Interventions to improve hip fracture guideline adherence for time to surgery and early mobilization: a systematic review and meta-analysis.

BACKGROUND: Clinical quality registries indicate that adherence to clinical practice guidelines for the management of hip fracture patients is often not as recommended. The aim of this systematic review is to evaluate interventions that improve adherence to hip fracture guideline recommendations for time to surgery and early mobilization. METHODS: A systematic review and meta-analysis of English language studies published between 1st January 2000 and 12th March 2025, available from Ovid MEDLINE, CINAHL, Embase, Scopus and Web of Science was conducted. Two reviewers independently screened stud

Predictors of early mobilization in patients requiring VV ECMO for greater than 7 days: An international cohort study.

BACKGROUND: Despite the demonstrated benefits of rehabilitation, active physical therapy and early mobilization are not universally performed during critical illness, especially among patients receiving extracorporeal membrane oxygenation (ECMO), with variation among sites. OBJECTIVE: What factors are predictive of physical mobility during venovenous (VV) ECMO support? METHODS: We performed an observational analysis of an international cohort using data from the Extracorporeal Life Support Organization (ELSO) Registry. We analyzed adults (≥18 years) supported with VV ECMO who survived for at

A suture anchor-based repair technique for type IV jersey finger injuries: a biomechanical investigation.

The aim of this biomechanical investigation was to evaluate a repair technique for type IV FDP tendon avulsions using a suture anchor, addressing the bony and the tendinous aspect of this injury simultaneously. In 45 distal phalanges from human anatomical specimens the injury was simulated and repairs were performed with a suture anchor using an innovative technique, interosseous sutures and a combination of screws and an interosseous suture. Repetitive loading for 500 cycles simulated postoperative mobilization. Repairs were loaded to failure thereafter. Elongation of the tendon-suture comple

Rehabilitating the neurological patient in the ICU: what is important?

PURPOSE OF REVIEW: To describe recent literature evaluating the effectiveness of early rehabilitation in neurocritical care patients. RECENT FINDINGS: There is a drive for early rehabilitation within the ICU; however, there are unique considerations for the neurocritically ill patient that include hemiplegia, cognitive impairments and impaired conscious state that can complicate rehabilitation. Additionally, neurological complications, such as hemorrhage expansion and cerebral edema can lead to the risk of further neurological damage. It is, therefore, important to consider the effect of exer

The changing landscape of cardiac rehabilitation; from early mobilisation and reduced mortality to chronic multi-morbidity management.

AIM: This paper aims to demonstrate how the rationale and delivery of cardiac rehabilitation (CR), in those countries with long term established standards of practice, has changed over the past eight decades. METHODS: A narrative report based on the evolution of key published guidelines, systematic reviews and medical policies since the 1940s. RESULTS: Case reports of the value of exercise in cardiac disease can be dated back to 1772. Formative groundwork for exercise-based CR was published between 1940 and 1970. However, it was not until the late 1980s that a large enough data set of contro

Move to improve: progressive mobility in the intensive care unit.

Bed rest has detrimental consequences, and therefore in the ICU, progressive early mobility should be the goal for every patient expected to survive. This article examines the consequences of immobility, barriers experienced when attempting to increase patients' mobility, and ways in which dedicated mobility teams can overcome these barriers.

A new hinged dynamic distractor, for immediate mobilization after knee dislocations: Technical note.

Knee dislocation is always a difficult topic mainly with regard to the structures that have been damaged and the best treatment option. The purpose of this study is to describe a new dynamic external fixator (EF) to be applied after knee dislocation, in combination with posterior cruciate ligament (PCL) reconstruction. This approach permits an immediate postoperative mobilization of the joint, with the possibility to perform a delayed ACL reconstruction and eventually reducing the complication rate related to multiple ligament surgery.

Mortality and morbidity in suspected acute myocardial infarction in relation to ambulance transport.

In 681 patients admitted to the coronary care unit (CCU) at Sahlgrenska hospital between 1 May 1983 and 31 May 1984, due to suspected acute myocardial infarction (MI), the hospital mortality and morbidity were related to whether the patients were transferred to hospital by ambulance or not. In the ambulance group acute MI developed in 48% (during the first 3 days in hospital) compared with 41% in the non-ambulance group (P = 0.10). The overall mortality rate was 10.4% in the ambulance group versus 3.8% in the non-ambulance group (P = 0.001). Corresponding figures for MI patients were 19.3% ver

Impending aortic aneurysm rupture - a case report and review of the warning signs.

