Beyond bus fare: deconstructing prenatal care travel among low-income urban mothers through a mix methods GIS study.

OBJECTIVE: Guided by critical theory, this study illustrates the value of interpretative mapping to deconstruct bus travel to publicly funded prenatal care in a city marked by health and social inequities. METHODS: This mixed methods study used GIS maps based on 61,305 births to study the known barrier of transportation to prenatal care among urban mothers most at risk for preterm birth. RESULTS: Among 350 census tracts, 36 census tracts had preterm rates between 25 -36.9%. Modeling travel time for the case vignette for routine prenatal care took 21 visits to different geographically located

Trends in socioeconomic inequalities in mortality in small areas of 33 Spanish cities.

BACKGROUND: In Spain, several ecological studies have analyzed trends in socioeconomic inequalities in mortality from all causes in urban areas over time. However, the results of these studies are quite heterogeneous finding, in general, that inequalities decreased, or remained stable. Therefore, the objectives of this study are: (1) to identify trends in geographical inequalities in all-cause mortality in the census tracts of 33 Spanish cities between the two periods 1996-1998 and 2005-2007; (2) to analyse trends in the relationship between these geographical inequalities and socioeconomic de

Association between walkability and diverse health metrics: moderating role of social vulnerability.

Little research has considered associations between walkability and multiple health outcomes and the moderating effect of social determinants of health. This study examined relationships between walkability and diverse health issues (depression, poor mental health, poor physical health, mobility disability, obesity) and no leisure-time physical activity and whether these relationships were moderated by social vulnerability. Then, we assessed whether these moderated effects varied between urbanicity. For 1089 census tracts in South Carolina, we compiled EPA National Walkability Index, 6 health

Place Still Matters: Racial/Ethnic and Geographic Disparities in HIV Transmission and Disease Burden.

Neighborhood-level structural interventions are needed to address HIV/AIDS in highly affected areas. To develop these interventions, we need a better understanding of contextual factors that drive the pandemic. We used multinomial logistic regression models to examine the relationship between census tract of current residence and mode of HIV transmission among HIV-positive cases. Compared to the predominantly white high HIV prevalence tract, both the predominantly black high and low HIV prevalence tracts had greater odds of transmission via injection drug use and heterosexual contact than male

Income-driven socioeconomic status and presenting illness severity in children with acute respiratory failure.

Children living in low socioeconomic communities are vulnerable to poor health outcomes, especially when critically ill. The purpose of this study was to investigate the association between socioeconomic status (SES) and illness severity upon pediatric intensive care unit (PICU) admission in children with acute respiratory failure. This secondary analysis of the multicenter Randomized Evaluation of Sedation Titration for Respiratory Failure clinical trial includes children, 2 weeks to 17 years old, mechanically ventilated for acute respiratory failure; specifically, subjects who had parental c

Demographic analysis of childhood pedestrian injuries.

Factors in a child's living environment and socioeconomic background that contribute to the risk of pedestrian injury were studied. In 1982, in the city of Memphis, there were 210 pedestrian injuries among children aged 0 to 14 years, a rate of 138/100,000 children. The injured child was most often male, with mean age of 7.3 years; the child was usually struck while crossing the street between intersections, most commonly during the hours from 2 to 7 PM. Pedestrian injuries occurred in 81 of the 142 census tracts in the city. Compared with census tracts without reported injuries, these tracts

Ambient air pollution and COVID-19 incidence during four 2020-2021 case surges.

BACKGROUND: Air pollution exposure may make people more vulnerable to COVID-19 infection. However, previous studies in this area mostly focused on infection before May 2020 and long-term exposure. OBJECTIVE: To assess both long-term and short-term exposure to air pollution and COVID-19 incidence across four case surges from 03/1/2020 to 02/28/2021. METHODS: The cohort included 4.6 million members from a large integrated health care system in southern California with comprehensive electronic medical records (EMR). COVID-19 cases were identified from EMR. Incidence of COVID-19 was computed at

Neighborhood-level association between release from incarceration and fatal overdose, Rhode Island, 2016-2020.

The association between recent release from incarceration and dramatically increased risk of fatal overdose is well-established at the individual level. Fatal overdose and. arrest/release are spatially clustered, suggesting that this association may persist at the neighborhood level. We analyzed multicomponent data from Rhode Island, 2016-2020, and observed a modest association at the census tract level between rates of release per 1000 population and fatal overdose per 100,000 person-years, adjusting for spatial autocorrelation in both the exposure and outcome. Our results suggest that for ea

Opioid-related deaths before and after COVID-19 stay-at-home orders in Los Angeles County.

