Preclinical and translational models for delirium: Recommendations for future research from the NIDUS delirium network.

Delirium is a common, morbid, and costly syndrome that is closely linked to Alzheimer's disease (AD) and AD-related dementias (ADRD) as a risk factor and outcome. Human studies of delirium have advanced our knowledge of delirium incidence and prevalence, risk factors, biomarkers, outcomes, prevention, and management. However, understanding of delirium neurobiology remains limited. Preclinical and translational models for delirium, while challenging to develop, could advance our knowledge of delirium neurobiology and inform the development of new prevention and treatment approaches. We discuss …

Delirium in older hospitalized patients-A prospective analysis of the detailed course of delirium in geriatric inpatients.

BACKGROUND: Delirium in older hospitalized patients (> 65) is a common clinical syndrome, which is frequently unrecognized. AIMS: We aimed to describe the detailed clinical course of delirium and related cognitive functioning in geriatric patients in a mainly non-postoperative setting in association with demographic and clinical parameters and additionally to identify risk factors for delirium in this common setting. METHODS: Inpatients of a geriatric ward were screened for delirium and in the case of presence of delirium included into the study. Patients received three assessments including…

Delirium and subsyndromal delirium are associated with the long-term risk of death after ischaemic stroke.

BACKGROUND: Post-stroke delirium has a negative impact on functional outcome. We explored if there is any association between delirium, subsyndromal delirium and long-term mortality after ischaemic stroke and transient ischaemic attack. METHODS: We included 564 patients with ischaemic stroke or transient ischaemic attack. We assessed symptoms of delirium during the first 7 days after admission. We used Cox proportional hazards models to analyse all-cause mortality during the first 5 years after stroke. RESULTS: We diagnosed delirium in 23.4% and subsyndromal delirium in 10.3% of patients. Du…

Venlafaxine and Delirium: Understanding the Association.

Delirium represents a significant clinical, economic, and societal challenge, frequently arising in hospitalized patients due to multiple factors, including pharmacological triggers. Recognizing and preventing delirium is crucial to improving patient outcomes and reducing healthcare costs. This review focuses on the association between venlafaxine, a commonly used antidepressant, and delirium. We explore potential mechanisms, clinical presentations, and risk factors linked to venlafaxine-induced delirium, emphasizing the need for heightened awareness among clinicians. The findings underscore t…

[Delirium in the elderly - common and dangerous].

Delirium is an acute, fluctuating neurocognitive disorder characterized by disturbances in consciousness and attention. Unlike dementia, delirium has somatic causes and is reversible if the triggers are identified and treated. Elderly people are particularly at risk, especially those in hospitals or long-term care facilities. Its development is multifactorial. The threshold concept explains the interaction of predisposing factors such as age, dementia, multimorbidity, malnutrition, substance use, and triggering factors such as surgery, infections, pain, and immobilization. Early detection and r…

Quetiapine for the Treatment of Pediatric Delirium.

BACKGROUND: Delirium is a common complication of critical illness, with a prevalence of 25% among pediatric intensive care unit (ICU) patients. Pharmacological treatment options for ICU delirium are limited to off-label use of antipsychotics, but their benefit remains uncertain. OBJECTIVE: The purpose of this study was to evaluate quetiapine effectiveness for the treatment of delirium in critically ill pediatric patients and to describe the safety profile of quetiapine. METHODS: A single-center, retrospective review of patients aged ≤ 18 years who screened positive for delirium via the Corne…

[Postoperative delirium in the recovery room].

Postoperative delirium during emergence from anesthesia is the most frequent neuropsychiatric complication in the post-anesthesia care unit. In addition to increased medical and especially nursing care efforts, affected patients are threatened with delayed rehabilitation with a longer hospital stay and an increased mortality. It is therefore essential to identify risk factors at an early stage and to implement preventive measures; however, should a postoperative delirium occur in the post-anesthesia care unit despite the use of these preventive measures, it should be detected and treated at an…

Diagnosis of delirium: a practical approach.

Delirium is an acute disorder of fluctuating attention and awareness with cardinal features that allow it to be positively distinguished from other causes of an acute confusional state. These features include fluctuations, prominent inattentiveness with other cognitive deficits, a change in awareness and visual hallucinations. We describe a framework for diagnosing delirium, noting the need to consider certain caveats and differential diagnoses. Delirium is a clinical diagnosis where a thorough history and clinical examination are much more helpful diagnostically than any single test or combin…

Factors associated with the severity of delirium.

BACKGROUND: Various factors affecting the development of delirium have been identified. However, the associations between the severity of delirium and potentially related factors have not been adequately investigated. The aim of the present study was to explore factors associated with the severity of delirium and to identify the reversible contributing factors. METHODS: A total of 577 patients with delirium referred to the Department of Psychiatry during the 5 years from May 2015 to April 2020 at a general hospital were included. The Delirium Rating Scale-revised-98 (DRS-R-98) was used to mea…

Valproic Acid in the Management of Delirium.

CONTEXT: Antipsychotics and benzodiazepines do not improve delirium. Valproic acid (VPA) has been used recently to treat agitation in delirium. OBJECTIVES: To review the evidence for VPA in the management of Delirium. METHODS: Systematic review. English language, age 19 and above, from 1946 to January 12, 2021. MESH TERMS: "Valproic acid", "valproate", "sodium valproate", "delirium", "acute mania with delirium" in PubMed and Ovid. EXCLUSION: Studies of VPA used for diagnoses other than delirium. RESULTS: 21 abstracts were identified and 10 studies were included in the review (252 patients…

Delirium in the critically ill patient.

Delirium is an acute and transient brain dysfunction that is characterized by disturbances in consciousness, affecting both its content (i.e., attention) and level (i.e., arousal). It affects as many as 50% of those admitted to an intensive care unit (ICU). Once believed to be an inconsequential outcome of critical illness, it is now recognized that delirium is harmful in both the short- and long-term. Despite occurring frequently in critically ill patients, delirium often goes unrecognized. Well-validated delirium screening tools, designed for use in the ICU, should be used to reliably detect…