BACKGROUND: Bed rest duration following deployment of a vascular closure device after transfemoral left-sided cardiac catheterization is not standardized. Despite research supporting reduced bed rest, many hospitals require prolonged bed rest. Delayed ambulation is associated with back pain, urine retention, difficulty eating, and longer stay. OBJECTIVE: To study length of stay, safety, and opportunity cost savings of reduced bed rest at a large urban hospital. METHODS: A single-site 12-week study of 1-hour bed rest after transfemoral cardiac catheterizations using vascular closure devices.

Outcomes of major lower extremity amputations n dysvascular patients: Room for improvement.

OBJECTIVES: Dysvascular patients account for >80% of major amputations in the US. We sought to determine if early mobilization and discharge disposition decreased post-operative hospital length of stay (PO-LOS) and expedited independent ambulation. METHODS: A retrospective review of dysvascular patients undergoing major amputations was performed. Primary outcomes included PO-LOS, discharge disposition, and days to ambulation. RESULTS: 130 patients were included. Patients evaluated by Physical Therapy (PT) within 1 day of formal amputation had decreased PO-LOS (5.6 vs 6.5 days, p = 0.029). Pa

Optimal Methods to Secure Extracorporeal Membrane Oxygenation Bicaval Dual-Lumen Cannulae: What Works?

Ambulation while on extracorporeal membrane oxygenation (ECMO) is critical to facilitate native pulmonary recovery for patients with acute respiratory failure and is a prerequisite for listing for lung transplantation to achieve optimal outcomes. The development of a bicaval dual-lumen cannula capable of providing venovenous (VV) ECMO support via the internal jugular vein has greatly facilitated ambulation and rehabilitation programs. But cannula dislodgement is a serious concern with ambulation and rehabilitation, especially when minor cannula migration can significantly impact VV-ECMO flow.

Comparison of Restricted Versus Liberal Postsurgical Ambulation Protocols on Outcomes After Lumbar Fusion Surgery.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: The objective of this study was to explore the impact of different hospital-based ambulation protocols on mobility and surgical outcomes after lumbar fusion surgery. SUMMARY OF BACKGROUND DATA: Previous research has highlighted the value of early ambulation after surgery; still some hospitals choose to adopt a more conservative ambulation approach due to concern for patient safety. METHODS: Adult patients ≥18 years of age who underwent primary posterior lumbar decompression and fusion (PLDF) surgery at a hospital with restricted ambulatio

Pneumatic orthosis for paraplegic patients: functional evaluation and prescription considerations.

Fourteen paraplegic patients were studied during ambulation training, 11 with pneumatic orthoses and with conventional metal orthoses, and 3 with pneumatic orthoses only. The purposes of the study were to establish whether pneumatic orthoses could be used by paraplegic patients for effective ambulation, whether these devices presented any advantages or disadvantages over conventional orthoses and how their use would affect the expected level of rehabilitation and independence. As far as application, standing up, sitting down and ambulation within a rehabilitation center are concerned, similar

Bed rest duration and development of cerebrospinal fluid leaks after intradural spinal surgery: a meta-analysis of comparative studies.

INTRODUCTION: Postoperative cerebrospinal fluid (CSF) leaks and related complications are a major concern after intradural spinal surgeries. The role of prolonged bed rest in reducing the incidence of these complications has been debated. This meta-analysis aimed to evaluate whether early versus late mobilization affects the incidence of CSF leak-related complications (CLRC) after intradural spinal surgery. EVIDENCE ACQUISITION: Following PRISMA guidelines, we conducted a systematic review and meta-analysis of comparative studies on early ambulation (EA) versus prolonged bed rest (PBR) in pat

Clinical Practice Guideline: Early Mobilization and Rehabilitation of Critically Ill Burn Patients.

This Clinical Practice Guideline addresses early mobilization and rehabilitation (EMR) of critically ill adult burn patients in an intensive care unit (ICU) setting. We defined EMR as any systematic or protocolized intervention that could include muscle activation, active exercises in bed, active resistance exercises, active side-to-side turning, or mobilization to sitting at the bedside, standing, or walking, including mobilization using assistance with hoists or tilt tables, which was initiated within at least 14 days of injury, while the patient was still in an ICU setting. After developing

No clinically significant difference in postoperative pain and side effects comparing conventional and enhanced recovery total hip arthroplasty with early mobilization.

INTRODUCTION: Enhanced recovery after surgery (ERAS) leads to less morbidity, faster recovery, and, therefore, shorter hospital stays. The expected increment of primary total hip arthroplasty (THA) in the U.S. highlights the need for sufficient pain management. The favorable use of short-lasting spinal anesthesia enables early mobilization but may lead to increased opioid consumption the first 24 h (h) postoperatively. METHODS: In a retrospective study design, we compared conventional THA with postoperative immobilization for two days (non-ERAS) and enhanced recovery THA with early mobilizati
