Right ventricular outflow tract endocardial unipolar substrate mapping: implications in risk stratification of Brugada syndrome.

Brugada syndrome (BrS) is a complex arrhythmogenic disease displaying electrical and micro-structural abnormalities mainly located at the epicardium of the right ventricular outflow tract (RVOT). It is well-known that fibrosis, fatty infiltration, inflammation and reduced gap junction expression have been demonstrated at the epicardial anterior aspect of the RVOT providing the arrhythmogenic substrate for ventricular arrhythmic events in BrS. A number of models have been proposed for the risk stratification of patients with BrS. Endocardial unipolar electroanatomical mapping is an emerging too

Catheter Ablation of Triggers and Arrhythmogenic Epicardial Substrates in Patients with QT Prolongation.

BACKGROUND AND AIMS: Long QT syndrome (LQTS) ablation is currently limited to high-risk or refractory cases, targeting the elimination of triggers such as ventricular premature contraction (VPC). However, the characteristics of the underlying substrate and the role of radiofrequency ablation have not been extensively discussed. METHODS AND RESULTS: Eight symptomatic patients with LQTS (all women; aged 36 ± 10.2 years) underwent 3-D endocardial and epicardial mapping of triggers and/or substrates of ventricular arrhythmias. Genetic testing revealed P1093A mutations in the KCNH2 gene, H558R mut…

Subxiphoid epicardial approach for catheter ablation of refractory perimitral atrial flutter: a case series.

BACKGROUND: Perimitral atrial flutter is a complex atrial tachycardia involving endocardial and epicardial structures. The standard ablation strategy includes endocardial radiofrequency and vein of Marshall ethanol infusion, but restoring sinus rhythm can be challenging even in experienced centres. CASE SUMMARY: We report the cases of three male patients (60-, 75-, and 58-year-old) with persistent and symptomatic atypical atrial flutter. They shared a previous history of numerous atrial fibrillation catheter ablations. Endocardial left atrial activation mapping found a perimitral macro-reentr

Direct and indirect mapping of intramural space in ventricular tachycardia.

BACKGROUND: The ventricular tachycardia (VT) circuit is often assumed to be located in the endocardium or epicardium. The plateauing success rate of VT ablation warrants reevaluation of this mapping paradigm. OBJECTIVE: The purpose of this study was to resolve the intramural components of VT circuits by mapping in human hearts. METHODS: Panoramic simultaneous endocardial-epicardial mapping (SEEM) during intraoperative mapping (IOM) was performed in human subjects. In explanted hearts (EH), SEEM and intramural multielectrode plunge needle mapping (NM) of the left ventricle were performed. Ove…

Peak-to-Peak Voltage Mapping Reveals Dynamic Arrhythmogenic Substrate Behavior in Isolated Rabbit Hearts.

Cardiac arrhythmias are associated with structural and electrical remodeling processes that generate heterogeneous arrhythmogenic substrates characterized by low-voltage areas and abnormal conduction. Although electroanatomical voltage mapping is widely used for substrate characterization, epicardial voltage thresholds remain highly dependent on species, chamber, and recording configuration, and have not been systematically established for isolated rabbit hearts. We hypothesized that healthy rabbit epicardial tissue exhibits reproducible chamber-specific voltage distributions and that arrhythm

Electrophysiologic evidence of epicardial connections between low right atrium and remote right atrial region or coronary sinus musculature: Relevance for catheter ablation of typical atrial flutter.

BACKGROUND: The coronary sinus (CS) is surrounded by a myocardial coat with extensive connections to the left and right atria that contributes to the interatrial electrical connection. Whereas epicardial connections between CS musculature and the left atrium have largely been demonstrated, clinically relevant epicardial connections from the CS musculature toward the low right atrium (LRA) and epicardial connections between two regions of the right atrium remain questionable. METHODS: Five patients underwent electrophysiology (EP) study for typical atrial flutter (AFl) using either conventiona

Transseptal conduction as an important determinant for cardiac resynchronization therapy, as revealed by extensive electrical mapping in the dyssynchronous canine heart.

