Atopic dermatitis and contact dermatitis are among the most common dermatologic conditions encountered by the primary care clinician. Their presentation varies and can mimic infectious and other noninfectious conditions, making a thorough history essential, with particular emphasis on exposure to environmental and occupational irritants, as well as recognition of commonly affected sites. For all of these conditions, control of symptoms is often the goal of treatment, starting with general supportive care measures like frequent emollient use and often relying on topical steroidal and nonsteroid

We report the case of a 67-year-old woman with a painful expanding ulcer on the left leg of 2 months' duration that initially was diagnosed as a skin and soft tissue infection (SSTI) with plans for surgical debridement. Upon dermatologic consultation, surgery was canceled due to the possible diagnosis of pyoderma gangrenosum (PG) and risk of pathergy. Notable features of this patient included a history of inflammatory bowel disease (IBD), poor response to antibiotics, chronicity, lack of signs of sepsis, potential complications of surgical intervention, and ultimately a response to immunosuppr

Postinflammatory hyperpigmentation (PIH) is a reactive hypermelanosis that develops following cutaneous inflammation. Common causes of PIH include intrinsic skin conditions (e.g., acne and eczema) as well as external insults to the skin, such as burn injuries and dermatologic procedures. PIH more commonly occurs in individuals with darker skin, for whom it is often a source of significant psychological distress. Several therapeutic modalities are available for the treatment of PIH, including topical agents, chemical peels, and energy-based devices. We review the epidemiology, clinical presenta

Biologic agents, or protein-based drugs derived from living cells, are becoming commonplace in dermatology for the treatment of psoriasis, but their use spans many dermatologic conditions beyond psoriasis and psoriatic arthritis. In recent years, there has been considerable interest in their use for other inflammatory skin diseases. This article will review currently available data and reports on the off-label use of biologics in the treatment of cutaneous diseases other than psoriasis, including inflammatory dermal processes, autoimmune bullous skin diseases, connective-tissue diseases, and h

Immune checkpoint inhibitors (ICIs) are among the most promising drugs in the field of immuno-oncology; they represent monoclonal antibodies that modulate the effects of immune checkpoints, such as Programmed Cell Death protein 1 (PD-1) which is a co-inhibitory signal responsible for immune suppression. Dermatologic immune-related toxicity is a common type of toxicity associated with these agents. However, oral skin reactions are rare. Herein, we describe an unusual case of a patient that developed oral lichen planus during treatment with anti-PD-1 inhibitor nivolumab.

Colorectal cancer is the second leading cause of cancer deaths worldwide, with a 5-year relative survival of 14% in patients with metastatic colorectal cancer (mCRC). Patients with BRAF V600E mutations, which occur in ∼10%-15% of patients with mCRC, have a poorer prognosis compared with those with wild-type BRAF tumours. The combination of the BRAF inhibitor encorafenib with the epidermal growth factor receptor inhibitor cetuximab currently represents the only chemotherapy-free targeted therapy approved in the USA and Europe for previously treated patients with BRAF V600E-mutated mCRC. INTRODUCTION: Chronic pruritus is a common symptom that arises from both dermatologic and non-dermatologic conditions including chronic kidney disease, cholestasis, lymphoma and neuropathy. Over the past decade, research has elucidated many of the receptors, neuropeptides and cytokines involved in itch sensation and transmission. In addition, the first biomarker for cholestatic itch has been discovered. These findings have led to the development of a host of novel antipruritic medications, both on the market and in the pipeline.
AREAS COVERED: A summary of new and emerging treatments for prur

Various mechanisms have been argued for skin wrinkling, one of the hallmarks of aging. We hypothesize that chronic sympathetic bias is a previously unrecognized mechanism for wrinkling. In the acute setting of water immersion, reversible skin wrinkling is a well-known reflex mediated by the autonomic nervous system. We postulate that skin wrinkling results as a local maladaptive manifestation of a global chronic sympathetic bias that emerges during aging. The persistence of such changes may induce additional compensatory remodeling to cause permanent alteration of the skin. Sympatholytic agent

