Cryptogenic organising pneumonia: an unusual cause of pleuritic chest pain.

A 76-year-old woman presented with a 2-hour history of pleuritic chest pain with no other associated symptoms. Blood investigations revealed raised inflammatory markers and an elevated white cell count. On chest radiograph, an airspace shadow indicative of a consolidation was prominent. This was followed by a CT scan of her thorax which showed a spiculated lesion in the right upper lobe, a lesion in the posterior segment of the left lower lobe and mildly enlarged right hilar lymph nodes. She was started on dual antibiotic therapy; however, the patient's clinical status and inflammatory markers

Organizing pneumonia secondary to influenza infection: Two case reports and a literature review.

BACKGROUND: Organizing pneumonia (OP) is a rare complication of influenza virus infection but scarce data are available. The recognition of this entity is important because require appropriate treatment. METHODS: We report two cases and perform a systematic review on PubMed database. Only cases with histological confirmation of OP and influenza virus positive laboratory test were included. RESULTS: We collected 16 patients. Median age was 52 year, 20% of patients were smokers and 43.8% had not any comorbidity. Influenza A virus infection was diagnosed in 75%. Clinical manifestation consisted

Unusual pulmonary toxicity of ipilimumab treated by macrolides.

INTRODUCTION: Ipilimumab is a human monoclonal antibody that blocks cytotoxic-T-lymphocyte-associated antigen 4 (CTLA-4) and promotes antitumour immunity. It has recently been approved for the treatment of metastatic melanoma patients. Ipilimumab is now widely used and the spectrum of side effects because of autoimmunity is expanding. To our knowledge, this is the first report of an ipilimumab-induced pulmonary immune-related adverse event (irAE) that was successfully treated by macrolides without corticosteroids. CASE REPORT: A 77-year-old man with metastatic melanoma developed fever, cough

Organising pneumonia presenting as acute life threatening pulmonary haemorrhage.

Organising pneumonia, previously called bronchiolitis obliterans organising pneumonia is a clinicopathological entity of unknown aetiology, which has been reported with increasing frequency. Various modes of presentation have been described such as cough, fever, weight loss and alveolar opacities on chest radiograph. Haemoptysis as primary presenting symptom has only rarely been reported. The authors report a case in which massive life-threatening haemoptysis was the major presenting symptom. No aetiology was identified for the haemoptysis and the diagnosis was confirmed on postmortem histolog

Pulmonary melioidosis associated with bronchiolitis obliterans organizing pneumonia.

We report a case of pulmonary melioidosis associated bronchiolitis obliterans with organizing pneumonia (BOOP). While antibiotic treatment alone did not show any major effect, addition of corticosteroids resulted in rapid clinical improvement. To our knowledge this is the first documented case of melioidosis associated with organising pneumonia. The possible therapeutic implications for the treatment of severe pulmonary melioidosis are discussed.

Organising pneumonia in ulcerative colitis with primary sclerosing cholangitis: drug-related toxicity or extraintestinal manifestation?

Pulmonary manifestations of inflammatory bowel disease (IBD) are rare but clinically significant. We present a case of a man in his 20s with ulcerative colitis and primary sclerosing cholangitis (PSC), who developed exertional breathlessness, cough and systemic symptoms. Imaging revealed bilateral pulmonary nodules, and histopathology confirmed organising pneumonia. Treatment with systemic corticosteroids led to significant clinical and radiological improvement, although a relapse occurred with early steroid tapering. This case underscores the importance of recognising pulmonary complications

Computed tomography findings of influenza A (H1N1) pneumonia in adults: pattern analysis and prognostic comparisons.

AIM: The aim of this study was to evaluate retrospectively the chest computed tomography findings of influenza A (H1N1) pneumonia and their relationship with clinical outcome. METHODS: Chest computed tomography findings and clinical outcomes of 76 patients with influenza A (H1N1) pneumonia were assessed. Computed tomography findings were evaluated for the presence and distribution of parenchymal abnormalities, which were then classified into 3 patterns: bronchopneumonia, cryptogenic organizing pneumonia (COP), and acute interstitial pneumonia (AIP) patterns. Clinical courses were divided into…

Clinical characteristics of autoimmune rheumatic disease-related organizing pneumonia.

To study the clinical characteristics of autoimmune rheumatic disease-related organizing pneumonia (AIRD-OP), the clinical presentation, radiological findings, treatment, and outcome of AIRD-OP patients were analyzed, in comparison with patients with cryptogenic organizing pneumonia (COP). A total of 131 OP patients were identified, including 57 cases of AIRD-OP, 35 cases of COP, and 39 cases of other disease-related OPs. Among AIRD-OP patients, 36 (63%) presented the symptoms of OP at onset. The primary disease of AIRDs included Sjogren's syndrome (38%), polymyositis/dermatomyositis (23%), rh…

Update for pathologists on idiopathic interstitial pneumonias.

CONTEXT: Idiopathic interstitial pneumonias are a subset of diffuse pulmonary interstitial diseases classified by international consensus in 2002 as idiopathic pulmonary fibrosis, nonspecific interstitial pneumonia, cryptogenic organizing pneumonia, acute interstitial pneumonia, respiratory bronchiolitis interstitial lung disease, desquamative interstitial pneumonia, and lymphoid interstitial pneumonia. Each is associated with a characteristic histopathologic pattern. In 2011, updated consensus guidelines were released for diagnosis and management of idiopathic pulmonary fibrosis. The entire g…

Mycoplasma pneumoniae-associated bronchiolitis causing severe restrictive lung disease in adults: report of three cases and literature review.

STUDY OBJECTIVES: To characterize adult Mycoplasma pneumoniae-induced bronchiolitis requiring hospitalization. DESIGN: We encountered an adult patient with severe bronchiolitis in the absence of pneumonia due to M. pneumoniae. To determine the relative frequency of such a condition, we retrospectively reviewed the medical records of adults over a 4-year period with a hospital discharge diagnosis of "bronchiolitis" from a university hospital. SETTING: University Hospital of the University of Colorado Health Sciences Center, Denver, CO. STUDY SUBJECTS: From 1994 to 1998, 10 adult inpatients w

Bronchiolitis obliterans organizing pneumonia associated with acute Mycoplasma pneumoniae infection.

A patient with bronchiolitis obliterans organizing pneumonia (BOOP) associated with acute Mycoplasma pneumoniae infection is described. Although mycoplasmas have occasionally been associated with bronchiolitis obliterans, to the best of our knowledge, this is the first well-documented case of BOOP associated with M. pneumoniae infection. The diagnosis of BOOP was made by open-lung biopsy after fiberoptic transbronchial biopsy proved nondiagnostic. Corticosteroid therapy was administered and a dramatic improvement was observed; the patient remained without complaints during a 3-year follow-up p