BACKGROUND: Simple conceptual ideas about cardiac resynchronization therapy assume that biventricular (BiV) pacing results in collision of right and left ventricular (LV) pacing-derived wavefronts. However, this concept is contradicted by the minor reduction in QRS duration usually observed. We investigated the electric mechanisms of cardiac resynchronization therapy by performing detailed electric mapping during extensive pacing protocols in dyssynchronous canine hearts. METHODS AND RESULTS: Studies were performed in anesthetized dogs with acute left bundle-branch block (LBBB, n=10) and chro

Epicardial left lateral accessory pathways: an uncommon cause of ablation failure-insights from a case series.

BACKGROUND: Catheter ablation is the treatment of choice for patients with recurrent, symptomatic accessory pathway (AP)-related tachycardias and for asymptomatic patients with high-risk pathways. Catheter ablation is associated with high success rates and low risk of complications, particularly when the pathway is located at the mitral annulus. Epicardial left lateral APs are rare but can account for ablation failures, posing a significant therapeutic challenge. CASE SUMMARY: We report two young patients with recurrent paroxysmal supraventricular tachycardias related to a left lateral AP. In

Open-chest epicardial ablation of ventricular tachycardia during a left ventricular assist device implantation: a case report.

BACKGROUND: Ventricular arrhythmias (VAs) are common after a left ventricular assist device (LVAD) implantation. Further, the majority of po … us endocardial ablation failed due to an epicardial arrhythmogenic substrate. Therefore, open-chest epicardial mapping during the LVAD implantation was indicated and three target areas of the arrhythmogenic substrate were found, which were ablated by radiofrequency applications. To minimize the cardiopulmonary bypass time, cardiopulmonary bypass was initiated after …

Trans-subclavian approach for radiofrequency ablation of premature ventricular contractions originating from subtricuspid annulus: a case report.

BACKGROUND: Catheter ablation has been established as a curative treatment strategy for ventricular arrhythmias. The standard procedure of m … ivation site via the right femoral vein could not eliminate the premature ventricular contractions. Epicardial mapping could not obtain an earlier ventricular activation when compared to the endocardial mapping, and at the earliest epicardial site could not provide an identical pace mapping. Finally, we redeployed the ablation catheter via the right subclavian vein …

Epicardial macro-reentrant ventricular tachycardia exhibiting an endocardial centrifugal activation pattern in a case with arrhythmogenic right ventricular cardiomyopathy.

Macro-Reentrant ARVC epi-VT with a Focal Endo-Activation. A 55-year-old man with arrhythmogenic right ventricular cardiomyopathy underwent catheter ablation of ventricular tachycardia (VT) with left bundle branch block and left superior axis QRS morphology with an early precordial transition. Endocardial mapping during the VT revealed a focal activation pattern from a small region of low voltage in the left ventricular (LV) septum. Despite earliest endocardial activation in the LV septum, epicardial mapping demonstrated a macro-reentrant circuit with successful catheter ablation at an inferior…

Intrapericardial cardiosphere-derived cells hinder epicardial dense scar expansion and promote electrical homogeneity in a porcine post-infarction model.

The arrhythmic substrate of ventricular tachycardias in many structural heart diseases is located in the epicardium, often resulting in poor outcomes with currently available therapies. Cardiosphere-derived cells (CDCs) have been shown to modify myocardial scarring. A total of 19 Large White pigs were infarcted by occlusion of the mid-left anterior descending coronary artery for 150 min. Baseline cardiac magnetic resonance (CMR) imaging with late gadolinium enhancement sequences was obtained 4 weeks post-infarction and pigs were randomized to a treatment group (intrapericardial administration

Epicardial muscle bundles in persistent atrial fibrillation: from overlooked bystanders to arrhythmogenic modulators.

