[Endocrine crises].

Endocrine crises can occur in diabetes mellitus, in pituitary failure when there is a lack of ACTH, TSH or ADH secretion, in severe hyper- or hypothyroidism (thyroid storm and myxedema coma), severe hyper- or hypoparathyroidism (parathyroid crisis and tetany), in adrenal failure and in patients with pheochromocytoma or carcinoid tumors. Cushing's syndrome can be associated with psychotic crises. This review describes the most important clinical features and the basic diagnostic and therapeutic aspects of the non diabetic endocrine crises.

Diabetes mellitus and hypopituitarism.

A 38-year-old diabetic woman developed hyponatraemia and fatal non-ketotic coma after elective cholecystectomy. At the autopsy, it was revealed that the immediate cause of death was cerebral oedema with secondary pontine haemorrhage. The cerebral oedema was associated with severe hyponatraemia and atrophy of the endocrine organs, including the adrenal glands. Biochemical analysis of serum taken immediately before death indicated that the primary defect was pituitary insufficiency, a recognised but rare complication of diabetes.

[Pelvic bone fractures in middle-aged and old persons].

Patients having pelvic fractures make 5% of all patients over 60 years of age with bone fractures. Transport traumas (traffic accidents) are predominating. Of 63 persons shock of a different degree due to massive intratissue blood loss was observed in 17. Three objects from this group died as a result of uncompensated shock (1) and septic complications (2). Forty six patients were admitted in a satisfactory condition, three of them died from concomitant complications (pneumonia, diabetic coma).

[Hyperosmolar coma in a premature newborn infant: iatrogenic complication of parenteral feeding].

The authors report the case of a 34-week preterm newborn weighing 1,545 g receiving a parenteral mixture by a central venous catheter. A mistake in the flow-rate program led to the infusion of 200 ml within 2 h. Coma occurred quickly and the patient showed intracellular dehydration, polyuria, metabolic acidosis, hyperglycemia and hyponatremia. The therapeutic aim was the correction of dehydration and acidosis, as well as the prevention of secondary hypoglycemia. Clinic and biologic symptoms disappeared 6 h later Volumetric infusion pumps need permanent safe and accurate control of volume flow-

A paper which changed clinical practice (slowly). Jacob Holler on potassium deficiency in diabetic acidosis (1946).

It is often said that the introduction of insulin into clinical medicine made a 'dramatic' difference to the mortality resulting from diabetic coma. This is true in the sense that before 1922 it was almost uniformly fatal, but until the 1950s the mortality in many large hospitals was as high as 30-50%. Often autopsy did not establish a cause of death. Many may have been a result of hypokalaemia, a complication which was not recognized until 1946; in that year in the Journal of the American Medical Association, Jacob Holler described a patient who developed respiratory paralysis 12h into treatm…

[Diabetes prevalence in a school population of Avellaneda, Argentina].

An interview system was used to survey 56,199 students in Avellaneda between the ages of 3 and 20 years, representing 60.6% of the populatio … ntries. The most frequent initial symptoms were the typical ones, with only 15% showing acidosis or diabetic coma as the initiation of the illness. Viral infections and stress appeared to be related to the onset of the disease. Diabetes was in the family history of 48.5% of the diabetic children, and 24.5% of the non-diabetics. The most common diabetic relative …

Atypical diabetic ketoacidosis: case report.

BACKGROUND: Diabetic ketoacidosis (DKA) is a potentially life-threatening complication of diabetes mellitus and can lead to death if untreated. It is a complex metabolic state characterised by hyperglycaemia, acidosis and ketonuria. Bonsai is one of the herbal incense products that contains synthetic cannabinoid and can be easily accessible via the internet in many countries. It cannot be detected in blood and urine studies using conventional methods. Synthetic cannabinoid abuse is associated with severe side effects, including tachycardia, high blood pressure, acidosis, excess sedation and co

Heat-related mortality burden of type 1 diabetes, type 2 diabetes, and diabetes complications in mainland China amid global warming: a nationwide, case-crossover study.

