Evolution of clinical pattern of parthenium dermatitis: a study of 74 cases.

Our aim was to study the evolution of clinical pattern of parthenium dermatitis. Patients with clinical picture consistent with parthenium dermatitis for 3 years or more along with positive patch test to parthenium were studied. Patients were questioned regarding the site(s) of dermatitis at the onset and change in localization in the following years. Patients were classified into airborne contact dermatitis, chronic actinic dermatitis (CAD) pattern or mixed pattern dermatitis. There were 74 patients (49 men and 25 women) with an age range of 22-70 years and the mean duration of 7.7 years. 60

[Review of the literature on adverse reactions to sunscreens 1947 to 1989].

Sunscreens in cosmetics may trigger off photoallergic contact dermatitis. In addition, they can induce allergic, irritant, or phototoxic dermatitis. Adverse reactions may also be caused by fragrances, preservatives, and other ingredients in light-protective cosmetics. We give a survey on 87 publications dealing with this issue, which appeared from 1947 through 1989. The survey comprises information on the nature of the sunscreen or other ingredient as well as the type of reaction observed in 357 patients. We also refer to the sex distribution and summarize the reasons for using products absorb

Thoughts on the interpretation of positive photopatch test reactions.

BACKGROUND: Positive photopatch test reactions are classified according to the International Contact Dermatitis Group. The various reaction patterns are interpreted to represent patterns such as contact allergy, photocontact allergy, photoaugmentation, and photoinhibition. OBJECTIVE: To investigate whether there are any weaknesses in the interpretation of reaction patterns. MATERIALS & METHODS: A dermatitis patient with photoallergic contact dermatitis due to ketoprofen was photopatch tested with serial dilutions of ketoprofen in ethanol. The reaction patterns for the various concentrations

Hand/face/neck localized pattern: sticky problems--resins.

Plastic resin systems have an increasingly diverse array of applications but also induce health hazards, the most common of which are allergic and irritant contact dermatitis. Contact urticaria, pigmentary changes, and photoallergic contact dermatitis may occasionally occur. Other health effects, especially respiratory and neurologic signs and symptoms, have also been reported. These resin systems include epoxies, the most frequent synthetic resin systems to cause contact dermatitis, (meth)acrylics, polyurethanes, phenol-formaldehydes, polyesters, amino resins (melamine-formaldehydes, urea-for

Parthenium dermatitis.

Allergic contact dermatitis (ACD) to Parthenium hysterophorus is the most common cause of plant dermatitis in India. Parthenium dermatitis is caused by dry powder of leaves and flowers and hair-like structures (trichomes). Sesquiterpene lactones (SQLs) are the most important allergens responsible for ACD to parthenium. The different patterns include classical airborne contact dermatitis, chronic actinic dermatitis (CAD), exfoliative and widespread dermatitis. There is a definite trend towards a change from an airborne pattern to a CAD pattern in the natural history of parthenium dermatitis. In

Allergic Contact Dermatitis From Methylisothiazolinone in Residential Wall Paint.

A 33-year-old woman presented to our clinic for suspected photoallergic contact dermatitis with a recent episode of severe, vesicular dermatitis involving exposed skin and correlating with relocation to a new home. Biopsy results showed spongiotic and lichenoid dermatitis with eosinophils. Patch test results showed a very strong (+++) reaction to methylisothiazolinone (MI), mild (+) reaction to MI/methylchloroisothiazolinone, and no reaction to benzisothiazolinone. These allergens were found in several personal products. However, the patient was suspicious of 4 wall paints recently used in her

[Quinidine-induced photoallergy. A clinical and experimental study].

Photoallergies are type-IV allergic reactions of a special kind. Photoallergies due to quinidine sulphate were described as early as 1942. We present a case of quinidine sulphate-induced photodermatitis. The photoallergic reaction to quinidine sulphate was reproducible not only after oral intake but also in the photopatch test. In addition, an eczematous dermatitis was provoked after intradermal injection of quinidine sulphate previously exposed to UV-A irradiation in vitro together with serum taken from the patient. No dermatitis was seen with quinidine sulphate irradiated in the absence of s

Afloqualone photosensitivity: immunogenicity of afloqualone-photomodified epidermal cells.

Afloqualone (AQ) is an oral muscle relaxant and evokes ultraviolet A (UVA)-induced photosensitivity dermatitis as a side effect. Histologic observations of the skin eruption suggest that AQ photosensitivity is mediated not only by phototoxic but also by photoallergic mechanisms. To address the immunological mechanisms of AQ photosensitivity, we examined the immunogenicity of AQ-photomodified epidermal cells in mice. Afloqualone was covalently coupled with bovine serum albumin by irradiation with UVA but not UVB. Because of this ability of AQ to photobind to protein, murine epidermal cells were

Standard photopatch testing with Musk Ambrette.