Persistent atrial fibrillation (PsAF) has a complex and multi-factorial disease substrate. Catheter ablation is a standard treatment but often fails to maintain long-term sinus rhythm. One reason for this failure involves epicardial muscle bundles. Structures like the Marshall bundle, Bachmann bundle, and septopulmonary bundle are usually bypassed during initial endocardial ablation. Current evidence highlights how these epicardial structures modulate the atrial substrate and contribute to arrhythmia recurrence. Animal and human mapping studies indicate that these muscle bundles interact with

Role of high-resolution image integration to visualize left phrenic nerve and coronary arteries during epicardial ventricular tachycardia ablation.

BACKGROUND: Epicardial ventricular tachycardia (VT) ablation is associated with risks of coronary artery (CA) and phrenic nerve (PN) injury. We investigated the role of multidetector computed tomography in visualizing CA and PN during VT ablation. METHODS AND RESULTS: Ninety-five consecutive patients (86 men; age, 57 ± 15) with VT underwent cardiac multidetector computed tomography. The PN detection rate and anatomic variability were analyzed. In 49 patients undergoing epicardial mapping, real-time multidetector computed tomographic integration was used to display CAs/PN locations in 3-dimens…

Re-entrant ventricular tachycardia as a complication of ablation of idiopathic ventricular premature beats from the right outflow tract: a case report.

BACKGROUND: We report an unusual case of non-sustained ventricular tachycardia (NSVT) from the epicardial part of the right ventricular outflow tract (RVOT). CASE SUMMARY: A 37-year-old woman who underwent in 2006 an ablation for idiopathic ventricular premature beats (VPBs) from the RVOT presented with pre-syncopal NSVT in 2016. A cardiac workup showed no coronary disease, normal biventricular function, and no enhancement on cardiac magnetic resonance imaging. A metabolic positron emission tomography scan excluded inflammation. Biopsies revealed normal desmosomal proteins. An endocardial map

Open chest epicardial and transapical endocardial substrate ablation for ventricular tachycardia with left ventricular aneurysm in a porcine model.

BACKGROUND: Endo-epicardial radiofrequency catheter ablation (RFCA) of ventricular tachycardia (VT) as a first-line strategy has been shown to improve outcomes. This study sought to evaluate the feasibility and validity of open-chest epicardial and transapical endocardial substrate ablation for VT with left ventricular aneurysm (LVA) applying to routine cardiac surgery. METHODS: Porcine models of LVA with VT were developed and were divided into a study group (RFCA from the epicardium via direct-view and endocardium via transapical access) and a control group (endocardial RFCA via retrograde t

Epicardial Ablation of Idiopathic Ventricular Tachycardia.

Ventricular arrhythmias (VAs) occurring in the absence of structural heart disease or ion channelopathies are referred to as idiopathic. They can clinically present with frequent monomorphic premature ventricular contractions, nonsustained ventricular tachycardia (VT), or sustained VT, and generally share a benign prognosis. Approximately 4% to 10% of idiopathic VAs have an epicardial site of origin, represented in most cases by the left ventricular summit and, less frequently, by the cardiac crux. Epicardial foci can be addressed by catheter ablation via the coronary venous system tributaries

Failure of surgery in preexcitation. Correlation with pathologic findings.

A 37-year-old man with mitral stenosis and recurrent drug-resistant paroxysmal atrial fibrillation, paroxysmal supraventricular tachycardia, and preexcitation, underwent two surgical attempts to ablate an anomalous pathway (AP). Electrophysiologic study demonstrated a left posterior AP with a short antegrade refractory period. Epicardial mapping at the time of mitral valve replacement (left lateral thoracotomy) suggested a posterior right AP. Mitral valve replacement and incision of the left atrial wall failed to cure preexcitation. Epicardial mapping at a second operation (median sternotomy) …
