Mature NK-cell lymphoma presenting with chylothorax and delirium: a rare emergency case.

BACKGROUND: Extranodal natural killer (NK)/T-cell lymphomas constitute less than 1% of non-Hodgkin lymphomas, with central nervous system involvement and chylothorax representing exceptionally rare presentations. This unique combination has not been previously reported in emergency medicine literature. CASE PRESENTATION: A previously healthy 51-year-old Chinese male presented with progressive dyspnea, high fever, and bilateral pleural effusion over eight days. Physical examination revealed extensive ecchymoses, bilateral lung consolidation, and splenomegaly. Laboratory studies demonstrated pan

[Predictive model and discriminant analysis of the development of dementia in patients with delirium in the emergency department].

OBJECTIVE: The main aim of our study is to know the sociodemographic, clinical, analytical, and functional variables that predict the probability of developing dementia in patients with delirium who attend the emergency room. METHOD: All patients with delirium (n=45) from the emergency room who were admitted to the Geriatrics service of the General University Hospital of Ciudad Real (HGUCR) in 2016-2018 and met the inclusion and exclusion criteria were included. Subsequently, we ran a bivariate and multivariate analysis of the variables that predicted a diagnosis of dementia at six months and

Relationship Between Pain, Opioid Treatment, and Delirium in Older Emergency Department Patients.

OBJECTIVES: Emergency department (ED) stay and its associated conditions (immobility, inadequate hydration and nutrition, lack of stimulation) increase the risk of delirium in older patients. Poorly controlled pain and paradoxically opioid pain treatment have also been identified as triggers for delirium. The aim of this study was to assess the relationship between pain, opioid treatment, and delirium in older ED patients. METHODS: A multicenter prospective cohort study was conducted in four hospitals across the province of Québec (Canada). Patients aged ≥ 65 years old, waiting for hospital a

The Effect of Binaural Sound on the Occurrence of Emergence Delirium in Children Undergoing Strabismus Surgery: A Randomized Controlled Trial.

BACKGROUND: Binaural beats, which induce brainwave entrainment, have reduced anxiety and sedative use. This study evaluated the effect of binaural beats on emergence delirium (ED) in children undergoing strabismus surgery under general anesthesia. METHODS: This randomized controlled trial included 73 pediatric patients aged 2-7 years receiving binaural beats (n = 36) or noise-canceled silence (n = 37) during general anesthesia. ED was defined as a Pediatric Anesthesia Emergence Delirium (PAED) score > 12 or Watcha score > 2. Primary outcome was the incidence of ED. Secondary outcomes included…

Concomitant Use of Intravenous Remimazolam With Inhalation Anesthesia and Subsequent Emergence Delirium in Children: A Systematic Review and Meta-Analysis.

Emergence delirium (ED) is a typical postoperative complication in pediatric anesthesia, especially with inhalational agents. Remimazolam, a short-acting benzodiazepine, shows potential for reducing the occurrence of ED. Given limited evidence of its use in pediatric anesthesia, we evaluated the efficacy and safety of remimazolam by conducting a systematic review and meta-analysis. MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, and International Clinical Trials Registry Platform (ICTRP) databases were explored for studies on remimazolam in pediatric anesth…

Scoring systems, expert assessment, and identification of risk factors for the emergence of delirium in paediatric patients: Prospective cohort study.

INTRODUCTION: Emergence delirium (ED) is a transient irritative and dissociative state that arises after the cessation of anaesthesia in patients who do not respond to calming measures. There are many risk factors for ED, but the exact cause and underlying mechanism have not been determined because the definition of ED is still unclear in consensus. This study aims to determine ED incidence, identify ED risk factors and external validation of Watcha, Cravero and expert assessment to Pediatric Anesthesia Emergence Delirium (PAED) scoring system in ED prediction. MATERIALS AND METHODS: This stu…

Exosomes: emerging biomarkers and therapeutic potential in postoperative delirium and postoperative cognitive dysfunction.

BACKGROUND: Postoperative delirium (POD) and postoperative cognitive dysfunction (POCD) are common and clinically significant perioperative neurocognitive disorders that disproportionately affect elderly surgical patients. Despite their substantial impact on postoperative recovery and long-term outcomes, the lack of validated biomarkers and effective targeted therapies remains a major challenge in perioperative medicine. DIAGNOSTIC POTENTIAL OF EXOSOMES: Owing to their intrinsic ability to cross the blood-brain barrier and stably encapsulate central nervous system-derived biomolecules, exosom

Cryptococcal Meningitis Diagnosed With Lumbar Puncture in an Emergency Department Patient With Acute Delirium.

Cryptococcal meningitis is a life-threatening fungal infection of the central nervous system. It primarily affects immunocompromised individuals, particularly those with advanced HIV/AIDS, but can occur in people of all ages. Risk factors include solid organ transplantation, hematological malignancies, immunosuppressive medications, and primary immunodeficiency disorders. We describe a case of a 68-year-old male with atypical risk factors for cryptococcal meningitis who presented acutely delirious with a negative diagnostic workup aside from a positive cerebrospinal fluid (CSF) meningitis scre

Incidence and Risk Factors of Emergence Delirium in Children Undergoing Orthopedic Surgery: A Secondary Analysis of Postanesthesia Care Unit Data.

BACKGROUND: Emergence delirium (ED) is a postanesthetic phenomenon more frequently observed in children, typically presenting as altered awareness and behavioral disturbances. While research has primarily focused on the hyperactive subtype of ED, emerging evidence highlights the under-recognized hypoactive subtype. Despite its high occurrence in various pediatric surgeries, ED in orthopedic contexts remains insufficiently studied, particularly regarding subtype characteristics and associated risk factors. DESIGN: Observational, cross-sectional study involved secondary data analysis. METHODS:…
