BACKGROUND: In colorectal surgery, a higher body mass index (BMI) is associated with increased technical difficulty, reduced use of minimally invasive surgery (MIS), and higher rates of conversion to open. While these associations are well established, less is known about how they vary across systems and providers. The study presents a methodological framework that evaluates the impact of obesity on operative approach and conversion risk in colorectal surgery and compares these associations between a provincial Canadian cohort (Nova Scotia (NS)) and National-Surgical-Quality-Improvement-Progra

HYPOTHESIS: Hand-assisted laparoscopic surgery (HALS) is a safe therapeutic approach to remove megaspleens of any size. Conventional laparoscopic splenectomy for splenomegaly is difficult because of limited exposure and complex vascular control, with increased risk of intraoperative bleeding and conversion to open surgery. HALS can overcome some of these limitations, reducing the risk of conversion to open surgery and resulting in a postoperative course similar to that of conventional laparoscopy.
DESIGN: Single-institution single-surgeon retrospective review.
SETTING: University hospital.

BACKGROUND: Laparoscopic surgery has gradually gained acceptance for abdominal surgical emergencies; however, limited reports exist on laparoscopic surgery for strangulated small bowel obstruction (SSBO). We aimed to demonstrate the efficacy and feasibility of laparoscopic surgery for SSBO.
METHODS: In this single-center retrospective study, patients who underwent emergency surgery for SSBO between January 2014 and December 2024 were included and divided into laparoscopy and open groups. Propensity score matching (PSM) was performed to compare the primary outcomes-intraoperative and short-ter

OBJECTIVE: The aim of this study was to investigate the clinical outcome of laparoscopic surgery for Crohn's disease and clarify the indications using the Vienna Classification.
METHOD: Between September 1994 and July 2004, 107 patients with Crohn's disease underwent 124 procedures. Of these, 91 laparoscopic procedures formed the basis of this study. The Vienna Classification, which consists of three subgroups - age at diagnosis (A1-2), location (L1-4) and behaviour (B1-3) - was applied to compare the conversion to open surgery and incidence of postoperative complications.
RESULTS: Conversio

BACKGROUND: Recent reports revealed that laparoscopic adhesiolysis is a feasible procedure for patients with adhesive small bowel obstruction (SBO). However, the feasibility of laparoscopic surgery for strangulated SBO has not been investigated in detail.
METHODS: Ninety-six patients who underwent surgery for strangulated SBO between April 2008 and September 2019 were included. Of these, 49 patients were intended to undergo laparoscopic surgery, while the other 47 underwent open surgery from the first. Comparisons were made between the patients who underwent laparoscopic and open surgery in t

BACKGROUND: Laparoscopic cholecystectomy (LC) is nowadays the gold standard in the surgical treatment of cholelithiasis and gallbladder diseases. But sometimes it may be inevitable to convert it to open surgery to safely end the procedure.
OBJECTIVES: In this study, we aimed to investigate the risk factors for conversion to open surgery from LC.
MATERIAL AND METHODS: The records of patients that underwent LC in Malatya State Hospital (Malatya, Turkey) between January 2013 and May 2014 were prospectively examined. One hundred and forty-five patients were involved in this study. The patients w

INTRODUCTION: Implementation of minimally invasive gastrectomy (MIG) for malignancy is increasing. However, risk factors for conversion to open surgery during laparoscopic and robotic gastrectomy are poorly understood. This study aimed to determine the risk factors for, and impact of, conversion during oncologic resection.
METHODS: The National Cancer Database (NCDB) was used to identify patients with clinical stage I-III gastric cancer from 2010 to 2017. Chi-squared test and t-test were used to compare the robotic versus laparoscopic groups. Propensity score weighted multivariable logistic r

BACKGROUND: Laparoscopic cholecystectomy has become the standard treatment for symptomatic gallstones. However, a conversion to open surgery is sometimes still required to complete the procedure safely. The aim of this study is to identify the predictive factors of conversion from laparoscopic to open cholecystectomy in both elective and emergency cases.
PATIENTS AND METHODS: A retrospective review of all patients underwent laparoscopic cholecystectomy for symptomatic gallstones from January 2011 to October 2013 was performed. Data considered for analysis were: demographic data, comorbidities

Laparoscopic cholecystectomy is a standard treatment for cholelithiasis. In situations where laparoscopic cholecystectomy is dangerous, a surgeon may be forced to change from laparoscopy to an open procedure. Data from the literature shows that 2 to 15% of laparoscopic cholecystectomies are converted to open surgery during surgery for various reasons. The aim of this study was to identify the risk factors for the conversion of laparoscopic cholecystectomy to open surgery. A retrospective analysis of medical records and operation protocols was performed. The study group consisted of 263 patient

PURPOSE: Conversion surgery (CS) is performed for locally advanced pancreatic cancer (LAPC); however, the criteria for CS varies among institutions. This study aimed to evaluate the clinical outcomes of CS for LAPC based on strict indications at our center. METHODS: This study included patients diagnosed with and treated for LAPC between 2011 and 2021. The CS criteria indicated that the tumors shrunk to the same status as resectable pancreatic cancer (RPC) on imaging and were maintained for > 3 months. Additionally, tumors that had shrunk to a size corresponding to borderline resectable pancre

Totally robotic low anterior resection (TR-LAR) has not been consistently compared with conventional laparoscopic low anterior resection (L-LAR), as previous studies often combined hybrid and fully robotic techniques. This limits the ability to assess the true impact of a completely robotic approach. Therefore, a focused evaluation of TR-LAR versus L-LAR is warranted. A PRISMA-guided systematic review and meta-analysis of studies comparing totally robotic versus laparoscopic LAR was performed. Random-effects models were used to pool outcomes, with heterogeneity assessed by I² and evidence eval

The robotic approach improves the feasibility of minimally invasive colectomy even where there may be an anatomic challenge with laparoscopy. Whether a failure in completing colectomy with this newer technology is associated with worse consequences needs to be considered when evaluating the relative benefit of robotic colectomy. The aim of this study is to evaluate rates of conversion to open surgery after robotic and laparoscopic colectomy and whether outcomes after conversion vary after the two techniques since this has not been well studied. From the American College of Surgeons (ACS) - Nat

A 44-year-old woman with a medical history of anti-phospholipid antibody syndrome complicated by recurrent pulmonary emboli with subsequent chronic hypoxic respiratory failure (3 L/min oxygen baseline) presented to the ED with 2 to 3 weeks of shortness of breath and pleuritic chest pain that radiated to the center of her back. These symptoms were accompanied by an increase in her oxygen requirement from 3 L/min to 6 L/min. She also reported nausea, vomiting, lightheadedness, and dizziness for the same period. The patient had two prior pulmonary emboli in the same year, which prompted a hyperco
