Hirschsprung's disease affects 1 in 5000 newborns and is caused by an absence of ganglion cells in a variable length of the distal gut. It commonly presents in the newborn period with life-threatening bowel obstruction requiring surgery. Despite apparently successful surgery the long-term outcomes are often unsatisfactory with some children facing a lifetime of continence issues or debilitating constipation. This article exams the reasons for this and describes advances that have occurred in the surgical management of the disease. In the last two decades rapid progress has been made in underst

A unified scenario emerges when it is considered that a major impact of stress on the intestinal tract is reflected by symptoms reminiscent … ier and being intercepted by enteric mast cells. Mast cells signal the presence of the agent to the enteric nervous system (ie, the brain-in-the-gut), which uses one of the specialized programs from its library of programs to remove the "threat." This is accomplished by stimulating mucosal secretion, which flushes the threatening agent into the lumen and maintains it in …

Platelet-activating factor (PAF) is a proinflammatory mediator that may influence neuronal activity in the enteric nervous system (ENS). Electrophysiology, immunofluorescence, Western blot analysis, and RT-PCR were used to study the action of PAF and the expression of PAF receptor (PAFR) in the ENS. PAFR immunoreactivity (IR) was expressed by 6.9% of the neurons in the myenteric plexus and 14.5% of the neurons in the submucosal plexus in all segments of the guinea pig intestinal tract as determined by double staining with anti-human neuronal protein antibody. PAFR IR was found in 6.1% of the n…

BACKGROUND/PURPOSE: After surgical repair of congenital small bowel atresias, intestinal motility disorders often are observed. These may be caused by changes in the enteric nervous system (ENS) secondary to obstruction. To assess these changes, small bowel atresias were induced experimentally in chick embryos.
METHODS: On day 11, the small intestines of 90 chicken embryos were ligated microsurgically in ovo. Breeding of the eggs was continued until day 19. The small bowel was removed, fixed, and embedded for silver-staining, semithin serial sections, and transmission electron microscopy. Add…

Leukaemia inhibitory factor (LIF) is a neuropoietic cytokine, which promotes the development of enteric neurons in vitro, particularly when administered together with neurotrophin-3 (NT-3). The purpose of this study was to map the LIF immunoreactivity in the human enteric nervous system in foetuses, children, adults, and in patients with Hirschsprung's disease. Normal bowel specimens were obtained at postmortem examination of 13 foetuses, at 13-31 weeks of gestation, and at surgery in five children and two adults. Bowel resected in seven patients with Hirschsprung's disease was also investigat…

BACKGROUND/AIMS: The cause of impaired motility of the aganglionic internal anal sphincter (IAS) in Hirschsprung's disease (HD) is unknown. To clarify the physiological significance of non-adrenergic non-cholinergic (NANC) excitatory and inhibitory nerves in the IAS of HD, we investigated the enteric nerve responses on lesional and normal IAS in patients with HD and in patients who had undergone rectal amputation for low rectal cancer.
METHODOLOGY: Twelve preparations were taken from the lesional IAS of 7 patients with HD (5 boys and 2 girls aged between 6 months and 1.5 years with a mean age

Maladaptive sensory coding within the enteric nervous system (ENS) is increasingly implicated in gastrointestinal dysmotility, abnormal secretion, neuroimmune dysfunction, and visceral hypersensitivity. Intrinsic primary afferent neurons (IPANs), classically corresponding to Dogiel type II/AH neurons, constitute a major sensory arm of intrinsic enteric reflex circuits. They detect mechanical, chemical, epithelial, microbial, and immune-derived signals and relay this information to interneurons, motor neurons, and secretomotor pathways, thereby coordinating gastrointestinal function independent…

The enteric nervous system (ENS) serves as the principal regulator of intestinal muscular activity in the poultry gastrointestinal tract, modulating secretory processes and digestive functions, while also contributing to immune protection at the muscularis level against pathogen infections. While ENS structure and function are well studied in mammals, knowledge of the poultry ENS remains limited. In this study, we provide the first comprehensive characterization of neuronal populations and structural organization within the three‑dimensional environment of the chicken ENS using tissue clearing…

Mucosal immune responses to enteric bacterial infections are highly coordinated processes that orchestrate host protection while minimizing the potential for immune-triggered pathology. In the intestinal tract, bidirectional communication occurs between the nervous and immune systems to affect local immune responses by modulating the activity of resident and recruited immune cells, and indirectly on the supporting stromal cells. These neuroimmune signaling pathways that alter host defense have focused on specialized sensory innervation and the unique neurotransmitters released from them. Altho