495 patients were standard photopatch tested with Musk Ambrette 5% in petrolatum and in dimethyl phthalate. 4 cases gave positive photopatches; 3 were men and 1 a woman. 2 cases, both men, had current or past relevant reactions to Musk Ambrette presented with typical perfume dermatitis. The other 2 cases had positive reactions to Musk Ambrette of uncertain relevance. Photoallergic reaction to Musk Ambrette is a rare event. Only a few relevant reactions will be missed by excluding it from standard phototesting.

Drug-induced hypersensitivity reactions include phototoxic and photoallergic reactions. Photoallergic reactions, which develop as cell-mediated immune responses to a light-activated compound, are described in this review article. The main topics include photobiology, list of common drugs inducing photoallergic reactions, immune response, clinical features, diagnostic and therapeutic approach. Photoallergic drug reactions can be avoided in most cases if the potential photoallergens are known and appropriate photoprotection is used.

Cutaneous adverse reactions of amiodarone.

Dermatological complications of amiodarone are commonly encountered problems in therapy. The incidence in the population of patients with prolonged use of amiodarone reaches nearly 75% according to various sources. Nevertheless, they are often misdiagnosed or overlooked. The aim of this review is to present the current state of knowledge about skin changes induced by amiodarone, including phototoxic and photoallergic reactions, as well as hyperpigmentation. In most cases, the adverse effects are reversible and disappear after discontinuation of the drug. Although the dermatological complicatio

Photosensitivity: results of investigation in 250 patients.

Between January 1972 and December 1974, 250 patients were referred for investigation of possible photosensitivity to a university-associated clinical research unit for photobiology. In addition to an appropriate history and clinical examination, phototesting was carried out with a solar simulator, monochromatic light and fluorescent light directed to patch-tested areas of skin. Photosensitivity was not demonstrated in 110 patients (44%). Among the laboratory-confirmed photosensitivities, diagnoses included polymorphous light eruption (4.4%), erythropoietic protoporphyria (3.6%), porphyria cuta

Benzophenone 4: an emerging allergen in cosmetics and toiletries?

Chemical ultraviolet (UV) filters have, over the last few decades, been increasingly used not only in conventional sunscreen products but also in many cosmetics and toiletries. Allergic contact dermatitis as well as photoallergic contact dermatitis reactions have been well documented as a consequence of such use. Over a 3-year period, we recorded the number of positive patch test reactions to a selection of chemical UV filters that we added to our usual cosmetics/facial series. Our objective was to investigate whether any of these filters produced significant numbers of positive patch test res

[When sunscreens do not help: allergic contact dermatitis to UV filters].

Ultraviolet (UV) filters may cause allergic and more frequently photoallergic contact dermatitis. Therefore, a photopach test should always be performed in case of a suspected contact sensitivity to UV filters. We report a case of a 65-year-old woman with a recurrent erythema of the face and décolleté after sun exposure despite application of a sunscreen. The (photo)patch test revealed a contact sensitivity to the UV filter butyl-methoxybenzoylmethane. Treatment with a topical glucocorticoid and avoidance of the particular UV filter led to a rapid improvement.

Evaluation of the photosensitive patient.

Evaluation of photosensitivity involves the following sequential steps. A history is useful to evaluate possible exposure to know photosensitizers, characteristics of the light-induced eruption, family history, and possible involvement of other organ systems. Distribution and morphology of the cutaneous lesions should help to delineate the disorder. Phototesting is helpful in documenting decrease in the minimal erythema doses. Photopatch testing is indicated when history and physical examination suggest the possibility of photoallergic dermatitis. Other useful investigations include skin biops

Photosensitivity due to a fluorouracil derivative.

A photosensitive lichen planus-like eruption developed in a 63-year-old man during postoperative chemotherapy with 1-(2-tetrahydrofuryl)-5-fluorouracil. The action spectrum for the photosensitivity was in the long-wave ultraviolet light (UV-A). The reaction was reproducible on readministration of the drug and exposure to UV-A. At the time of this test, a flare-up phenomenon was observed at previously involved sites. The photopatch test was negative with the drug. Patients taking the drug who had no dermatitis were not sensitive to UV-A. These observations suggest that the photosensitivity that

Sunscreens: A Review of UV Filters and Their Allergic Potential.

Abstract Active ingredients of sunscreens, or UV filters, have increased in use because public awareness of sun safety has risen. In addition to this intentional use, unintentional exposures to UV filters also occur through application of personal care products, where the filters are incorporated into the product. There are 2 main types of UV filters: organic (chemical) filters and the 2 inorganic (mineral) filters, titanium dioxide and zinc oxide. Both allergic contact dermatitis (ACD) and photoallergic contact dermatitis (PACD) have been caused by organic filters; oxybenzone (benzophenone-3)
