The chronic catheterisation of the amniotic sac of eight pregnant cows of 180 to 200 days gestational age is described using an indwelling catheter of polyvinyl tubing. Although strict asepsis was followed, the main problem encountered was preventing contaminant bacteria from overwhelming the fetus, causing its death. This was successfully achieved in four animals, three of which received a mixture of ampicillin and cloxacillin sodium intra-amniotically and parenterally in the dam. Daily sequential samples of amniotic fluid were readily obtained until just before fetal death.

Soft tissue abscess used to be an easy emergency department (ED) presentation: perform an incision and drainage (I + D) and discharge your patient. Times have changed. Methicillin-resistant Staphylococcus aureus (MRSA) is now a major cause of soft tissue abscess in ED patients. MRSA is, by definition, resistant to cloxacillin and cephalosporins. Almost all Canadian strains are susceptible to vancomycin and linezolid. MRSA strains are variably susceptible to trimethoprim-sulfamethoxazole (TMP-SMX), tetra/doxycycline, and clindamycin, with pooled Canadian clindamycin resistance just over 40%.

A 72-year-old, healthy white woman presented with rapidly advancing necrotizing erysipelas of the upper and lower eyelids. There was no previous history of trauma or skin abrasions. The patient was treated with intravenous methicillin (Staphcillin) for 2 weeks and oral cloxacillin (Tegopen), for 1 week with intensive wound cleaning and debridement. After 3 months of continuous wound care, the patient has healed her wounds by second intention, with a moderate amount of lid contracture. Further surgical correction of the lower lid defect may be necessary.
