A dynamic anatomical study of ulnar nerve motion after anterior transposition for cubital tunnel syndrome.

Cubital tunnel syndrome is the second most commonly encountered compression neuropathy of the upper limb. Multiple techniques for surgical management have been proposed but no universally accepted algorithm for management exists. Six cadaveric upper limbs underwent ulnar nerve decompression and anterior transposition into subcutaneous and then submuscular positions. After marking nerves with tungsten, radiological examination of nerve motion was performed and nerve angulations were measured in the region of the flexor carpi ulnaris (FCU) origin. Comparison of ulnar nerves in each position reve

Osteoporosis: the structural and reparative consequences for the skeleton.

We have discussed the biomechanical and biomaterial properties of bone with emphasis on the microenvironment. The microdensity of bone tissues determines the resistance to compressive forces (strength = density2). The structural orientation of bone determines the ability of bone to withstand torsion and bending forces. Highly remodeled bone is brittle and fractures easily because of the multiple reversal planes within the bone plates. Conversely, augmentation of bone on the periphery by modeling leads to increased material microstrength and structural macrostrength. Consequently, appositional

Exploiting Scale-Variant Attention for Segmenting Small Medical Objects.

Early detection and accurate diagnosis can predict the risk of malignant disease transformation, thereby increasing the probability of effective treatment. Identifying mild syndrome with small pathological regions serves as an ominous warning and is fundamental in the early diagnosis of diseases. While deep learning algorithms, particularly convolutional neural networks (CNNs), have shown promise in segmenting medical objects, analyzing small areas in medical images remains challenging. This difficulty arises due to information losses and compression defects from convolutional and pooling oper

Comprehensive guide to the diagnosis, management, and treatment of metastatic spinal cord compression syndrome.

Spinal metastases are the most common tumors found in the neuraxis. They correspond to 90 % of all tumors at this level. The thoracic region is the most frequently affected area, followed by the lumbar region, with the cervical region being the least frequently affected place. Back pain is the most common symptom, appearing in up to 95 % of patients who present with spinal cord compression syndrome. The evaluation of patients with spinal cord compression syndrome is complex and requires an entire multidisciplinary team for diagnosis, management, and treatment. In the medical part, treatment is

[Solitary mediastinal fibrous tumor complicated by hypoglycemic crises (Doege-Potter syndrome)].

Diagnostic and treatment algorithms for large mediastinal tumors are clear. However, long-term results are not always good. They largely depend on early diagnosis and morphological structure of tumor. Neoplasms may be asymptomatic for a long time, especially in case of slow growth. These tumors are usually diagnosed as soon as complications occur (for example, compression syndrome). Routine X-ray screening is rarer situation. Paraneoplastic syndromes are rare, and some ones are casuistic and unknown to surgical community. We describe the diagnosis and treatment of a patient with giant solitary

Dynamic Feature Elimination-Based Visual-Inertial Navigation Algorithm.

To address the problem of degraded positioning accuracy in traditional visual-inertial navigation systems (VINS) due to interference from moving objects in dynamic scenarios, this paper proposes an improved algorithm based on the VINS-Fusion framework, which resolves this issue through a synergistic combination of multi-scale feature optimization and real-time dynamic feature elimination. First, at the feature extraction front-end, the SuperPoint encoder structure is reconstructed. By integrating dual-branch multi-scale feature fusion and 1 × 1 convolutional channel compression, it simultaneou

Current algorithm for the surgical treatment of facial pain.

BACKGROUND: Facial pain may be divided into several distinct categories, each requiring a specific treatment approach. In some cases, however, such categorization is difficult and treatment is ineffective. We reviewed our extensive clinical experience and designed an algorithmic approach to the treatment of medically intractable facial pain that can be treated through surgical intervention. METHODS: Our treatment algorithm is based on taking into account underlying pathological processes, the anatomical distribution of pain, pain characteristics, the patient's age and medical condition, assoc

Interpretable machine learning model integrating MRI-derived paraspinal muscle parameters for predicting new vertebral compression fractures after vertebral augmentation.

OBJECTIVES: To develop and validate interpretable machine learning (ML) models incorporating MRI-derived paraspinal muscle parameters to predict new vertebral compression fractures (NVCF) after vertebral augmentation. MATERIALS AND METHODS: This multicenter retrospective study included patients with osteoporotic vertebral compression fractures (OVCF) who underwent vertebral augmentation with a 2-year follow-up. Data from two centers were merged and randomly divided (8:2) into training and test sets, while data from the third center served as an independent external validation set. Clinical va

Accuracy in diagnosing deep and pelvic vein thrombosis in primary care: an analysis of 395 cases seen by 58 primary care physicians.

BACKGROUND: Ruling out a deep vein thrombosis (DVT) is difficult in general practice because the clinical manifestations of DVT are nonspecific and more often due to other diseases. The aim of diagnostic screening in primary care must be to rule out a DVT with high accuracy in most patients, so that only those who are likely to have a DVT will undergo further testing. In this study, we tested the accuracy of exclusion of DVT by the combination of a clinical score (the Wells score) with either a bedside D-dimer test or selective compression sonography. METHOD: This cohort study included all pa

Enhancing foundational validity of forensic findings in nonlethal medico-legal strangulation examinations.

