Efficacy of wound protectors in reducing surgical site infections in patients undergoing open colorectal surgery: A systematic review and meta-analysis.

BACKGROUND: The incidence of surgical site infections (SSIs) has been reported to be significantly higher in colorectal surgeries compared to other surgical procedures. Wound protectors have been developed to help reduce the incidence of SSIs by creating a barrier between the incisional wound and the contaminated surgical field, but their efficacy remains uncertain. OBJECTIVE: To assess whether the use of wound protectors reduces the incidence of SSIs in patients undergoing open colorectal surgery. DATA SOURCES: A comprehensive literature review was conducted using PubMed, Embase, Web of Sci

HIBISCUS trial (hernia incidence with continuous barbed vs interrupted suturing in colorectal surgery with small incisions): a contemporary study protocol for a multicenter randomized controlled trial.

BACKGROUND: Incisional hernia is the most common long-term complication after abdominal surgery, including minimally invasive colorectal procedures. In Japan, fascial closure of small midline incisions is generally performed using interrupted suturing, but high-quality evidence comparing continuous barbed suturing with interrupted suturing is limited. Continuous closure may reduce the risk of incisional hernia; however, robust randomized data in laparoscopic and robot-assisted colorectal surgery are lacking. This study aims to determine whether continuous barbed suturing is superior to convent

Multivariate analysis of risk factors for surgical site infection after laparoscopic colorectal surgery.

BACKGROUND: Surgical site infection (SSI) and incisional hernia (IH) are among the most common complications after colorectal surgery. While many risk factors for these complications are unavoidable, evidence suggests that use of Pfannenstiel incisions for specimen extraction during laparoscopic procedures may reduce their incidence. The objectives of this study were to identify risk factors for extraction site SSI (primary objective) and IH (secondary objective) in patients undergoing laparoscopic colorectal surgery. METHODS: Patients who underwent laparoscopic colorectal resections at The O

Causes of Open Conversion in Colorectal Cancer Surgery and Impact on Prognosis.

PURPOSE: To examine the risk factors for open conversion during minimally invasive colorectal surgery and to investigate the impact of open conversion on short- and long-term prognosis. METHODS: Between 2016 and 2023, we retrospectively reviewed 4943 patients who underwent robotic or laparoscopic colorectal surgery. Patients were divided into those who underwent intraoperative open conversion (conversion group, n = 30) and those who underwent surgery without conversion (nonconversion group, n = 4913). Clinical characteristics were compared between the groups. RESULTS: Multivariate analysis r

KangDuo surgical robot KD-SR-01 versus da Vinci Xi in robot-assisted colorectal cancer resection: oncologic outcomes at three years in a multicenter randomized trial.

BACKGROUND: Earlier results from a multicenter randomized trial showed that colorectal cancer resection with KangDuo surgical robot KD-SR-01 (KD) and da Vinci Xi (DV) had similar perioperative and pathological outcomes, whereas long-term oncologic outcomes remained uncertain. METHODS: Three-year follow-up data were analyzed for the full 100-patient cohort of the multicenter randomized trial (KD, n = 50; DV, n = 50). The long-term endpoints were disease-free survival (DFS) and overall survival (OS). RESULTS: Median follow-up in both groups was 37 months (interquartile range [IQR], 34-40). Kap

Techniques and feasibility of the cranial-to-caudal approach for laparoscopic left colectomy in obstructive splenic flexure cancers: A consecutive case series (with video).

AIM: Laparoscopic surgery of splenic flexure cancers (SFCs) is one of the most challenging operations among other colorectal surgical procedures. The most commonly performed and recommended laparoscopic surgical technique is the medial-to-lateral approach. On the other hand, in obstructive splenic flexure cancers (OSFCs), the medial-to-lateral approach may not be possible due to the narrowed surgical field by dilated bowel and difficulty in positioning the bowel. This study aims to present the surgical technique and feasibility of laparoscopic left colectomy with cranial-to-caudal dissection a…

Iatrogenic ureteral injuries and their prevention in colorectal surgery: results from a nationwide survey.

AIM: Iatrogenic ureteral injury (IUI) occurs rarely during colorectal surgery but is associated with significant mortality, morbidity and medicolegal issues. Few cases are reported, and recommendations regarding prevention are lacking. The aim of this study is to describe the current state of practice regarding IUI and its prevention among general surgeons in Switzerland. METHOD: All Swiss general surgeons who are members of either the Swiss Association of Laparoscopic and Thoracoscopic Surgery or the Swiss Surgical Society were invited to participate in an anonymous online survey. Short-term Outcomes of Laparoscopic Versus Open Treatment of Internal Enteric Fistulae: A Logistic Regression Analysis.

Although laparoscopy improves outcomes for common general surgical procedures, its role in complex colorectal procedures is not clearly defined. We sought to evaluate whether laparoscopy retains its short-term benefits when used for treatment of complex intra-abdominal fistulae. A retrospective analysis was conducted including patients undergoing surgeries for enteric fistulas over a 7-year period. The χ tests, the Fisher exact tests, and Student t tests were used. Logistic regression models were used to assess the relationship between outcome and predictors. A total of 74 patients (31 open, 4

Role of associating liver partition and portal vein ligation for staged hepatectomy for advanced colorectal liver disease: indispensable or overstressed?

BACKGROUND: The aim of this study was to analyse the role of associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) for advanced colorectal liver disease. Surgery offers the best long-term survival in patients with colorectal liver metastases (CRLM). To increase the rates of resectability, two-stage procedures (TSH) and ALPPS are established methods in cases of advanced colorectal liver disease to avoid post hepatectomy liver failure (PHLF). There is still a debate of the oncological utility and the surgical ranking of ALPPS in this clinical scenario. The aim of th

Colorectal carcinoma in children.

BACKGROUND/PURPOSE: Colorecta I carcinoma is extremely rare in children and presents with a poor prognosis. Surgical management and long-term follow-up of this entity are still obscure because of lack of data. Therefore, a retrospective clinical trial was performed to evaluate the clinical characteristics of childhood colorectal carcinoma and to determine the predictors of poor outcome. METHODS: Records of children who had colorectal carcinoma and were treated at our unit between 1972 and 1997, inclusive, were reviewed retrospectively. Information recorded for each patient included age, sex,

Management of left sided colorectal emergencies. Results of a postal questionnaire.

AIM: Controversy continues over management of left sided colorectal emergencies. Historically, management was by a three-staged procedure but more recently a two-staged procedure (Hartmann's) was adopted. Since Dudley's description of the safety of peroperative antegrade colonic irrigation and primary anastomosis (PA) in the 1980s, some surgeons have adopted this one staged procedure. Studies have reported good outcomes. However, uptake of a one-staged approach is by no means universal. The aim of this study was to establish the operative choices of surgeons for emergency left sided colonic pa

The morbidity and mortality of emergent operations for colorectal disease.

p6 participants in the Veterans Administration Cooperative Studies Program investigating antibiotic prophylaxis in colorectal operations, 693 consecutive patients (1978-1981) from three hospitals were studied. This report concerns 20% of these patients who presented with either perforation, obstruction, or hemorrhage necessitating emergent surgical intervention on unprepared bowel. The 30-day, in-hospital mortality of the 138 patients undergoing emergency operations was 28%. Elective colorectal procedures during the same period had a six per cent mortality rate. There is a striking difference
