BACKGROUND: Children with immunoglobulin A vasculitis (IgAV Henoch-Schönlein purpura) frequently encounter nephritis (IgAV-N) with 1-2% risk of kidney failure. Serum C1q, C3, C4, and C5 components of complement levels were measured in 56 healthy subjects and 98 patients with trachoma. Serum C1q and C3 levels were found to be significantly low in stages II and III. There was no change in serum C4 and C5 levels in any of the stages. The levels of C1q and C3 complement components in serum in stages II and III returned to normal as the disease resolved following the medical treatment.

The third (C3) and fourth (C4) components of complement and C3 proactivator (C3PA) were determined in 55 children with low serum levels of calcium and magnesium and 30 normal children. The concentrations of serum C3, C4 and C3PA were significantly reduced in children with double deficiences of calcium and magnesium. There were significant correlations between calcium and C3 and magnesium and C3PA. The relations between calcium, magnesium and the classical or alternate pathway of complement systems are discussed.

Using in vitro complement immunofluorescent staining methods, serum samples from 5 active cases of bullous pemphigoid, with pemphigoid antibody titers of 320 or greater, were tested for their ability to fix the regulatory protein beta 1H globulin in addition to C4 and C3. Complement components of C1-C9 were estimated in children with protein-calorie malnutrition. OBJECTIVE: Serum CH50 and C4 levels are usually normal or elevated in rheumatoid arthritis (RA) but are classically decreased in patients with serious extra-articular manifestations (SEAMs) of the disease.