BACKGROUND: Nonlethal strangulation occurs when the brain is deprived of oxygen because of external compression leading to the occlusion of the neck's blood vessels and/or airway. The current state of strangulation science confines expert testimony to merely describing injuries attributed to strangulation 'based on the expert's experience and training.' Expert testimony that can quantify the likelihood that observed injuries are attributable to strangulation would strengthen the scientific validity of such testimony. PURPOSE: This study aims to identify those documented features, or clusters

Image reconstruction algorithm based on frequency-wavenumber decoupling for three-dimensional MIMO-SAR imaging.

In this paper, a frequency-wavenumber decoupling algorithm with high-efficiency and high-precise for three-dimensional (3-D) multiple-input-multiple-output synthetic aperture radar (MIMO-SAR) imaging is proposed. Based on one-dimensional (1-D) MIMO array combined with synthetic aperture scan along another dimension, MIMO-SAR imaging scheme allows the number of array elements to be greatly reduced compared with the two-dimensional (2-D) MIMO arrays. By multi-dimensional Fourier transforming and Method of Stationary Phase (MSP), analytical expression of the object function in the frequency-waven

A biologically-based algorithm for companding computerized tomography (CT) images.

Computerized Tomography (CT) images are High Dynamic Range (HDR) images of the X-ray attenuation coefficients of the body's tissues. The inability to see abnormalities in tissues with marked differences in their X-ray attenuation coefficients, in a single CT window, poses a significant clinical problem in radiology. In order to provide proper contrast, which reveals all the required clinical details within each specifically imaged tissue, a single CT slice must be viewed by a radiologist four times: the first viewing focuses on the lung window; the second viewing focuses on the soft tissues wi

Deep Learning Based Approach for Lossless ECG Compression.

OBJECTIVE: Tele-monitoring is a useful platform for remote monitoring of cardiac patients, where compression plays a significant role in reducing the link burden and memory utilization of the source device. This paper describes a new approach for lossless ECG compression based on a deep-learning method via an adaptive autoregressive integrated moving average (ARIMA) model. METHODS: Raw ECG signals were denoised and preprocessed to generate beat-cells for further processing. The ARIMA model uses the individual cardiac cycles to generate model parameters, which are then compressed. In this rese

Printed Circuit Board Defect Detection Based on Lightweight Deep Learning Fusion Model.

Printed circuit boards (PCBs) are ubiquitous and essential electronic components. Tiny targets and high precision are the focus of PCB defect detection. This paper proposes an improved model focusing on tiny defect detection and model compression to achieve better performance in PCB defect detection. The improved model has a compact structure based on MobileNet v3 Small-CA and an image-cutting layer. Moreover, it applies an improved multi-scale fusion step with a location weighted mechanism to enhance representation performance. The proposed model outperforms state-of-the-art detection algorit

The role of radiological imaging in the diagnosis of acute appendicitis.

Several strategies have been employed to improve the accuracy of the diagnosis of appendicitis and to reduce the associated perforation rate. Because clinical algorithms have been disappointing, many physicians resort to radiological modalities. Plain abdominal x-rays are nonspecific, barium enema examination has relatively low accuracy, scintigraphy scans require considerable time and are difficult to interpret, and magnetic resonance imaging is relatively unstudied. The most promising modalities are graded compression sonography and computed tomography. In expert hands, these techniques can

Pitfalls in compressed sensing reconstruction and how to avoid them.

Multidimensional NMR can provide unmatched spectral resolution, which is crucial when dealing with samples of biological macromolecules. The resolution, however, comes at the high price of long experimental time. Non-uniform sampling (NUS) of the evolution time domain allows to suppress this limitation by sampling only a small fraction of the data, but requires sophisticated algorithms to reconstruct omitted data points. A significant group of such algorithms known as compressed sensing (CS) is based on the assumption of sparsity of a reconstructed spectrum. Several papers on the application o

Craniovertebral junction database analysis: incidence, classification, presentation, and treatment algorithms.

INTRODUCTION: The initial treatment of bony lesions at the craniocervical junction consisted of posterior decompression with enlargement of the foramen and removal of the posterior arch of the atlas and axis vertebra. This was regardless of the site of compression. Needless to say, the results were poor when an irreducible ventral lesion at the craniocervical junction caused compression of the cervicomedullary area. It was predictable that an adverse outcome would occur in approximately 35-40% of patients treated with dorsal decompression with a fixed anterior abnormality at the craniovertebra

Complex Mechanical Loading and Pro-Inflammatory Cytokines in Intervertebral Disc Degeneration.

BACKGROUND: Intervertebral disc (IVD) degeneration is a major contributor to low back pain, yet its initiating factors remain unclear. While the individual effects of pro-inflammatory cytokines and mechanical loading on IVDs have been studied, their combined impact is poorly understood. This study investigated how dynamic compression and torsion interact with interleukin-1 beta (IL-1β) and its inhibitor, interleukin-1 receptor antagonist (IL-1Ra), using bovine IVDs in an ex vivo organ culture system. METHODS: Whole bovine caudal IVDs were cultured for one week in a custom bioreactor applying

Human ECGs corrupted with real CPR artefacts in an animal model: generating a database to evaluate and refine algorithms for eliminating CPR artefacts.

AIM: For the analysis of ECG rhythms during ongoing CPR, single- or two-channel methods have been proposed to eliminate artefacts from the CPR-corrupted ECG. To refine, test and evaluate these algorithms with a realistic data set, we introduce an animal model with which we created an extended database of human ECGs with real CPR artefacts. MATERIAL AND METHODS: In a pig model real CPR-related artefacts were added to annotated human emergency ECGs. Via a special catheter placed in the oesophagus, ECG sequences (duration>10s) were fed in close to the dead pig's heart. The resulting surface pote
