Acute epiglottitis in a community hospital.

Acute epiglottitis is a fulminating pediatric emergency which should be managed with antibiotics and the establishment of an airway. Either tracheostomy or nasotracheal intubation is employed for airway intervention. Because of the morbidity associated with tracheostomy, recent reports have stressed the use of nasotracheal intubation. Experience in 21 cases of acute epiglottitis emphasizes nasotracheal intubation as the treatment of choice in a community hospital.

Epiglottitis. Twenty-year study with tracheotomy.

One hundred and seventy patients with acute epiglottitis were admitted to the Childrens Hospital of Los Angeles between the years 1957 to 1977. No deaths occurred in any of these patients, all of whom were treated with tracheotomy to support and ensure the airway. The records of 147 of these patients were available for review. No serious complications occurred from tracheotomy in any of the patients in this study. Tracheotomy is a safe method for caring for the airway problem in epiglottitis in a setting such as is available at the Childrens Hospital of Los Angeles. The median range of total d…

Acute epiglottitis in infants and children.

Sixty-one cases of acute epiglottitis at the Royal Alexandra Hospital for Children, Sydney, from July 1968 to December 1974 are reviewed. The average age of 2.7 years is lower than previously reported. The average time from the initial symptom to arrival at the hospital was 14 hours. The average time from arrival to performance of tracheotomy was two hours. The diagnosis was made when first seen and assessed in 50 of the 61 cases. There were eight respiratory arrests in the Casualty and five of these were successfully resuscitated. We believe that an artificial airway is necessary in most case…

Giant Epiglottic Inclusion Cyst.

Epiglottic inclusion cysts are extremely rare benign laryngeal lesions. A 41-year-old woman suffered from persistent pharyngeal foreign body sensation for one year. Laryngoscopy detected a giant epiglottic cyst, and contrast-enhanced cervical CT showed the typical pedunculated 'apple sign'. Under fiber optic intubation, complete resection was performed by suspension laryngoscopic coblation. Histopathology verified epiglottic inclusion cyst. Pathognomonic apple sign significantly contributes to precise preoperative diagnosis and individualized surgical planning. Transoral plasma resection provi

A case of epiglottic entrapment in a cat.

OBJECTIVE: The objective of this study was to describe a case of epiglottic entrapment in a cat. CASE SUMMARY: A 5-month-old male neutered Russian Blue cat was evaluated for progressive stertorous upper airway sounds, acute onset vestibulopathy and abnormal laryngeal anatomy. Endotracheal intubation was only able to be achieved using videoscopic guidance and identified concern for severe nasopharyngeal stenosis. A computerized tomography scan revealed otitis interna, narrowed nasopharynx and no definitive cause for the stertorous breathing. The cat recovered very slowly from anaesthesia due t

Neonatal Lateral Epiglottic Defects.

INTRODUCTION: Multiple congenital abnormalities of the epiglottis have been reported and iatrogenic injuries to the larynx and subglottis are well known. We present a new pattern of defect not previously reported in the literature. METHODS: Epiglottic abnormalities at two institutions are reviewed. Cases of defects involving the lateral aspect of the epiglottis and aryepiglottic fold are identified. A literature review of known epiglottic defects is performed. RESULTS: Two children possessing lateral notch injuries at the aryepiglottic attachment to the epiglottis are described. Both childre

Acoustic Characteristics in Epiglottic Cyst.

OBJECTIVE: The purpose of this study was to analyze the acoustic characteristics associated with alternation deformation of the vocal tract due to large epiglottic cyst, and to confirm the relation between the anatomical change and resonant function of the vocal tract. METHODS: Eight men with epiglottic cyst were enrolled in this study. The jitter, shimmer, noise-to-harmonic ratio, and first two formants were analyzed in vowels /a:/, /e:/, /i:/, /o:/, and /u:/. These values were analyzed before and after laryngeal microsurgery. RESULTS: The F1 value of /a:/ was significantly raised after sur

Caustic and thermal epiglottitis in the adult.

The presence of dysphagia, drooling, and stridor in an adult subsequent to thermal or caustic injury should alert the treating physician to the possibility of injury to the supraglottic structures with resultant epiglottitis. These adults possess many of the features seen in acute infectious epiglottitis and should be handled with the same consideration for potential upper airway obstruction. Epiglottic injuries of this type should be suspected in adults with mental disorders or communication difficulties.

Airway-obstructing epiglottic cyst.

Laryngeal cysts, particularly epiglottic cysts, are generally benign lesions which cause mild dysphagia or hoarseness. We report a case of an epiglottic cyst that caused almost complete airway obstruction. A 43-year-old man presented with progressive dysphagia, hoarseness, and airway obstruction secondary to a large cystic mass involving the entire epiglottis and filling the hypopharynx. He required emergency tracheotomy to secure the airway, and the cyst was incised and drained. Following a subsequent recurrence, laser excision of the cyst was performed and the problem resolved. The cystic ma

Equine epiglottitis: Diagnosis, treatment and outcome.

BACKGROUND: Epiglottitis is an uncommon upper airway pathology manifested endoscopically by mucosal oedema, reddening and thickening of the epiglottic mucosa. Literature on epiglottitis in horses is sparse. OBJECTIVES: To describe the diagnosis, treatment and outcome of Thoroughbred and Standardbred racehorses diagnosed with epiglottitis at a single centre. A secondary objective is to demonstrate the efficacy of inhaled and topical anti-inflammatories for this condition. STUDY DESIGN: Retrospective case series. METHODS: Medical records (2009-2023) were reviewed for racehorses treated for ep…

A Case of Brodifacoum-Induced Epiglottitis.

This case report presents a 33-year-old woman who presented to the emergency department with abdominal pain and gingival and vaginal bleedin … tubated two days later. Her INR fully normalized, and she was then discharged on day 4. Our case of epiglottitis could demonstrate thermal injury associated with smoking synthetic cannabinoids, but given diffuse ecchymosis and severe coagulopathy, hematoma associated with brodifacoum poisoning was considered the most likely etiology. The patient's coagulopathy …

[A Clinical Study of Acute Epiglottitis].

We carried out a retrospective review of 285 cases of acute epiglottitis (180 males, 105 females, mean age 49.6 years) that required inpatients hospital care between 1998 and 2014. All the patients complained of sore throat, and 62 patients complained of respiratory discomfort; 17 patients had severe dyspnea, and 27 patients required airway management (tracheotomy in 25, cricothyroidotomy in 2 patients). All the patients survived. As acute epiglottitis can cause rapidly progressive airway obstruction and death, emergent airway management should be undertaken in patients with dyspnea. However, …