BACKGROUND: Rising global temperatures and diabetes pose growing health risks worldwide. Individuals with diabetes are particularly vulnerab … el for overall diabetes, primary diabetes subtypes (type 1 and type 2), and specific complications (diabetic coma, diabetic ketoacidosis, diabetic nephropathy, and diabetes with peripheral vascular disease [PVD]). We examined how the associations varied across the temperate continental, temperate monsoon, and subtropical monsoon zones. The future heat-attributa …

Hypoglycemia induced by hydroxychloroquine in a type II diabetic treated for polyarthritis.

A 77-year-old man with type II diabetes taking a stable dose of subcutaneous, twice daily human insulin developed symmetrical, inflammatory, rheumatoid factor positive polyarthritis. Within 2 weeks of starting therapy with prednisone 5 mg daily and hydroxychloroquine 400 mg daily he had 2 episodes of severe hypoglycemic coma requiring emergency care. His blood glucose became controlled again when his insulin was decreased by 37%. There are no reported cases of hypoglycemia in diabetic or nondiabetic patients treated with hydroxychloroquine. Hydroxychloroquine has been reported to reduce insuli

History of the team approach to amputation prevention: pioneers and milestones.

BACKGROUND: This historical perspective highlights some of the pioneers, milestones, teams, and system changes that have had a major impact on the management of the diabetic foot during the past 100 years. In 1934, American diabetologist Elliott P. Joslin noted that mortality from diabetic coma had fallen from 60% to 5% after the introduction of insulin, yet deaths from diabetic gangrene of the lower extremity had risen significantly. He believed that diabetic gangrene was preventable. His remedy was a team approach that included foot care, diet, exercise, prompt treatment of foot infections, …

Oral myiasis complicating endotracheal intubation in a diabetic patient: a case report.

Myiasis is a rare parasitic disease caused by infestation of dipterous fly larvae into living human or animal tissues. Although nosocomial myiasis has been occasionally reported in critically ill patients, cases of oral myiasis in patients with diabetic ketoacidosis (DKA) coma undergoing tracheal intubation remain rarely reported in South China. Herein, we report a clinical case of a 77-year-old male with type 2 diabetes mellitus who developed oral myiasis after emergency tracheal intubation for DKA coma. Comprehensive therapeutic strategies were implemented, including manual removal of larvae

[Progress and developments in insulin therapy].

BANTING and BEST revolutionized diabetes therapy with the discovery of insulin 57 years ago. Since then, progress in this area has been slow despite tremendous reseach efforts. The subcutaneous injection of a depot insulin does not provide optimal control of blood sugar. True progress has been brought about by intravenous insulin administration for the management of diabetic coma. The authors do not recommend ultra low dose therapy. The prognosis of diabetic coma is much better than 20 years ago, in particular because of much improved and continuous supervision of the circulation (CVP, ECG, K+…

Foot drop in children with newly diagnosed type 1 diabetes: three case reports.

SUMMARY: The etiology of foot drop is diverse from various diseases to mechanic injuries and includes neuropathy of the peroneal nerve. Peroneal neuropathy might also be one of the forms of diabetic neuropathy, very rarely reported as the first sign of diabetes. We describe three cases of children with newly diagnosed type 1 diabetes (TID) who developed unilateral peroneal nerve palsies and tibial nerve palsies, presenting clinically as a foot drop. In two of our cases, the symptoms of foot drop occurred shortly after starting treatment for severe diabetes ketoacidosis. In the third patient, f

Persistent Vegetative State after Diabetic Ketoacidosis Triggered by an Overdose of Psychotropic Agents.

A 28-year-old man with type 1 diabetes mellitus was admitted for shock and coma due to diabetic ketoacidosis. Despite aggressive treatment and management of the patient's underlying clinical issues, the patient remained in a comatose state. Further investigations revealed an excess consumption of psychotropic agents; however, there was no evidence of an insulin overdose. Physicians should be aware that, in patients who are highly dependent upon insulin, an overdose of psychotropic agents can lead to hypoxic-ischemic brain injury.
