Presurgical evaluation: current role of invasive EEG.

Our purpose was to review the current role of invasive and semi-invasive EEG in the presurgical evaluation of candidates for epilepsy surgery. The use of stereotactically implanted intracranial depth (stereo-EEG), subdural strip and grid, and foramen ovale electrodes, as well as intraoperative electrocorticography and electrical brain stimulation ("functional mapping") at the Epilepsy Center University Hospital Zurich, from 1984 to 1998, is analyzed. Advantages and disadvantages of the various intracranial EEG techniques are critically discussed. Out of 422 selective amygdalohippocampectomies …

Biophysically constrained learning from healthy neural priors improves motor decoding in pathological cortical networks.

High-performance brain-computer interfaces (BCIs) rely on stable neural representations, yet brain functional impairments inherently disrupt … thy neural recordings to guide neural decoding. In a gesture classification task using high-density electrocorticography from patients with gliomas infiltrating the motor cortex, PACT improved the accuracy by more than 10%, effectively restoring the distorted semantic geometry of motor commands and realigning them with healthy canonical representations (correlation in …

The Human Intracerebral EEG Platform: A Cloud-Based Trusted Research Environment for Collaborative SEEG Research in Epilepsy and Neuroscience.

INTRODUCTION: Lack of standardised methodologies and secure processing environments for intracerebral EEG (iEEG) data creates critical barriers to international collaboration and reproducibility in clinical neuroscience. Within the framework of the Human Brain Project and EBRAINS, we have developed a specific platform, the Human Intracerebral EEG Platform (HIP), to address this challenge. METHODS: The specifications considered to develop the HIP included: (1) secure access to sensitive datasets; (2) standardised data organisation following the BIDS for iEEG (BIDS-iEEG) to ensure interoperabil

A novel micro-ECoG recording method for recording multisensory neural activity from the parietal to temporal cortices in mice.

Characterization of inter-regional interactions in brain is essential for understanding the mechanism relevant to normal brain function and neurological disease. The recently developed flexible micro (μ)-electrocorticography (μECoG) device is one prominent method used to examine large-scale cortical activity across multiple regions. The sheet-shaped μECoG electrodes arrays can be placed on a relatively wide area of cortical surface beneath the skull by inserting the device into the space between skull and brain. Although rats and mice are useful tools for neuroscience, current μECoG recording …

Spreading Depolarizations Suppress Hematoma Growth in Hyperacute Intracerebral Hemorrhage in Mice.

BACKGROUND: Spreading depolarizations (SDs) occur in all types of brain injury and may be associated with detrimental effects in ischemic stroke and subarachnoid hemorrhage. While rapid hematoma growth during intracerebral hemorrhage triggers SDs, their role in intracerebral hemorrhage is unknown. METHODS: We used intrinsic optical signal and laser speckle imaging, combined with electrocorticography, to investigate the effects of SD on hematoma growth during the hyperacute phase (0-4 hours) after intracortical collagenase injection in mice. Hematoma expansion, SDs, and cerebral blood flow wer…

Electrocorticographic Network Feature Space Constriction as a Preictal Biomarker.

In patients with epilepsy, seizures are associated with pathological neural synchronization. However, the preictal period preceding a seizure often exhibits reduced spatial synchronization compared to normal cognition. This observation aligns with the concept of the brain as a complex dynamical system, where a reduction in dimensionality and resilience can precede a phase transition. The Critical Brain Hypothesis suggests a connection between the loss of healthy scale-free behavior and various disorders, including epilepsy. Our study investigates preictal changes by utilizing network features,

Chronically Stable, High-Resolution Micro-Electrocorticographic Brain-Computer Interfaces for Real-Time Motor Decoding.

Brain-computer interfaces (BCIs) enable communication between individuals and computers or other assistive devices by decoding brain activity, thereby reconstructing speech and motor functions for patients with neurological disorders. This study presents a high-resolution micro-electrocorticography (µECoG) BCI based on a flexible, high-density µECoG electrode array, capable of chronically stable and real-time motor decoding. Leveraging micro-nano manufacturing technology, the µECoG BCI achieves a 64-fold increase in electrode density compared to conventional clinical electrode arrays, enhancin…

Electrophysiological Response of the Non-Anesthetized Primate Brain to Minimally Invasive Local Infrared Neural Stimulation in Chronic Experiments.

