Comparison of Three Polymerase Chain Reaction Protocols for the Diagnosis of Leprosy in Ecuador.

Leprosy or Hansen's disease remains a public health concern in Ecuador, where diagnosis still relies on clinical evaluation and bacilloscopy. This pilot study evaluated the performance of three polymerase chain reaction (PCR) protocols for detection of Mycobacterium leprae in 31 clinical specimens from patients clinically suspected of leprosy. Two real-time PCR assays (quantitative polymerase chain reaction [qPCR]) and one conventional PCR were compared with bacilloscopy. The two qPCR assays showed identical positivity rates of 32.3% (n = 10/31), whereas the conventional PCR detected only 3.2%…

Salmonella in pigs slaughtered in Ecuador: prevalence, serotypes, genotypes and antibiotic resistance.

Salmonella enterica subsp. enterica (S. enterica) is a leading foodborne pathogen worldwide, and pork products are major sources of infection. Given the lack of data on S. enterica infection in pigs slaughtered in Ecuador, this study aimed to determine its prevalence, as well as the serotypes, genotypes and antimicrobial susceptibility profiles of isolates from cecum contents (CCs) and mesenteric lymph nodes (MLNs) sampled from 365 pigs at the evisceration step at a slaughterhouse in Quito city. Salmonella spp. in the samples was detected by culture (ISO 6570-1 method). Strains were serotyped …

Environmental barriers to participation for people with spinal cord injury. The case of Ecuador.

This study assesses the environmental barriers faced by people with spinal cord injury (SCI) in Ecuador. Using cross-sectional data from the International Spinal Cord Injury Survey (InSCI), we analyzed responses from 510 participants across 23 provinces. We calculated the total environmental barrier index using the Nottwil Environmental Factor Inventory (NEFI) and applied regression analysis to identify key influencing factors. We also examined the severity of barriers by region, comparing rural and urban areas. Our findings reveal significant disparities. Individuals with lower incomes, those…

Economic burden of avoidable blindness due to diabetic macular edema in Ecuador.

INTRODUCTION: In Ecuador, the economic burden of diabetes-related visual impairment has not been studied cohesively. More evidence-that takes into account lost productivity, direct costs, and intangible costs-is required to support public policies that prevent and treat diabetes-related visual impairment. OBJECTIVE: The purpose of this research is to estimate the economic burden of avoidable blindness due to diabetic macular edema in Ecuador. METHODS: Costs were estimated in a one-year, retrospective, cost-of-illness study focusing on two groups of people. The first group contains those that…

Epidemiological and geodemographic patterns of scorpionism in Ecuador: A nationwide analysis (2021-2024).

BACKGROUND: Approximately 1.2 million scorpion stings are reported globally each year, resulting in an estimated 3000 deaths. Of the 2500 known scorpion species, about 40 are considered medically significant. In Ecuador, where at least 47 scorpion species exist, information on scorpion stings remains scarce. METHODS: A nationwide cross-sectional analysis was conducted on all officially reported cases of scorpion stings documented in the epidemiological surveillance reports from the Ministry of Public Health in Ecuador between 2021 and 2024. RESULTS: A total of 1633 cases were identified, wit…

Use of a "tablet pole" for the administration of ivermectin for strongyloidiasis in a field study in Ecuador.

BACKGROUND: Establishment of efficient control programs for strongyloidiasis, the infection by Strongyloides stercoralis, is among the World Health Organization (WHO) targets for 2030. Ivermectin is a drug of choice for strongyloidiasis, but its weight-based administration can be unfeasible in remote areas. We evaluated a WHO tablet pole for administration of ivermectin in school-age children living in remote villages in Ecuador. METHODS: Children were enrolled in 16 villages in Esmeraldas Province of Ecuador, between July 2021 and June 2022. The pole identified four height intervals correspo…

Identification of the Mechanical Failure Factors with Potential Influencing Road Accidents in Ecuador.

Road traffic accidents result in injury or even death of passengers. One potential cause of these accidents is mechanical failures due to a lack of vehicle maintenance. In the quest to identify these mechanical failures, this paper aims to set up the procedure to identify the mechanical failures that contribute to traffic accidents in cities located in developing countries, including the city of Cuenca-Ecuador. For present research, a database provided by the entity responsible for the Vehicle Technical Inspection, the Empresa Pública Municipal de Movilidad, Tránsito y Transporte and for the o…

Low levels of awareness, treatment, and control of hypertension in Andean communities of Ecuador.

The major burden of hypertension (HTN) occurs in low-middle-income countries (LMIC) and it is the main modifiable risk factor for cardiovascular diseases (CVD). Few population studies on HTN prevalence have been carried out in Ecuador where there is limited information regarding its prevalence, awareness, treatment, and control. Thus, the aim of the present study was to determine the prevalence, awareness, treatment, and control of HTN and its association with socio-economic, nutritional, and lifestyle habits in urban and rural Andean communities of Pichincha province in Ecuador. The authors s…

Community Epidemiology Approach to Parasitic Infection Screening in a Remote Community in Ecuador.

