Pulmonary cryptococcosis in AIDS.

Cryptococcus neoformans is emerging as an important etiologic agent of disseminated infection in patients with the acquired immunodeficiency syndrome (AIDS). Little attention has been placed on the pulmonary expression of this systemic infection. We report five patients with AIDS and cryptococcosis with primary pulmonary involvement. Patients usually presented with fever, cough, dyspnea, and pleuritic chest pain. Chest x-ray findings varied from localized and diffuse infiltration to lymphadenopathy and pleural effusions. All patients developed disseminated disease despite antifungal therapy. P…

Pulmonary cryptococcosis in the rat.

We evaluated a rat model of pulmonary cryptococcosis. Rats immunized subcutaneously with cryptococcal antigen developed peritoneal but not pulmonary macrophage migration inhibition factor (MIF) to cryptococcal cell extract antigens. They did not develop MIF to capsular polysaccharide antigens. Conversely, when rats were immunized subcutaneously with heat-killed cryptococcus neoformans, migration inhibition factor was detected in alveolar but not in peritoneal macrophages. Finally, rats given viable cryptococci subcutaneously developed both pulmonary and extrapulmonary cell-mediated immune resp…

Cryptococcosis in the Northern Territory.

This report represents a review of cryptococcosis in the Northern Territory from 1957 to 1975. There were 26 cases over a 19-year period; 25 of these were in full-blooded Aborigines. The disease occurred throughout the rural areas of the Territory as isolated cases. There were 24 cases of cryptococcal meningitis and only two with solitary lung involvement. The overall mortality was 50%. In all five untreated cases the disease was fatal. There were eight deaths among the 20 patients receiving chemotherapy, a mortality of 40%. Lung resection was performed in six cases of localized pulmonary cryp…

Cerebral cryptococcosis in Malaysia.

Cryptococcal infection of the brain as encountered in a tropical country is reviewed. The meningitic form is not uncommon and there has been, in the last decade, an apparent, if not real, rise in incidence in Malaysia as in Singapore. Only exceptionally was there overt evidence of immunological deficiency. Hydrocephalus was present in about three-quarters of the patients with meningitis and shunts were employed readily. The presence of multiple small intracerebral cysts could be suspected clinically but treatment for this complication was ineffective. The antifungal agent used most frequently

[Cryptococcosis during HIV infection].

Cryptococcosis is a cosmopolitan fungal serious condition due to an encapsulated yeast Cryptococcus neoformans. This is the systemic fungal infection the most common in HIV infection. This yeast is present in the environment and its main entrance in the body is the respiratory tract. Its gravity is linked to its tropism for the central nervous system. It generally affects subjects with severe deficit of cellular immunity and in particular, patients living with HIV. The diagnosis of neuromeningeal cryptococcosis is based on the detection of encapsulated yeasts at microscopic examination of cere…

Host defenses against cryptococcosis.

The interaction of pathogenic Cryptococcus species with their various hosts is somewhat unique compared to other fungal pathogens such as Aspergillus fumigatus and Candida albicans. Cryptococcus shares an intimate association with host immune cells, leading to enhanced intracellular growth. Furthermore, unlike most other fungal pathogens, the signs and symptoms of cryptococcal disease are typically self-inflicted by the host during the host's attempt to clear this invader from sensitive organ systems such as the central nervous system. In this review, we will summarize the story of host-Crypto

Chapter 3--pulmonary cryptococcosis.

Cryptococcosis is a systemic mycosis caused by two species of the encapsulated basidiomycetes, Cryptococcus neoformans and C. gattii, which, respectively, cause infection in immunocompromised individuals and in immunologically normal hosts. Patients with T-cell deficiencies are more susceptible to this infection. The spectrum of the disease ranges from asymptomatic pulmonary lesions to disseminated infection with meningoencephalitis. The medical relevance of cryptococcosis increased dramatically as a consequence of the AIDS epidemic and organ transplants.

[Nasal cutaneous cryptococcosis localization].

Cryptococcosis is a polymorphous disease occurring especially in the course of the Acquired Immuno Deficiency Syndrome (AIDS). It puts so diagnostic that therapeutical problems in African countries. The objective of this observation is to report a case of nasal localization with a cutaneous cryptococcosis. It has been confirmed by the histopathologic study of the cutaneous biopsy. The patient was 42 year-old female, HIV positive, bachelor and hospitalised for epistaxis with cutaneous tumefaction and lesions of the nose. The rate of CD4 was 98/mm3. The treatment was based on amphotericine B. Pa…

Vertebral cryptococcosis simulating tuberculosis.

Infection with the fungus Cryptococcus neoformans is seen predominantly in two forms: (a) pulmonary and (b) cerebromeningeal. Skeletal cryptococcosis is uncommon. There have been only occasional case reports of thoracic vertebral cryptococcosis presenting as cord compression. A young female had cervical lymphadenopathy diagnosed as tuberculosis by fine needle aspiration cytology (FNAC) and was on antitubercular therapy (ATT) for 5 months. She developed rapidly progressive paraparesis and imaging demonstrated a destructive vertebral body lesion involving T2-3 with a paraspinal abscess producing…