Infrared neural stimulation (INS) represents an invasive technique for modulating brain activity in animals, particularly primates, which serve as effective models for human brain research. Noninvasive approaches, such as transcranial laser stimulation, are safer but have lower spatial and temporal resolution, primarily altering metabolic processes rather than directly stimulating specific neurons. Invasive techniques provide better resolution by targeting neurons with focused laser beams but require intricate surgeries that damage the meninges, limiting studies to short-term experiments condu

A corollary discharge circuit in human speech.

When we vocalize, our brain distinguishes self-generated sounds from external ones. A corollary discharge signal supports this function in animals; however, in humans, its exact origin and temporal dynamics remain unknown. We report electrocorticographic recordings in neurosurgical patients and a connectivity analysis framework based on Granger causality that reveals major neural communications. We find a reproducible source for corollary discharge across multiple speech production paradigms localized to the ventral speech motor cortex before speech articulation. The uncovered discharge predic

Resting state functional connectivity demonstrates increased segregation in bilateral temporal lobe epilepsy.

OBJECTIVE: Temporal lobe epilepsy (TLE) is the most common type of focal epilepsy. An increasingly identified subset of patients with TLE consists of those who show bilaterally independent temporal lobe seizures. The purpose of this study was to leverage network neuroscience to better understand the interictal whole brain network of bilateral TLE (BiTLE). METHODS: In this study, using a multicenter resting state functional magnetic resonance imaging (rs-fMRI) data set, we constructed whole-brain functional networks of 19 patients with BiTLE, and compared them to those of 75 patients with unil

Dynamic neural representations of memory and space during human ambulatory navigation.

Our ability to recall memories of personal experiences is an essential part of daily life. These episodic memories often involve movement through space and thus require continuous encoding of one's position relative to the surrounding environment. The medial temporal lobe (MTL) is thought to be critically involved, based on studies in freely moving rodents and stationary humans. However, it remains unclear if and how the MTL represents both space and memory especially during physical navigation, given challenges associated with deep brain recordings in humans during movement. We recorded intra

Expanding eligibility for intracranial electroencephalography using Dexmedetomidine Hydrochloride in children with behavioral dyscontrol.

INTRODUCTION: Invasive intracranial electroencephalography (IEEG) is advantageous for identifying epileptogenic foci in pediatric patients with medically intractable epilepsy. Patients with behavioral challenges due to autism, intellectual disabilities, and hyperactivity have greater difficulty tolerating prolonged IEEG recording and risk injuring themselves or others. There is a need for therapies that increase the safety of IEEG but do not interfere with IEEG recording or prolong hospitalization. Dexmedetomidine Hydrochloride's (DH) use has been reported to improve safety in patients with be

Causal dynamics and information flow in parietal-temporal-hippocampal circuits during mental arithmetic revealed by high-temporal resolution human intracranial EEG.

Mental arithmetic involves distributed brain regions spanning parietal and temporal cortices, yet little is known about the neural dynamics of causal functional circuits that link them. Here we use high-temporal resolution (1000 Hz sampling rate) intracranial EEG from 35 participants, 362 electrodes, and 1727 electrode pairs, to investigate dynamic causal circuits linking posterior parietal cortex (PPC) with ventral temporal-occipital cortex and hippocampal regions which constitute the perceptual, visuospatial, and mnemonic building blocks of mental arithmetic. Operculo-Insular Epilepsy: Scalp and Intracranial Electroencephalographic Findings.

PURPOSE: Operculo-insular seizures are heterogeneous and may resemble seizures originating from the temporal, frontal, or parietal lobe. Although surface and invasive EEG recordings are often necessary to detect operculo-insular seizures, electrophysiological features of operculo-insular epilepsies remain poorly characterized. This study describes the EEG findings of patients with operculo-insular epilepsy. METHODS: We reviewed electrophysiological data of all patients (n = 9) with operculo-insular seizures revealed by intracranial EEG and for whom operculo-insular epilepsy was confirmed by g