The Awa indigenous people of Ecuador live in remote areas and were included in health programs only recently. The first screening for parasitic infections in the Awa communities was implemented in the context of community epidemiology. During the site visits in each community, the health-care staff collected the samples for stool microscopy and serology for Strongyloides. A total of 705 individuals consented for the study, representing 40% of the Awa population living in the targeted communities; 184 (26%) participants supplied a stool sample. Giardia intestinalis was found in about 11% of sam…

Seasonal and geographic variation in insecticide resistance in Aedes aegypti in southern Ecuador.

Insecticide resistance (IR) can undermine efforts to control vectors of public health importance. Aedes aegypti is the main vector of resurging diseases in the Americas such as yellow fever and dengue, and recently emerging chikungunya and Zika fever, which have caused unprecedented epidemics in the region. Vector control remains the primary intervention to prevent outbreaks of Aedes-transmitted diseases. In many high-risk regions, like southern Ecuador, we have limited information on IR. In this study, Ae. aegypti IR was measured across four cities in southern Ecuador using phenotypic assays …

Seroprevalence and risk factors for infection with Bartonella bacilliformis in Loja province, Ecuador.

The seroprevalence and epidemiology of Bartonella bacilliformis infection in the Andean highlands of Ecuador is largely unknown. We conducted a sero-epidemiologic survey of 319 healthy children aged 1-15 years living in six rural, mountain communities in Loja Province, Ecuador. Blood was collected by finger stick onto filter paper and dried, and the eluted sera analyzed for antibodies to B. bacilliformis by rPap31 ELISA. Demographic, entomologic, and household variables were assessed to investigate associated risk factors for antibody seropositivity to B. bacilliformis. Seroprevalence of 28% w…

Spatiotemporal Variation in Environmental Vibrio cholerae in an Estuary in Southern Coastal Ecuador.

Cholera emergence is strongly linked to local environmental and ecological context. The 1991-2004 pandemic emerged in Perú and spread north into Ecuador's El Oro province, making this a key site for potential re-emergence. Machala, El Oro, is a port city of 250,000 inhabitants, near the Peruvian border. Many livelihoods depend on the estuarine system, from fishing for subsistence and trade, to domestic water use. In 2014, we conducted biweekly sampling for 10 months in five estuarine locations, across a gradient of human use, and ranging from inland to ocean. We measured water-specific environ…

Distribution of triatomine species in domestic and peridomestic environments in central coastal Ecuador.

BACKGROUND: Although the central coast of the Ecuador is considered endemic for Chagas disease, few studies have focused on determining the risk of transmission in this region. In this study we describe the triatomine household infestation in Manabí province (Central Coast region), determine the rate of Trypanosoma cruzi infection and study the risk factors associated with infestation by Rhodnius ecuadoriensis. METHODOLOGY/PRINCIPAL FINDINGS: An entomological survey found three triatomine species (Rhodnius ecuadoriensis, Panstrongylus rufotuberculatus and P. howardi) infesting domiciles in 47…

Study of the Huntington's disease IT-15 gene in different ethnic groups in Ecuador.

This study aims to establish the current state of the IT-15 (HTT) gene in different Ecuadorian ethnic groups and patients by determining CAG triplet repeats, compared with the ethnicity of individuals. A total of 412 individuals were studied using nested polymerase chain reaction and Sanger sequencing: 75 individuals were indigenous (Kichwas), 211 mestizos, and 65 Afro-Ecuadorians. We included 31 patients who were clinically diagnosed with Huntington's disease (HD) and relatives of the affected patients (n = 30). Moreover, we correlated the presence of HD in Ecuadorian patients with 46 genetic…

Socioeconomic deprivation and the development of neuropsychological functions: A study with "street children" in Ecuador.

Socioeconomic status (SES) is known to have a considerable influence on the development of neuropsychological functions. In particular there is strong evidence for less efficient development of prefrontal-cortex-related functions in children raised in low-SES households. "Street children" are a common feature of low SES in many low- and middle-income countries, and some researchers have suggested that the unique life experiences of street children may drive their neurocognitive development. This study compares a group of 36 former street children in Quito, Ecuador with a control group of 26 ne…

Risk factors for atopic and non-atopic asthma in a rural area of Ecuador.

BACKGROUND Asthma has emerged as an important public health problem of urban populations in Latin America. Epidemiological data suggest that a minority of asthma cases in Latin America may be associated with allergic sensitisation and that other mechanisms causing asthma have been overlooked. The aim of the present study was to investigate risk factors for atopic and non-atopic asthma in school-age children. METHODS A cross-sectional study was conducted among 3960 children aged 6-16 years living in Afro-Ecuadorian rural communities in Esmeraldas province in Ecuador. Allergic diseases and risk …
