Comparison of Disulfiram and Naltrexone in Cases of Alcohol Dependence Syndrome.

BACKGROUND: Relapse prevention in alcoholism is recognised as an important component of management. Use of pharmacotherapies to prevent relapse in combination to psychological intervention is emerging. Disulfiram and Naltrexone are two of three FDA approved drugs for pharmacotherapy. The aim of the study is to compare the effectiveness of these two drugs in preventing relapse in alcohol dependence syndrome cases. METHODS: A prospective crossectional study was conducted to compare disulfiram and naltrexone in alcohol dependent patients in tertiary institution. Cases of alcohol dependence syndr…

Fatal fulminant hepatic failure related to the use of disulfiram.

We report a case of fatal fulminant hepatic failure related to the use of disulfiram. This is a commonly used medication; however there are few reported cases in the medical literature of fatal liver failure related to its use. Patients using disulfiram for alcohol cessation typically have multiple risk factors for liver disease and are not acutely candidates for orthotopic liver transplant due to recent alcohol dependence. This case demonstrates a rare adverse reaction to a commonly used medication with a fatal outcome. Our patient was a sixty-six year old man who had recently started using d…

ANKK1 and DRD2 pharmacogenetics of disulfiram treatment for cocaine abuse.

OBJECTIVE: Disulfiram is a potential cocaine addiction pharmacotherapy. Since dopamine deficiency has been found with cocaine addiction, our objective was to examine whether functional variants in the ankyrin repeat and kinase domain-containing 1 (ANKK1) and/or the dopamine receptor D2 (DRD2) genes interact with response to treatment with disulfiram. MATERIALS AND METHODS: Cocaine and opioid codependent (DSM-IV) patients were stabilized on methadone and subsequently randomized into treatment groups - disulfiram (250 mg/day, N=31) or placebo (N=37). They were genotyped for ANKK1 (rs1800497) an…

Supervised disulfiram in the treatment of alcohol use disorder: a commentary.

BACKGROUND: This commentary discusses the systematic review "The efficacy of disulfiram for the treatment of alcohol use disorder (AUD)" by Jørgensen and colleagues (2011, Alcohol Clin Exp Res DOI: 10.1111/j.1530-0277.2011.01523.x). The main focus of the commentary is on long-term effects, long-term use, and psychotherapeutic application of supervised disulfiram. METHODS: A brief qualitative overview is given of previous and recent clinical studies on disulfiram in alcoholism treatment. RESULTS: The alcohol deterrent disulfiram is an effective pharmacological adjunct to the treatment of AUD …

Tolerance to disulfiram induced by chronic alcohol intake in the rat.

BACKGROUND: Disulfiram, an inhibitor of aldehyde dehydrogenase used in the treatment of alcoholism, is an effective medication when its intake is supervised by a third person. However, its therapeutic efficacy varies widely, in part due to the fact that disulfiram is a pro-drug that requires its transformation into an active form and because it shows a wide range of secondary effects which often prevent the use of doses that ensure full therapeutic effectiveness. In this preclinical study in rats we report the development of tolerance to disulfiram induced by the chronic ingestion of ethanol, …

Naltrexone and disulfiram in patients with alcohol dependence and current depression.

OBJECTIVE: Although disulfiram and naltrexone have been approved by the Food and Drug Administration for the treatment of alcoholism, no medications have been approved for individuals with alcohol dependence and comorbid psychiatric disorders. In particular, the effect of these medications on alcohol use outcomes and on specific psychiatric symptoms is still unknown in patients with the most common co-occurring disorder, major depression. METHOD: Two hundred fifty-four patients with a major Axis I psychiatric disorder and comorbid alcohol dependence were treated for 12 weeks in an outpatient …

Stimulation of the Ca(2+)-ATPase of sarcoplasmic reticulum by disulfiram.

