OBJECTIVE: This study aimed to investigate the following: (1) the preferences of public dentists for procedures and materials in carious or non-carious pulp exposure of permanent teeth; (2) how factors such as clinical experience, scientific literature reading, and material availability affect their choices; (3) the self-assessed risk of root canal treatment after pulp capping.
MATERIALS AND METHODS: An online questionnaire consisting of 20 multiple choice and open-ended questions was e-mailed to Norwegian public dental clinics. It assessed dentists' material preferences for direct pulp cappi

AIM: To evaluate the dental pulp response to a novel mineral trioxide aggregate containing phosphorylated pullulan (MTAPPL) in rats after direct pulp capping.
METHODS: Ninety-six cavities were prepared in the maxillary first molars of 56 male Wistar rats. The dental pulps were intentionally exposed and randomly divided into four groups according to the application of pulp capping materials: MTAPPL; phosphorylated pullulan (PPL); a conventional MTA (Nex-Cem MTA, NCMTA; positive control); and Super-Bond (SB; negative control). All cavities were restored with SB and observed for pulpal responses

Outcome expectations of direct pulp capping in carious teeth are obscured by a clinically unknown infiltration and breakdown of the dental pulp tissue. Histologic studies showed that this soft tissue breakdown is related to the innate immune system. We hypothesized 1) that a neutrophil biomarker could predict the outcome of direct pulp capping and 2) that using sodium hypochlorite (NaOCl) as a lavage solution to remove necrotized infected pulp tissue could improve it. In this randomized trial in mature posterior teeth causing no or mild discomfort with carious pulpal exposures, pulpal fluid wa

BACKGROUND: This study aimed to evaluate and compare the discoloration potential of four bioceramic pulp-capping materials in the presence of blood or 0.9% normal saline (NS) under in vitro conditions.
METHODS: Extracted maxillary central incisors were prepared with standardized Class I cavities and allocated to blood or NS environments. Each medium was further subdivided according to the pulp-capping material used: Biodentine, NeoMTA, TotalFill Putty (TFP), and TheraCal PT (TPT). Following composite restoration, color measurements were obtained using a spectrophotometer at baseline, 1 week,

Vital pulp therapy (VPT) has been the principal treatment option used to preserve pulp function during apical development of immature permanent teeth affected by caries and trauma. Advances in VPT protocols and bioactive materials have made this approach a possible treatment option for mature permanent teeth. This article presents a clinical VPT protocol for mature permanent teeth and compares 2 failed cases and 2 successful cases completed in the general dentistry clinic of a dental school. The objective is to allow general dentists to be more comfortable with providing VPT procedures as a tr

The intra- and intercellular signaling molecule nitric oxide (NO) is produced in endothelial cells by the activity of endothelial NO synthase (eNOS). Upon formation, NO diffuses into the underlying vascular smooth muscle cells, where it activates NO-sensitive guanylyl cyclase (NO-GC) resulting in the production of cyclic guanosine 3',5'-monophosphate (cGMP) from guanosine 5'-triphosphate (GTP). Inducing vasodilatation, inhibiting platelet aggregation and leukocyte adhesion, and inhibiting the proliferation and migration of vascular smooth muscle cells, the NO-cGMP signaling leads to a number o

In this article the defense mechanisms of the pulp and pulp reactions to injury were briefly reviewed. Iatrogenic pulp injury was emphasized and guidelines for prevention of such injury were provided. These could be summarized as follows: Never cut dry. Always use a water coolant to prevent heat generation and desiccation of dentin. Use a protective base or liner with materials that are known to be irritating to the pulp (groups II and III) regardless of cavity depth. Use a protective base in all deep cavities where the possibility of a microscopic exposure of the pulp exists, regardless of th

BACKGROUND: The advancement in the dentin regeneration aiming at the preservation of pulpal vitality has challenged the traditional concept of substituting diseased tooth with inert materials. Clinical studies, demonstrating preservation of pulpal vitality with the use of biomimetic materials in deep carious lesions, are lacking in the literature.
OBJECTIVE: This clinical study aims to assess the treatment outcome of vital pulp therapy using Biodentine in cases of deep carious lesion in a single visit treatment protocol.
METHODOLOGY: Twelve posterior teeth with deep carious lesion with no si

AIM: To compare the histomorphological response of rat dental pulps capped with MM-MTA and ProRoot MTA at 1, 2 and 4 weeks postoperatively.
MATERIALS AND METHODS: Direct pulp capping with MM-MTA and ProRoot MTA, overlaid with light-cured composite resin, was performed on right and left maxillary first molars of 20 Wistar rats. Animals were killed at 1, 2 and 4 weeks postoperatively. Biopsy samples were stained and viewed by light microscopy to determine dentin bridge formation. Data were statistically evaluated with the Kruskal-Wallis test (p < 0.05).
RESULTS: Absence of dentin bridge format

INSTRUCTION: The purpose of this study was to evaluate the three-year success rate of the treatment for early childhood caries (ECC) under general anesthesia in different periods (2011 and 2018).
METHODS: Children (<6 years old) who had severe caries and were treated under general anesthesia in 2011 and 2018 were selected and followed up by telephone appointment and clinical examination. Success rate of each treatment was determined and possible factors associated with treatment failure were evaluated.
RESULTS: There were 153 patients (with an average age of 48.55 ± 13.37 months) and a total

BACKGROUND: The exposed pulp has been the topic of numerous studies, but well-designed and well-executed comparative trials on the outcome and treatment of these teeth have been limited.
OBJECTIVES: This study was conducted to answer the following questions: in patients with nontraumatic pulpitis associated with no or nonspontaneous pain in permanent teeth, (i) is direct pulp capping or pulpotomy (partial/full) as effective as selective or stepwise caries removal [Population/participants, Intervention(s), Comparator(s)/control, Outcome(s) (PICO) 1], (ii) is pulpotomy (partial/full) as effecti

OBJECTIVE: There has been increasing evidence indicating rather high success rates of vital pulp therapies. This umbrella review aimed to provide an overview of existing systematic reviews regarding the outcomes of vital pulp therapy, specifically direct pulp capping, partial pulpotomy and full pulpotomy, in carious pulp-exposed permanent human teeth.
MATERIALS AND METHODS: The specific PICO questions were as follows: Population - permanent human teeth with carious pulp exposure; Intervention - direct pulp capping with pulp capping materials; Comparators - partial pulpotomy and full pulpotomy

Objective. This study presents a clinical and histological evaluation of human pulp tissue responses after direct capping using a new dentin adhesive system. Methods. Twenty-eight caries-free third molar teeth scheduled for extraction were evaluated. The pulps of 22 teeth were mechanically exposed and randomly assigned to 1 of 2 groups: Single Bond Universal or calcium hydroxide. Another group of 6 teeth acted as the intact control group. The periapical response was assayed, and a clinical examination was performed. The teeth were extracted after 6 weeks, and a histological analysis was perfor

This study reports a case of complicated crown-root fractures of the maxillary central incisors (teeth 8 and 9) in a 10-year-old boy. Vital pulpotomy and iRoot BP Plus capping were performed to protect the pulp. The fractured crown fragments were used to restore the tooth morphology, and the esthetics and function of both central incisors were restored. The roots of the affected teeth continued to develop normally. Twenty-five months after the initial treatment, the patient suffered another trauma, resulting in a second dislodgement of the fractured crown of the left maxillary central incisor.
