We describe a case about a 79-year-old woman with rheumatoid arthritis with chronic left olecranon bursitis refractory to conventional therapy. Following appropriate diagnostic workup, Prototheca spp alga was identified as the causative agent. The exceptional nature of the case is emphasized.

BACKGROUND: Bursitis is the inflammation of a synovial bursa, a small synovial fluid-filled sac that acts as a cushion between muscles, tendons, and bones. Further, calcific bursitis results from calcium deposits on the synovial joint that exacerbates pain and swelling. The Gruberi bursa is located dorsolaterally in the ankle, between the extensor digitorium longus and the talus. Despite limited literature on its pathophysiology, the aim of this case is to discuss the bursa's association with calcific bursitis and its management via a case presented to our clinic.
CASE PRESENTATION: A 47-year…

Intermetatarsal bursitis (IMB) is an inflammation of the intermetatarsal bursas. The condition causes forefoot pain with symptoms similar to those of Morton's neuroma (MN). Some studies suggest that IMB is a contributing factor to the development of MN, while others describe the condition as a differential diagnosis. Among patients with rheumatic diseases, IMB is frequent, but the scope is yet to be understood. The aim of this paper was to investigate the diagnostic considerations of IMB and its role in metatarsalgia by a systematic review approach. We identified studies about IMB by searching…

He recovered with surgical debridement and antimicrobial therapy. This is the first case of C. ulcerans causing prepatellar bursitis with an abscess as per medical literature review.

Background. The prepatellar bursa can become inflamed owing to repeated trauma. Prepatellar bursitis is extremely rare in children. Methods. We report the case of an 8-year-old boy who was treated for an erythematous, swollen, and severely painful right knee, fever, inability to bear weight on the leg, and purulent material draining from a puncture wound. We describe the differential diagnosis for tender swollen knee, including infection, gout, rheumatoid arthritis, and osteoarthritis. If untreated, prepatellar bursitis can progress to patellar osteomyelitis. Results. Wound cultures grew Strep…

OBJECTIVE: Trochanteric bursitis (TB) is a self-limiting disorder in the majority of patients and typically responds to conservative measures. However, multiple courses of nonoperative treatment or surgical intervention may be necessary in refractory cases. The purpose of this systematic review was to evaluate the efficacy of the treatment of TB.
DATA SOURCES: A literature search in the PubMed, MEDLINE, CINAHL, and ISI Web of Knowledge databases was performed for all English language studies up to April 2010. Terms combined in a Boolean search were greater trochanteric pain syndrome, trochant…

BACKGROUND: Cutaneous infections due to Mycobacterium intracellulare are infrequent. We report a case of bursitis in an immunocompetent patient.
CASE-REPORT: A 80-year-old patient with a history of tuberculosis reported swelling of the elbow for six months with effusion due to olecranon bursitis, progressing to an abscess and skin ulceration. Examination of the synovial fluid revealed the presence of M. intracellulare. Histologic investigations showed epithelioid and gigantocellular granulations. Screening for immunodepression was negative. Treatment with clarithromycin and rifabutin proved e…

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2002eng
The use of arthroscopy to treat recalcitrant trochanteric bursitis is reported and its role in treating this common clinical entity is discussed. During a 7-year period, 27 patients underwent this procedure. All patients complained of recurrent lateral hip pain despite at least 2 localized injections with corticosteroids. Minimum follow-up was 1 year, and the cases of 22 patients were reviewed after 5 years. Twenty-three of the 27 patients had good or excellent results immediately following the procedure and experienced no complications. At 1 year, only 1 patient had experienced symptom recurr…

A 44-year-old woman was admitted to the authors' institution for evaluation of two masses in the right forefoot. Standard radiographs showed foci of calcification within the mass. Magnetic resonance imaging and macroscopic findings during surgery revealed the lesion was composed mainly of two different portions: dorsal cystic masses and a solid mass in the plantar side of the fifth metatarsal head. Histologic and immunohistochemical examinations showed that the former was an intermetatarsophalangeal bursitis induced by keratinous material, whereas the latter was a ruptured epidermal cyst. To t…

We describe a 59-year-old woman with diabetes and chronic asthma treated with prednisone and methotrexate who developed chronic olecranon bursitis caused by Candida lusitaniae. Infection, especially with unusual microbial pathogens, should be considered in cases of chronic bursitis in patients taking immunosuppressive medicine, even if the classic signs of septic bursitis are absent. Infection with C. lusitaniae, a component of the normal mycoflora, may be a marker of serious immunosuppression, as this patient ultimately died of a Pneumocystis carinii infection.

Bursitis is a periarticular rheumatism which occurs as a result of trauma, infection, metabolic disease or rheumatoid arthritis. The disease is usually manifested as a solitary lesion except in a few reported cases of multiple lesions. We describe here a case of bursitis in a 58-year-old woman with multifocal cystic lesions containing sterile bloody or yellowish fluid in the four extremities. Serological abnormalities were also noted, suggesting the presence of rheumatoid arthritis or another autoimmune connective tissue disease.

A woman with rheumatoid arthritis developed persistent sterile drainage from a cutaneous fistula after biopsy of an inflamed supraclavicular mass. Radiographs showed several cavities in the underlying clavicle. Inability to culture a pathogen and failure of the fistula to heal despite empirical courses of antibiotic therapy led to surgical intervention. The final diagnosis, based on careful histological analysis by special staining techniques, was rheumatoid bursitis extending into the clavicle and to the skin surface.

Two cases of extraarticular Group G streptococcal infection are reported: one patient suffered from tenosynovitis of the extensor digitorum communis tendons of a wrist and the other from a prepatellar bursitis. In both cases penicillin therapy rapidly cleared all signs of infection.

An operation for chronic olecranon bursitis is described and the results of 11 cases reviewed. It is suggested that a number of patients with this condition have a prominent olecranon process or spur. The technique involves excision of the olecranon process only, the bursa itself is preserved. The overlying skin remains undamaged and this manoeuvre avoids the unpleasant sequelae that may follow its removal. The operation appears to give satisfactory results.

Prepatellar bursitis is a common cause of anterior knee pain. We present an unusual case of massive bilateral traumatic haemorrhagic prepatellar bursitis managed with surgical excision and describe our operative findings. The patient presented with large bilateral knee swellings which had been present for six years following a fall onto both knees. The swellings interfered with his ability to wear trousers and lean up against ladders. Excision of the prepatellar swellings was performed sequentially. A thick-walled, well-demarcated cyst was excised from each knee, which was dissected from the s…
