OBJECTIVES: Naloxone is a harm reduction tool for mitigating the rising rate of opioid overdose deaths. We sought to develop and implement an alert in the electronic health record outlining which patients are at higher risk of opioid overdose and should be coprescribed naloxone. Our aim was to increase coprescribing of naloxone to qualified patients. We also endeavored to evaluate naloxone prescription volume, fill rates, and statewide dispenses before and after alert implementation.
METHODS: We developed the electronic alert according to a state opioid safety initiative specifying under whic

Widespread adoption of electronic health records (EHRs) has resulted in the collection of massive amounts of clinical data. In ophthalmology in particular, the volume range of data captured in EHR systems has been growing rapidly. Yet making effective secondary use of this EHR data for improving patient care and facilitating clinical decision-making has remained challenging due to the complexity and heterogeneity of these data. Artificial intelligence (AI) techniques present a promising way to analyze these multimodal data sets. While AI techniques have been extensively applied to imaging data…

Screening for latent tuberculosis infection is recommended for foreign-born persons in the United States. We used proxy data from electronic health records to determine that 17.5% of foreign-born outpatients attending the UC San Diego Health clinic (San Diego, CA, USA) underwent screening. Ending the global tuberculosis epidemic requires improved screening.

BACKGROUND: Hospital electronic health record (EHR) data are increasingly being called upon for research purposes, yet only recently has it been tested to examine its reliability. Studies that have examined reliability of EHR data for research purposes have varied widely in methods used and field of inquiry, with little reporting of the reliability of perinatal and obstetric variables in the current literature.
OBJECTIVE: To assess the reliability of data extracted from a commercially available inpatient EHR as compared with manually abstracted data for common attributes used in obstetrical r

Electronic health record systems (EHRs) play an increasingly important role in opioid agonist treatment. In Italy, an EHR called the Multi Functional Platform (MFP) is in use in 150 opioid-agonist treatment facilities in 8 of Italy's 23 regions. This report describes MFP and presents 2010 data from 65 sites that treated 8145 patients, of whom 72.3% were treated with methadone and 27.7% with buprenorphine. Patients treated with buprenorphine compared to methadone were more likely to be male (p < .01) and younger (p < .001). Methadone compared to buprenorphine patients had a higher percentage of

Mortality transition in Greece is a well-studied phenomenon in several of its aspects. It is characterised by an almost constant increase in life expectancy at birth and other ages and a parallel decrease in death probabilities. The scope of this paper is a comprehensive assessment of the mortality transition in Greece since 1961, in the light of holistic analysis. Within this paper, life tables by gender were calculated and the temporal trends of life expectancy at several ages were examined. Moreover, a cluster analysis was used in order to verify the temporal changes in the mortality patter

In Alzheimer's Disease and Related Dementia (ADRD) prediction, Electronic Health Records (EHRs) provide rich but fragmented information. Without a coherent clinical narrative, models tend to rely on a few dominant signals (e.g., age-related patterns) rather than capturing the underlying clinical mechanisms. Such reliance becomes problematic in younger-onset cases, where these signals are less informative. To address this challenge, we propose RaiLED-AD, a dual-encoder teacher-student framework where the student learns from serialized EHR data and the teacher leverages LLM-generated narratives …

BACKGROUND: The secondary use of electronic health records (EHRs) offers a major opportunity to generate real-world evidence (RWE) to complement clinical trials and support regulatory and clinical decision making. However, a large proportion of clinically relevant information remains embedded in unstructured text, limiting its systematic reuse, particularly in complex areas such as oncology. This article presents the Savana Next-Generation Registry (SNGR), a methodology for building oncology-focused registries through the extraction and structuring of unstructured EHR data.

