OBJECTIVES: The aim of this study was to evaluate changes in aortic growth rate and factors influencing aneurysmal dilatation after uncomplicated acute type B aortic dissection (ABAD).
METHODS: Medically treated patients with uncomplicated ABAD between September 2004 and January 2020 were retrospectively reviewed. Diameters of 6 different sites in the descending aorta were measured and aortic growth rate was calculated according to the time interval. Factors associated with aneurysmal changes were also investigated.
RESULTS: This study enrolled a total of 105 patients who underwent >2 serial

BACKGROUND: Mega-aortic syndrome including aortic arch and descending aortic aneurysm is a challenging surgical case. Because the aorta continuously dilates, creating the distal anastomosis sites becomes an issue. Despite the developments in endovascular techniques including frozen elephant trunk, in the case of mega-aortic syndrome or mycotic aneurysm, extensive surgical repair is still a strong armamentarium. Our patient had a mega-aorta with chronic aortic dissection. Herein, we show tips regarding concurrent ascending, aortic arch, and descending aortic replacement via posterolateral thora

The so-called "inflammatory" aneurysm of the aorta of the abdominal wall is a special form of the arteriosclerotic aneurysm of the aorta observed in 5-15% of the abdominal aorta aneurysms. It is characterized by more or less marked periaortal fibrotization. The pathological correlate is represented by lymphocytous and plasmacellular infiltrates that are mainly seen and identified in the region of the adventitia. The etiology has not yet been clarified. Since surgery of the inflammatory aneurysm of the abdominal wall aorta is rendered difficult due to neighbouring organs (e.g. ureters) becoming

We report on a patient with Marfan syndrome who presented a Stanford type B dissection of the descending thoracic aorta in late pregnancy. After a cesarean section, the patient presented a severe obstruction of the mesenteric superior artery. An endovascular fenestration was performed (balloon and guidewire based fenestration). Computed tomography (CT) angiography revealed an intussusception 'like' image of the abdominal aortic layers as a consequence of the fenestration procedure. Because of aneurismal progression in the abdominal aorta, surgical repair of the abdominal aorta and intussuscept

We report a patient who developed a type B aortic dissection and ruptured his aneurysmal sac 1 year after endovascular abdominal aortic aneurysm repair (EVAR), despite standard follow-up. This 79-year-old man was presented to emergency room with acute abdominal pain and an acute lower limb ischemia. Computed tomography scan showed an acute type B aortic dissection feeding the aneurysmal sac of the EVAR. The aneurysm rupture occurred during imaging. Type B aortic dissection is a rare cause of aneurysmal rupture after EVAR. The first postoperative computed tomography scan should maybe include th

Aortic dissection is a rare but fatal emergency. It typically presents with sudden, severe chest pain or back pain, but can manifest atypically, complicating the diagnosis. We report a case of a 44-year-old woman who is a thalassemia carrier and presented with a seizure, without preceding chest or back pain. Computed tomography (CT) angiography revealed a penetrating atherosclerotic ulcer causing a dissection originating from the mid aortic arch, with retrograde extension into the proximal innominate artery and inferior extension down the descending thoracic and abdominal aorta, terminating at

We report a case in which a stent graft and an Amplatzer® vascular plug (AVP) were effective for the treatment of chronic aneurysmal aortic dissection. The patient was a 52-year-old man. At 45 years of age, he developed acute aortic dissection, for which he underwent surgery 4 times with prosthetic graft replacement in the abdominal aorta, descending thoracic, ascending aorta (without neck branch reconstruction), and thoracoabdominal aorta with the reconstruction of the celiac, superior mesenteric, and bilateral renal arteries. At the time of thoracoabdominal aortic surgery, strong adhesion wa

INTRODUCTION: Coral reef aorta (CRA) is a rare clinical entity characterised by hard, protruding calcifications in the juxta and supra-renal aorta, which cause haemodynamically significant stenosis of the aorta and its branches. We are presenting a case report of a 65-year-old female patient with bilateral 30-meter claudication on both lower extremities and a haemodynamically significant stenosis of the left renal artery.
CASE REPORT: The patient underwent computed tomography angiography (CTAG) of the abdominal aorta and lower extremities. CTAG revealed severe abdominal aortic wall calcificat