Disulfiram [bis(diethylthiocarbamoyl)disulphide] has been found to stimulate reversibly the Ca(2+)-ATPase of skeletal muscle sarcoplasmic reticulum. At pH 7.2, 2.1 mM ATP and 25 degrees C, ATPase activity was found to double on addition of 120 microM disulfiram. Stimulation fitted to binding of disulfiram at a single site with a Kd of 61 microM. Disulfiram had no effect on the Ca2+ affinity of the ATPase or on the rate of phosphorylation of the ATPase by ATP, but increased the rate of dissociation of Ca2+ from the phosphorylated ATPase (the transport step) and increased the rate of dephosphory…

Photolysis of sulfiram: a mechanism for its disulfiram-like reaction.

Sulfiram, a drug applied topically to treat scabies, produces effects similar to those of disulfiram after subsequent ingestion of ethanol. Disulfiram, used in aversion therapy in the treatment of alcoholism, inhibits hepatic aldehyde dehydrogenase (ALDH) causing an accumulation of acetaldehyde after ethanol ingestion. The increased tissue levels of acetaldehyde cause a spectrum of undesirable side-effects including flushing, nausea, vomiting, and tachycardia, which are referred to as the disulfiram reaction. Previous studies have shown that in vitro sulfiram is a very weak inhibitor of ALDH, …

Disulfiram hepatotoxicity: a consideration of its mechanism and clinical spectrum.

Disulfiram-induced hepatotoxicity is a recognised entity despite the difficulty in determining its contribution to the damage when alcoholism itself is a frequent cause of liver disease. Less well characterised is the mechanism of disulfiram-induced hepatotoxicity and the high fatality rate associated with this entity. The clinical spectrum of the disease is reviewed and experimental data suggesting the mechanism of toxicity are presented. Failure to recognise this problem and to terminate therapy as soon as possible may result in death due to massive hepatic necrosis.

Disulfiram neuropathy: a review (1971-1988) and report of a case.

Neuropathy is one of the most severe side effects of disulfiram therapy. We report the case of a young man who developed a neuropathy following disulfiram administration, with a virtually complete recovery in 14 months. We then discuss 37 cases of disulfiram neuropathy reported since 1971. Evidence is given that: (1) there is no numerical sex prevalence, although the incidence of the disease in women is probably disproportionately high; (2) symptom onset latency is dose-dependent, being longer at 250 mg/day or less; (3) neurological deficits are also dose-dependent, being milder at 250 mg/day …

Excretion of TTCA in human urine after administration of disulfiram.

2-Thiothiazolidine-4-carboxylic acid (TTCA) was measured in urine of two persons given an oral dose of 250 mg disulfiram. Maximum concentrations up to 0.33 mmol TTCA/mol creatinine were found, 10 to 14 h after dosing. The total amounts of TTCA excreted in urine were 1.8 and 3.5 mumol, respectively, which correspond to 0.2 and 0.4% of the dose.

Repurposing Disulfiram for Cancer Therapy: Mechanistic Insights and Translational Challenges.

Disulfiram (DSF), long used as an aversive agent in alcohol dependence therapy, has recently regained attention as a promising candidate for oncological drug repurposing. After administration, DSF is rapidly reduced to diethyldithiocarbamate (DDC), which, in the presence of Cu2+, forms the complex Cu(DDC)2. This compound acts as a strong inducer of oxidative stress, an inhibitor of the ubiquitin-proteasome system, and a suppressor of endogenous hydrogen sulfide (H2S) synthesis. DSF also modifies protein and non-protein thiol groups, disrupting cancer cell metabolism and promoting apoptosis. De…

[Disulfiram alleviates cardiac hypertrophic injury by inhibiting TAK1-mediated PANoptosis].

The study aims to examine the effects and potential mechanisms of disulfiram (DSF) on cardiac hypertrophic injury, focusing on the role of transforming growth factor-β-activated kinase 1 (TAK1)-mediated pan-apoptosis (PANoptosis). H9C2 cardiomyocytes were treated with angiotensin II (Ang II, 1 µmol/L) to establish an in vitro model of myocardial hypertrophy. DSF (40 µmol/L) was used to treat cardiomyocyte hypertrophic injury models, either along or in combination with the TAK1 inhibitor, 5z-7-oxozeaenol (5z-7, 0.1 µmol/L). We assessed cell damage using propidium iodide (PI) staining, measured …
