Diverticular disease of the colon occurs quite frequently in developed countries, and its prevalence has recently increased in Japan. The appearance of diverticulosis increases with age, although mostly remaining asymptomatic. Approximately 20% of cases require treatment. As the Western lifestyle and number of elderly people increase, the need for medical treatment also increases. Computed tomography (CT) is the gold standard for diagnosing diverticulitis. Complicated diverticulitis is classified by the size and range of abscess formation and the severity of the peritonitis. Each case should b…

Uncomplicated diverticulitis is localized diverticular inflammation, whereas complicated diverticulitis is diverticular inflammation associated with an abscess, phlegmon, fistula, obstruction, bleeding, or perforation. Patients with acute diverticulitis may present with left lower quadrant pain, tenderness, abdominal distention, and fever. Other symptoms may include anorexia, constipation, nausea, diarrhea, and dysuria. Initial laboratory studies include a complete blood count, basic metabolic panel, urinalysis, and measurement of C-reactive protein. Computed tomography, the most commonly perf…

Terminal ileum diverticulitis is a very rare disease that may lead to an acute abdomen mimicking appendicitis. Because of its rarity, an optimal treatment for this disease has not been established yet. In this study, we evaluated the clinical features and outcomes of nonoperative management of terminal ileum diverticulitis, including suspected perforation. From 2000 to 2007, 346 patients were treated for acute symptomatic right-sided diverticulitis. Radiographic evaluation revealed that nine patients (2.6%) had terminal ileum diverticulitis. All patients presented with pain in the right lower …

Solitary caecal diverticulitis is a rare and often misdiagnosed cause of abdominal pain. A 63-year-old Caucasian woman was admitted with a 3-day history of left upper quadrant pain and constipation. Preoperative imaging identified a possible transverse colonic tumour. At laparotomy a long, mobile ascending colon resulted in the caecal pole lying in the left upper quadrant and an inflamed gangrenous solitary caecal diverticulum was found. A right hemicolectomy was performed and the patient recovered promptly.

The clinical diagnosis of diverticulitis is often uncertain and frequently incorrect. Diagnostic imaging such as with helical CT offers a rapid and accurate diagnosis of diverticulitis and its complications as well as alternative conditions. In particular, helical CT combined with contrast material administered through the colon is highly accurate and can be obtained quickly. CT signs of diverticulitis include focal inflammatory wall thickening and paracolic inflammation superimposed on diverticular disease (diverticula, muscular wall hypertrophy). Common alternative conditions that can clinic…

Thirteen (13) patients with proven diverticulitis are presented with the aim of demonstrating the current evaluation and management. Radiological evaluation were obtained with plain abdominal x-rays and computed tomography (CT) in all cases, abdominal ultrasonography (US) in 8 cases and contrast enema in 5 patients. Radiological percutaneous abscess drainage (PAD) were performed in 5 cases, two of which preceded surgery. A clinical suspicion of diverticulitis was made in only 3 of the 13 cases. CT provided the diagnosis in all cases and helped in directing the appropriate management. Ultrasoun…

This study demonstrates that elective colon surgery for diverticular disease is benign, with a low mortality and morbidity. The acute severe complications of the colonic diverticular disease are characterized by a large mortality and morbidity. The results reported here, as all those published recently on the same subject, are in favor of broadening the indications of colectomy in diverticular disease, before the occurrence of a severe acute complication.

BACKGROUND: Colonic diverticulitis in the young has been considered to have a virulent course, high morbidity, and high operative rate. As a result, elective resection of the involved colonic segment after the first clinical episode has been the usual practice.
PURPOSE: This study presents our experience with acute diverticulitis in the young.
METHODS: In the last nine years, 63 patients younger than 45 years were treated for acute diverticulitis at our institution. A retrospective review was performed to determine the clinical course and outcome of these patients.
RESULTS: Clinical present…

BACKGROUND: Acute diverticulitis of the right colon is a rare condition with a higher incidence in Oriental populations than in Occidental populations.
PATIENTS AND METHODS: A retrospective review was conducted between 1982 and 1993 on 22 surgically treated Chinese patients (14 men, 8 women; mean age 47 years) with documented right colon diverticulitis.
RESULTS: Most patients presented with right lower quadrant pain and local peritoneal signs. Acute appendicitis was the preoperative diagnosis in 82% (18/22) of the patients. Only one diagnosis subsequently proved correct. The pathology was ea…

The sigmoid colon is the part of the large intestine which is most commonly involved in diverticular disease due to its anatomical properties. True diverticula containing all layers of the colonic wall are rare. The majority of diverticula found are pseudodiverticula. They are herniations of mucosa and submucosa through clefts in the colonic muscle layers as a result of morphologic changes of the colonic wall and an imbalance between the intraluminal pressure and the pressure in the abdominal cavity. Microperforations of the tip of the diverticula are an important factor leading to inflammatio

Acute pain in the lower right side of the abdomen caused by caecal diverticulitis is very rare and hardly to distinguish from acute appendicitis. Therefore it is usually diagnosed intraoperatively. We report our experience with 12 patients, who received surgical treatment from 1984 to 1991. The diagnosis was made not before operation in 11 cases. Because of a severe inflammation of the caecal wall we performed an ileocaecal resection in 10 cases, in 2 cases it was necessary to perform a right hemicolectomy because of additional diverticulitis of the ascending colon. The surgical resection of t…

The AA. report about a case of hepatic abscess found in a operated patient for perforation of a sigmoid diverticulum. The AA. made some consideration on the rarity of the hepatic abscess as complication of diverticulum perforation.

Diverticulitis can be detected accurately by computed tomography (CT) and staged according to an established system that is meaningful to both radiologists and surgeons. Diverticulitis limited to the wall of the sigmoid colon or small diverticular abscesses contained within the pericolic mesentery generally respond well to antibiotic therapy; larger abscesses must be drained. CT-guided catheter drainage is an effective method to treat these large diverticular abscesses. Overall management is simplified since one-stage sigmoid resection can be performed electively on a nonseptic patient. High-r…

Twelve cases of acute solitary diverticulitis of the cecum or ascending colon are reviewed. This is frequently indistinguishable from acute appendicitis preoperatively and is often mistaken for carcinoma at laparotomy. There are subtle clinical features that may help to suggest the diagnosis both before and during surgery. If diagnosed preoperatively, it can be treated effectively with broad spectrum antibiotics without surgical intervention. When diagnosed intraoperatively, hemicolectomy can often be avoided. Appendectomy should be done if resection is not performed.