Photodynamic therapy (PDT) is used for the prevention and treatment of non-melanoma skin cancer. Until recently, clinically approved indications have been restricted to actinic keratoses, nodular and superficial basal cell carcinoma, and--since 2006--Bowen disease. However, the range of indications has been expanding continuously. PDT is also used for the treatment of non-malignant conditions such as acne vulgaris and leishmaniasis, as well as for treating premature skin aging due to sun exposure. Here, PDT is used for the stimulation of immunomodulatory effects in contrast to the induction of

Anesthesia is an essential component of dermatologic procedures, influencing pain management and patient outcomes, including wound healing, infection control, and cosmetic appearance. This review examines the impact of various anesthetic techniques, topical, local, regional, and general, on dermatological outcomes. The findings reveal that while local anesthesia is preferred due to its efficacy and safety, specialized considerations are necessary for pediatric, geriatric, and high-risk patients. Anesthesia-related complications, such as allergic reactions, systemic toxicity, and delayed healin

BACKGROUND: There is limited data on the use of dual or combination biologic therapy in dermatological patients, particularly in psoriasis or with other coexisting dermatological conditions. However, this may be necessary for a small proportion of patients.
METHODS: We conducted a retrospective review of 46 patients treated with dual or combination biologic therapy at a single dermatology practice in Sydney, Australia. The patient cohort consisted of 44 patients with psoriasis, 1 patient with hidradenitis suppurativa and 1 patient with a combination of hidradenitis suppurativa and psoriasis.

Mycophenolate mofetil (MMF) is a relatively new systemic immunosuppressive agent whose use is rapidly increasing within the field of dermatologic. Originally used in the 1970s for the treatment of psoriasis, MMF has demonstrated efficacy in multiple types of inflammatory skin diseases. Although the safety data within the dermatologic literature is sparse, MMF's extensive use within the field of organ transplantation has demonstrated a favorable safety profile. MMF's lack of hepatic or renal toxicity is a significant advantage over immunosuppressive medications currently used in dermatology and

BACKGROUND: Monoclonal antibodies encompass an increasingly important treatment for a variety of dermatologic conditions including hidradenitis suppurativa (HS). The high failure rate and cost of anti-tumor necrosis alpha (TNF-α) agents and emergence of biologic treatments critically warrant treatment strategies that identify treatment failures early and optimize therapy. This review’s primary objective is to understand the current literature on biologic therapeutic drug monitoring (TDM) used in chronic inflammatory diseases and apply this knowledge to future dermatologic studies a

The properties of engineered nanomaterials and nanoparticles such as zinc oxide and titanium dioxide may differ substantially from naturally occurring materials and particles. Nanoparticles have unique physical properties making them ideal for use in various skin care products currently on the market. Nano-preparations are currently under investigation as novel treatments of acne vulgaris, recurrent condyloma accuminata, atopic dermatitis, hyperpigmented skin lesions, and other non-dermatologic diseases. Because of their increased surface area, nanoparticles have increased reactivity and a sma

Sulfur has antifungal, antibacterial, and keratolytic activity. In the past, its use was widespread in dermatological disorders such as acne vulgaris, rosacea, seborrheic dermatitis, dandruff, pityriasis versicolor, scabies, and warts. Adverse events associated with topically applied sulfur are rare and mainly involve mild application site reactions. Sulfur, used alone or in combination with agents such as sodium sulfacetamide or salicylic acid, has demonstrated efficacy in the treatment of many dermatological conditions.

AIM: Biologic agents play a pivotal role in controlling both articular and cutaneous manifestations of psoriatic arthritis (PsA). Although international guidelines emphasize the appropriate use of biologics, Korea lacks standardized treatment protocols tailored for PsA across specialties. We therefore aimed to examine specialty-specific prescription patterns and treatment retention using nationwide claims data.
METHODS: Health Insurance Review and Assessment (HIRA) claims data from 2011 to 2022 were analyzed, including patients with PsA who initiated treatment with their first biologic agent.

Biological agents are widely used in rheumatology, dermatology and inflammatory bowel disease. Evidence about their efficacy and safety has been strengthened for all those therapeutic indications over the last decade. Biosimilar agents are monoclonal antibodies similar to previously approved biologics. In the European Union, they have been approved for all the indications in the management of immune-mediated inflammatory diseases (IMIDs), although data only in rheumatoid arthritis and ankylosing spondylitis are currently available. Direct evidence on efficacy, safety, and immunogenicity of bio