Cervical spondylotic myelopathy (CSM) is the leading cause of spinal cord dysfunction in older adults, yet diagnosis is frequently delayed due to insidious symptom onset and limited clinical recognition. We developed and externally validated machine learning models using structured electronic health record (EHR) data to predict incident CSM diagnoses up to 30 months in advance. Using data from ~2 million patients in the Merative™ MarketScan® claims database and our institutional EHR, we evaluated a spectrum of modeling strategies, ranging from simple, clinically guided architectures to large-s

BACKGROUND: Alzheimer's Disease (AD) is a complex neurodegenerative disorder, with women comprising nearly two-thirds of individuals with AD. However, sex-specific heterogeneity in AD progression remains insufficiently understood. A data-driven approach is needed to characterise such heterogeneity from longitudinal electronic health records (EHRs).
METHODS: We developed a deep learning-based framework to uncover sex-specific AD sub-phenotypes using longitudinal EHRs from OneFlorida+ Clinical Research Consortium. We constructed temporal representations of these EHRs and employed an autoencoder…

INTRODUCTION: Chronic overlapping pain conditions (COPCs) are a group of pain conditions that often co-occur and present with challenging nociplastic pain. Individuals with COPCs describe clinician skepticism and prolonged diagnosis, leading to periods of inadequate pain treatment. The lack of methods to identify nociplastic pain in electronic health records (EHRs) limits large-scale studies of nociplastic pain trajectories and phenotyping that could improve treatment.
OBJECTIVE: To develop and validate the Centralized Pain Score (CPS) as a method for identifying nociplastic pain in EHR data.…

Rare disease studies often rely on small, selected cohorts, are resource-intensive and difficult to scale. UK primary care electronic health record (EHR) databases provide population-based, longitudinal data, but their use for rare genetic disease research has not been systematically examined. Through systematic mapping of publications from five UK primary care EHR databases (CPRD, OPCRD, QResearch, SAIL Databank and THIN), we found that only 0.82% (47 of 5754) of studies reported on rare genetic diseases. Of these, 77% (36 of 47) linked to external datasets. Study designs included case-contro

BACKGROUND: Antimicrobial resistance (AMR), which refers to the ability of pathogenic bacteria to withstand the effects of antibiotics, is a critical global health issue. Traditional methods for identifying AMRs in clinical settings rely on in-lab testing, which hampers timely medical decision-making. Moreover, there is a notable delay in updating empirical treatment guidelines in response to the rapid evolution of pathogens. Recent advances in AMR research have illuminated the potential of machine learning-based patient information analysis using electronic health records (EHRs).

Alzheimer's Disease (AD) is a complex neurodegenerative disorder strongly influenced by sex differences, with women comprising nearly two-thirds of cases. However, sex-specific progression patterns remain underexplored due to unclear clinical and molecular mechanisms. To address this gap, we developed a temporal autoencoder framework to identify sex-specific AD sub-phenotypes using longitudinal electronic health record (EHR) data from the OneFlorida+ Clinical Research Consortium. Sequential EHRs were encoded into latent representations and clustered to derive disease states, which were assembl

Randomized clinical trials (RCTs) are the cornerstone of evidence-based medicine, as they minimize bias in the allocation of interventions. However, RCTs performed in a very selective population and overcontrolled conditions may impair the generalizability of results. Moreover, increasing running costs and regulatory complexity compromise the conduct of these studies. The need for pragmatic trial designs, with streamlined procedures and low running costs, will shape the short-term future of research in RCTs. Electronic health records (EHR) are routinely collected as part of the treatment of pa…

Alzheimer's Disease and Related Dementias (ADRD) pose a major public health challenge, with a critical need for accurate and scalable tools for detecting cognitive impairment (CI). Readily available electronic health records (EHRs) contain valuable cognitive health data, but much of it is embedded in unstructured clinical notes. To address this problem, we developed a GPT-4o-powered framework for CI stage classification, leveraging longitudinal patient history summarization, multi-step reasoning, and confidence-aware decision-making. Evaluated on 165,926 notes from 1002 Medicare patients from …

OBJECTIVE: Diagnosis codes documented in electronic health records (EHR) are often relied upon to clinically phenotype patients for biomedical research. However, these diagnoses can be incomplete and inaccurate, leading to false negatives when searching for patients with phenotypes of interest. This study aims to determine whether PheMAP, a comprehensive knowledgebase integrating multiple clinical terminologies beyond diagnosis to capture phenotypes, can effectively identify patients lacking relevant EHR diagnosis codes.