BACKGROUND: Opioid-related morbidity and mortality has increased during the COVID-19 pandemic, yet specific information about the communities most affected remains unknown. Our objective is to evaluate decedent-level associations with an opioid-related death following the implementation of stay-at-home orders in Los Angeles County. METHODS: This retrospective cohort study used data from the L.A. County Medical Examiner-Coroner to identify opioid-related deaths in 2019 and 2020. We used logistic regression to analyze the change in opioid-related deaths following a 30-day washout period after t

Pediatric dental clinic location and utilization in a high-resource setting.

OBJECTIVES: Examine dental utilization by Medicaid-insured children living in a high-resource area. Characterize distance and travel-related variables to accessing care. METHODS: Cross-sectional data were collected on dental clinics in Pittsburgh, Pennsylvania, caring for Medicaid-insured children ≥1 year. Shortest distances, drive times, and bus travel between dental clinics and high-poverty census tracts were determined through geographical information systems analysis. Primary care clinic (PCC) survey data were analyzed for children's dental use. Demographic characteristics and travel-rela

Maternal exposure to ambient levels of benzene and neural tube defects among offspring: Texas, 1999-2004.

BACKGROUND: Previous studies have reported positive associations between maternal exposure to air pollutants and several adverse birth outcomes. However, there have been no studies assessing the association between environmental levels of hazardous air pollutants, such as benzene, and neural tube defects (NTDs), a common and serious group of congenital malformations. OBJECTIVE: Our goal was to conduct a case-control study assessing the association between ambient air levels of benzene, toluene, ethylbenzene, and xylene (BTEX) and the prevalence of NTDs among offspring. METHODS: The Texas Bir

Community correlates of outcomes in subjects with panic attacks.

The purpose of this study was to document the association between community factors and mental health outcomes in subjects with panic attacks. Randomly selected adults from 18 census tracts were screened for the presence of panic attacks. A structured interview was used to assess health care utilization, psychiatric morbidity, quality of life, and sense of control over panic. Community measures were obtained from census data. Regression analyses found that each community measure was associated with at least one outcome even when adjusted for individual socioeconomic status and barriers to acce

Persistence of racial/ethnic and socioeconomic status disparities among non-institutionalized patients hospitalized with COVID-19 in Connecticut, July to December 2020.

BACKGROUND: COVID-19 hospitalizations of non-institutionalized persons during the first COVID-19 wave in Connecticut disproportionately affected the elderly, communities of color, and individuals of low socioeconomic status (SES). Whether the magnitude of these disparities changed after the initial lockdown and before vaccine rollout is not well documented. METHODS: All first-time hospitalizations with laboratory-confirmed COVID-19 during July to December 2020, including patients' geocoded residential addresses, were obtained from the Connecticut Department of Public Health. Those living in c

An Analysis of Distance from Collision Site to Pedestrian Residence in Pedestrian versus Automobile Collisions Presenting to a Level 1 Trauma Center.

This study tests the hypothesis that most pedestrian collisions occur near victims' homes. Patients involved in automobile versus pedestrian collisions who presented to the emergency department at a Level I trauma center between January 2000 and December 2009 were included in the study. Patient demographics were obtained from the trauma registry. Home address was determined from hospital records, collision site was determined from the paramedic run sheet, and the shortest walking distance between the collision site and pedestrian residence was determined using Google Maps. We summarized distan

Compositional and contextual factors associated with drug overdose deaths in the United States.

Background: In 2017, the US Department of Health and Human Services declared the Opioid epidemic a public health emergency. In the US, emergency rooms treat more than 1,000 people each day for drug overdose, and 115 of them die. This study examines compositional and contextual factors associated with drug overdose deaths rates in the US. Methods: Local spatial autocorrelation statistics were used to estimate hot spot areas to identify census tracts with high risk of drug overdose death. Logistic regressions investigated the relationship between drug overdose death rates and various composition

Income inequality, residential poverty clustering and infant mortality: a study in Rio de Janeiro, Brazil.

In this paper, we propose an approach to investigate the hypothesis that the residential concentration of poverty affects health status more deeply than when poverty is randomly scattered in a given geographical area. To characterize the geographic pattern of poverty in the city of Rio de Janeiro, Brazil, an index that measures the heterogeneity of poverty concentration among sub-areas was proposed. We used census data and defined poverty by means of the household head monthly income. The 153 neighborhoods that compose the city were used as the geographic units, and the census tracts as the su

Exploring the association between neighborhood disadvantage and cannabis retail density: A multi-measure analysis.

INTRODUCTION: There is growing interest in the relationship between neighborhood disadvantage and increased cannabis retail density, driven by evidence suggesting higher density is associated with increased cannabis use. Yet little is known on how this relationship varies across different measures of cannabis retail density. This study explores how measures of neighborhood advantage and disadvantage relate to four cannabis retail density measures in the US. METHODS: Data on licensed recreational cannabis retailers (n = 5586) were obtained from 18 state agency websites, geocoded, and spatially