INTRODUCTION: Pelvic and inguinal pain are a rare manifestation of arterial disease.
METHODS: Description of four patients with acute or chronic pelvic or inguinal pain due to symptomatic dissection of the infrarenal aorta and/or iliac arteries.
RESULTS: In two cases the dissection was limited to the left iliac artery with an entry at the beginning of the common iliac artery. The re-entry was located in the distal external iliac or common femoral artery. In one patient additionally a infrarenal abdominal aortic aneurysm was found. In the other two patients the entry of the spontaneous dissec

A successful replacement of the aorta from the ascending aorta to the supraceliac abdominal aorta in one operation is herein reported. The patient was 66-year-old woman who had DeBakey type I chronic aortic dissection with a pre-disseminated intravascular coagulation (DIC) condition. The image diagnosis revealed a markedly enlarged false lumen with a narrow true lumen in the ascending aorta extending to the diaphragm level while below the diaphragm level, the aortic dilatation was mild. The operation was performed through both a thoracoabdominal incision and a median sternotomy to expose the w

A sound knowledge of variations in the branching pattern of the abdominal aorta is crucial for operative, diagnostic, and endovascular procedures. We report a unilateral anomalous lateral branch of the abdominal aorta observed during dissection classes. This anomalous branch took origin from the anterior surface of the abdominal aorta behind the left renal vein and pierced the left renal vein. After piercing the left renal vein, it traced a tortuous course through the peri-renal fat pad in front of the lower part of the left kidney. The artery then ran downward and laterally, pierced the anteri

We present the case of a 68 year old Caucasian woman, in extremis, with left hemiparesis and right hemothorax, in hypovolemic shock, secondary to a blow-out of a large penetrating ulcer at the junction of innominate trunk and aortic arch. She underwent interposition graft replacement of innominate trunk and repair of aortic arch, on cardiopulmonary bypass, employing total circulatory arrest and selective antegrade cerebral perfusion and had total resolution of hemiparesis. She, however, represented, 6 months later, with threatened exsanguination after a sternal wire cheese-wired through the st

Aortic dissection is a common complication of Marfan's syndrome. We report such a patient who survived two separate dissections which both occurred shortly after uneventful pregnancies. The first dissection was a self-limiting DeBakey type III but the second commenced in the left common iliac artery and extended retrogradely into the lower abdominal aorta. This latter type of dissection does not appear to have been previously reported.

We report a rare complication of acute onset paraplegia after repair of an abdominal aortic aneurysm in a patient with acute type B aortic dissection. A 53-year-old man, suffering from abdominal aortic aneurysm (AAA), was admitted to our hospital with type IIIB acute aortic dissection. Ten days after admission, emergency Y-grafting was performed for impending rupture of the AAA. Twenty hours after Y-grafting, weakness of his lower extremities developed and progressed to paraplegia. Enhanced computed tomography scan revealed expansion of a thrombosed false lumen at the thoracoabdominal aorta, r

We performed simultaneous graft replacement of the total aortic arch and ascending aorta for type A aortic dissection with a patent false lumen extending through the arch into the descending or abdominal aorta. During the past 7 years, this procedure was performed in 42 patients (28 men and 14 women), aged 20 to 72 years (mean age, 50 years). Nineteen patients underwent the procedure during the acute period, and 23 during the chronic period. The site of the initial intimal tear was the ascending aorta in 17 patients and the transverse aortic arch in 25 patients. Artificial graft replacement wa

The dissection is termed Type A according to the Stanford classification, if the ascending aorta is involved. It is termed type B, if the ascending aorta is not involved. Most patients with Type A aortic dissection die from intrapericardial rupture with cardiac tamponade, free pleural rupture, massive aortic regurgitation, or coronary or cerebral malperfusion (ischemic heart disease or stroke). Most patients with Type B dissection die from free pleural rupture or renal or visceral vascular complications. The resultant compromise of various aortic branches (inomunate, carotid, subclavian, spina