Major advances have been achieved in imaging technologies but most methodological approaches currently used to study the enteric neuronal functions rely on exogenous contrast dyes that can interfere with cellular functions or survival. In the present paper, we investigated whether full-field optical coherence tomography (FFOCT), could be used to visualize and analyze the cells of the enteric nervous system. Experimental work on whole-mount preparations of unfixed mouse colons showed that FFOCT enables the visualization of the myenteric plexus network whereas dynamic FFOCT enables to visualize …

The communication between the intestinal epithelium and the enteric nervous system has been considered indirect. Mechanical or chemical stimuli activate enteroendocrine cells inducing hormone secretion, which act on sub-epithelial nerve ends, activating the enteric nervous system. However, we identified an epithelial cell that expresses NKAIN4, a neuronal protein associated with the β-subunit of Na+/K+-ATPase. This cell overexpresses Na+/K+-ATPase and ouabain-insensitive Na+-ATPase, enzymes involved in active sodium transport. NKAIN4-positive cells also express neuronal markers as NeuN, acetyl…

Normal activity and functional integration of the enteric nervous system (ENS) into the gut tissue circuitry and the luminal ecosystem are essential for digestive physiology and human health. A range of debilitating gastrointestinal disorders are linked to ENS dysfunction, caused either by developmental deficits, such as congenital megacolon (Hirschsprung's disease-HSCR) or a host of acquired intestinal neuropathies with unclear molecular or cellular pathogenesis. Recent advances in cell engineering underscore the potential use of cell replacement technologies for the treatment of ENS disorder…

Enteric nervous system (ENS) development is governed by interactions between neural crest cells (NCC) and the extracellular matrix (ECM). Hirschsprung disease (HSCR) results from incomplete NCC migration and failure to form an appropriate ENS. Prior studies implicate abnormal ECM in NCC migration failure. We performed a comparative microarray of the embryonic distal hindgut of wild-type and EdnrBNCC-/- mice that model HSCR and identified laminin-β1 as upregulated in EdnrBNCC-/- colon. We identified decreased expression of 37/67 kDa laminin receptor (LAMR), which binds laminin-β1, in human HSCR…

The gut-brain axis is of crucial importance for controlling glucose homeostasis. Alteration of this axis promotes the type 2 diabetes (T2D) phenotype (hyperglycaemia, insulin resistance). Recently, a new concept has emerged to demonstrate the crucial role of the enteric nervous system in the control of glycaemia via the hypothalamus. In diabetic patients and mice, modification of enteric neurons activity in the proximal part of the intestine generates a duodenal hyper-contractility that generates an aberrant message from the gut to the brain. In turn, the hypothalamus sends an aberrant efferen…

The enteric nervous system (ENS) of the gastrointestinal tract controls many diverse functions, including motility and epithelial permeability. Perturbations in ENS development or function are common, yet there is no human model for studying ENS-intestinal biology and disease. We used a tissue-engineering approach with embryonic and induced pluripotent stem cells (PSCs) to generate human intestinal tissue containing a functional ENS. We recapitulated normal intestinal ENS development by combining human-PSC-derived neural crest cells (NCCs) and developing human intestinal organoids (HIOs). NCCs…

Intestinal function is primarily controlled by an intrinsic nervous system of the bowel called the enteric nervous system (ENS). The cells of the ENS are neural crest derivatives that migrate into and through the bowel during early stages of organogenesis before differentiating into a wide variety of neurons and glia. Although genetic factors critically underlie ENS development, it is now clear that many non-genetic factors may influence the number of enteric neurons, types of enteric neurons, and ratio of neurons to glia. These non-genetic influences include dietary nutrients and medicines th…

UNLABELLED: Elucidation of the cellular identity of neuronal precursors provides mechanistic insights into the development and pathophysiology of the nervous system. In the enteric nervous system (ENS), neurogenesis persists from midgestation to the postnatal period. Cellular mechanism underlying the long-term neurogenesis in the ENS has remained unclear. Using genetic fate mapping in mice, we show here that a subset of Schwann cell precursors (SCPs), which invades the gut alongside the extrinsic nerves, adopts a neuronal fate in the postnatal period and contributes to the ENS. We found SCP-de…
