daily. Seven subjects stopped their bulimic behaviour completely, two improved and one was unchanged. The results indicate that fluoxetine may have a role in the treatment of bulimia and that further investigation is warranted. A brief review of other drug studies on bulimia nervosa is presented.

The clinical features of bulimia nervosa.

The clinical features of 35 patients with bulimia nervosa are described. All the patients came from the Oxford area and each fulfilled conservative operational diagnostic criteria. Standardised assessment procedures were used including a structured interview designed to assess the psychopathology characteristic of patients with eating disorders. It was confirmed that these patients have grossly disturbed eating habits accompanied by morbid beliefs and values concerning their shape and weight. Although the majority had a weight within the normal range, a history of weight disturbance was common…

The development of satiation in bulimia nervosa.

Bulimia nervosa (BN) is characterized by the recurrent consumption of excessive amounts of food (binge eating) followed by inappropriate compensatory behaviors. A leading hypothesis is that the persistence of BN may be due, in part, to a disturbance in the development of satiation. Because patients with BN consume larger meals than controls, previous studies have not been able to directly compare the development of satiation. In order to address this problem, subjects consumed large meals of predetermined size without knowing when they would be stopped. Twenty-one women with BN and 13 control …

[On the psychodynamics of an adolescent bulimia patient].

The aim is to understand the meaning and significance of the symptoms by means of psychoanalytical theoretical models and to give an insight into the functioning of analytical therapy, using the example of an adolescent bulimia patient. In this therapy, the cause of the bulimic illness is seen as being a failure to build an intrapsychic space, and it is shown how triangulation can be achieved via the three-dimensional analytical relationship. Particular attention is given to the theories of M. Klein and W. R. Bion. The theory of S. Freud is based upon the portrayal of bulimic symptoms as the e…

Threat processing in women with bulimia.

Cognitive-behavioral treatments are the most commonly used approaches for bulimia nervosa. However, these treatments tend to produce only moderate remission rates. Therefore, it is useful to consider how experimental cognitive approaches can help to revise current cognitive-behavioral models of bulimia. Information processing models highlight the role of attentional biases and schema avoidance in the etiology and maintenance of pathology. This review considers how these processes relate to bulimia. In particular, a growing body of evidence suggests that bulimic behaviors may function to "block…

[Hypertrophy of the salivary glands in bulimia].

We report a case of bulimia nervosa with bilateral swelling of the parotid and submandibular glands. It was the only symptom of the behavioral disorder. Sjögren's disease was excluded histologically. In our case, the parotomegaly as a symptom of sialadenosis was found at the same time as the eating disorder. MRI or ultrasound are important diagnostic tools to confirm the diagnosis.

Psychotherapy for bulimia nervosa and binging.

BACKGROUND: Bulimia nervosa and like syndromes, such as binge eating disorder, are common in young Western women. A specific manual based psychotherapy, cognitive behaviour therapy (CBT) has been developed for the treatment of bulimia nervosa (CBT-BN). Other psychotherapies, some from a different theoretical framework, and some modifications of CBT are also used. OBJECTIVES: To evaluate the efficacy of psychotherapeutic treatments for those with binge eating syndromes, that have been tested in randomised controlled trials. The efficacy of CBT in the specific treatment of bulimia nervosa and b…

Impact of pregnancy on bulimia nervosa.

BACKGROUND: Bulimia nervosa affects women at a peak age of reproductive functioning, but few studies have examined the impact of pregnancy on bulimia. AIM: To examine the impact of pregnancy on symptoms of bulimia nervosa and associated psychopathology. METHOD: Women actively suffering from bulimia nervosa during pregnancy (n = 94) were interviewed using the eating disorder examination (12th edn) and structured clinical interview for DSM-III-R, with additional structured questions. Behaviours were recorded at conception, each trimester and postnatally. Relative risks were calculated for prog…

The epidemiology and classification of bulimia nervosa.

BACKGROUND: We sought to determine whether there was empirical support for the diagnostic thresholds of DSM-IV bulimia nervosa (BN) and whether an empirically derived typology resembled the diagnostic categories of DSM-IV. METHODS: Detailed information about bulimic behaviours were assessed via personal interview in a population-based sample of 1897 Caucasian female twins. We assessed the lifetime prevalence of the component bulimic behaviours and DSM-IV and DSM-III-R BN. Latent class analysis of nine separate bulimic symptoms was used to develop an empirical typology of bulimic behaviour. R…

Diagnosis and treatment of bulimia nervosa.

Bulimia nervosa often has an obscure presentation that requires a high level of suspicion by physicians. Awareness of subtle signs and knowledge of important questions to pursue are critical to a physician's ability to diagnose this disorder. Since bulimia nervosa may have several comorbid psychiatric disorders, such as depression, substance abuse, and personality disorders, it is important to refer patients for further evaluation and treatment. The treatment of bulimia nervosa is comprehensive and individualized and may include cognitive-behavioral therapy, group therapy, family therapy, indi…

Earliest recollections in anorexia and bulimia.

Earliest Recollections (ER) are very useful as they reveal information about a person's main aims and lifestyle, according to the Individual Psychology assumption of the inner coherence of the personality. This paper looks at the utility of analyzing ER in psychopathological research, diagnosis and therapy. Its purpose is to compare particular ER in a group of anorectic outpatients, a group of bulimic outpatients and in a control group. The results provide further evidence that ER are a useful instrument to identify various aspects of family dynamics, particular pathological nuclei which are o
