Celiac Disease and Other Causes of Duodenitis.

CONTEXT: - Patients who receive an upper gastrointestinal endoscopic examination frequently have biopsies taken from the duodenum. Accurate interpretation of duodenal biopsies is essential for patient care. Celiac disease is a common clinical concern, but pathologists need to be aware of other conditions of the duodenum that mimic celiac disease. OBJECTIVE: - To review the normal histologic features of duodenal mucosa and describe the clinical and histologic findings in celiac disease and its mimics, listing the differentiating features of biopsies with villous atrophy and epithelial lymphocy

Cytomegalovirus as an Insidious Pathogen Causing Duodenitis.

A 60-year-old woman with rheumatoid arthritis treated with methotrexate for a decade complained of slight epigastric discomfort. A positive cytomegalovirus (CMV) antigenemia test indicated the probability of CMV-related gastrointestinal infection, for which esophagogastroduodenoscopy was performed. Endoscopic findings showed a non-specific duodenal mucosal lesion;however, pathological investigation revealed evidence of CMV duodenitis. There is scarce information on the clinical and pathological features of CMV-related duodenitis, likely due to its low prevalence. CMV infection in the upper gas…

Endoscopic duodenitis, gastric metaplasia and Helicobacter pylori.

BACKGROUND AND AIMS: The purpose of this study was to investigate the relationship between gastric metaplasia and Helicobacter pylori in patients with endoscopic duodenitis. METHODS: The subjects were 57 patients with endoscopic duodentitis with or without H. pylori-associated gastritis. Biopsy specimens were obtained from the stomach and duodenal bulb to assess the histological findings and H. pylori infection. Gastric metaplasia was divided into three types: complete, intermediate and incomplete, according to the amount of mucus in the metaplastic cells. In 10 H. pylori-positive patients, e…

Epidemiology and etiology of "autonomous" nonspecific duodenitis.

In this prospective study we looked for possible epidemiological and etiological factors in "autonomous" nonspecific duodenitis. Of 136 dyspeptic patients who entered the study, duodenitis was found in 25.6% (94.4% chronic duodenitis and 5.6% isolated active duodenitis). Men predominated with a significant prevalence of 74%; 49% of them had white-collar jobs, but age, psychological factors, and the season of the year played no role. We found that smoking and alcohol and coffee intake bore no relation to duodenitis. Helicobacter pylori (HP) was present in only 17.1% of patients with duodenitis,…

Clinical and histologic study of endoscopic duodenitis.

Duodenitis was investigated by endoscopy in 93 cases from among 1242 subjects. Endoscopic duodenitis was classified into three types endoscopically--reddening type, erosive type and nodular type. There was a relatively high correlation between the endoscopic and the histological diagnosis of duodenitis. Severely inflamed cases were found histologically more frequently in erosive- or nodular-type duodenitis than in reddening-type duodenitis. During the follow-up period, changes in endoscopic findings were observed more frequently, from the erosive type to the reddening type, and from the redden…

[Erosive-hemorrhagic duodenitis: natural history and management].

A group of 32 patients with erosive-haemorrhagic duodenitis or EHD (group 5) was followed up for 36 months in order to monitor the natural history of EHD and any connections with peptic ulcers. The control group (group C) was formed of patients with similar clinical symptoms but an endoscopic picture of either a healthy duodenum or only superficial mucosal damage (non-erosive-haemorrhagic duodenitis or NEHD). The follow-up clinical, secretory and endoscopic examinations annually or whenever there were new acute attacks revealed a far higher incidence of peptic ulcer in group S than group C or …

Nodular duodenitis in chronic maintenance hemodialysis patients.

A retrospective study of 138 cases of chronic hemodialysis between 1977 and 1982 were reviewed for endoscopic, radiographic, and histologic characteristics of duodenitis. Forty patients underwent upper gastrointestinal barium x-rays; 13 were found to have multiple duodenal bulb nodules and three of these patients had very prominent duodenal bulb folds. There were 42 patients on whom upper panendoscopy was performed, and multiple duodenal nodules were seen in 15 patients and thickened folds in three patients. There were five patients in whom nodules were seen only on endoscopy. The size of the …

Quantitative histological study of duodenitis in biopsies.

In 48 patients, suffering from an endoscopically verified ulcerative or nonulcerative duodenitis, biopsies of the bulb and of the lower duodenum have been studied with quantitative histological methods. The evaluation of the data with simple and multivariate statistical methods revealed: In healthy duodenal mucosa the height of the villous epithelium significantly decreases with rising age, while the cellular infiltration of the stroma increases. Conclusive, sex dependent differences are not present. Many histological variables are significantly correlated to the endoscopical degree of inflamm…

Does malabsorption occur in chronic atrophic duodenitis?

Absorption behavior was studied in 6 patients with chronic atrophic duodenitis and compared with that observed in 6 cases of untreated celiac disease. The histobioptic study of the duodenal and jejunal mucosa showed that in atrophic duodenitis the lesions involve just one or two duodenal portions with normal jejunal mucosa: in contrast it was confirmed that in celiac disease atrophic lesions extended from the duodenal to the jejunal mucosa. From the clinical point of view, patients with chronic atrophic duodenitis do not show evidence of malabsorption while such phenomena occur consistently, e…
