Wedge-shaped epiphyses of the knees in two siblings: a new recessive rare dysplasia?

A peculiar form of metaphyseal dysplasia, mainly of the lower limbs, occurred in two male siblings born to healthy, unrelated parents. The clinical and radiological features were short stature, psychomotor retardation, accelerated bone maturation, the limbs and especially the knees showing cup-shaped widening of the ends of the metaphyses and wedge-shaped widening of the epiphyses. This condition does not fit the description of any syndrome reported so far and may therefore be classified as a new recessive dysplasia.

Modified Dunn procedure versus percutaneous pinning in moderate/severe stable slipped capital femoral epiphyses.

BACKGROUND: The modified Dunn procedure (MDP) has risen enthusiasm in treating slipped capital femoral epiphyses (SCFE) due to the anatomic reduction and high patients' satisfaction rates at long-term follow-up. Main aim of this study is to compare clinical and radiographic outcomes of 2 cohorts with moderate to severe stable SCFE treated by MDP and in situ fixation. METHODS: Medical records were analysed to collect demographic data, comorbidities and time from slip to surgery. The collected postoperative data were: avascular necrosis (AVN); complications; progression of osteoarthritis and su…

Cartilaginous epiphyses in extant archosaurs and their implications for reconstructing limb function in dinosaurs.

Extinct archosaurs, including many non-avian dinosaurs, exhibit relatively simply shaped condylar regions in their appendicular bones, sugge … were dissected, disarticulated, and defleshed. Lengths and condylar shapes of elements with intact epiphyses were measured. Limbs were subsequently completely skeletonized and the measurements repeated. Removal of cartilaginous condylar regions resulted in statistically significant changes in element length and condylar breadth. Moreover, there was marked …

Lissencephaly type III, stippled epiphyses and loose, thick skin: a new recessively inherited syndrome.

We report on two new cases of syndromic lissencephaly in two consanguineous sibs, with skeletal abnormality, born to young, healthy, second cousin parents with healthy children. In Case 1, fetal ultrasound screening at 32 weeks of gestation showed microcephaly, skin infiltration and equinovarus feet. MRI disclosed cerebral agyria, hypoplastic cerebral mantle and posterior agenesis of the corpus callosum. The propositus, a boy, died soon after birth at term. In Case 2, fetal ultrasound study performed at 16 weeks of gestation disclosed skin infiltration. MRI at 22 weeks of gestation showed micr

[The surface of epiphyses of the knee: index of the duration of neonatal hypothyroidism].

The surfaces of the epiphyses of the knees were calculated in 34 neonates with hypothyroidism detected with a systematic screening and in 32 normal neonates. In the group of neonates with hypothyroidism, the surface of the lower femoral point was 12.7 +/- 0.9 mm2 and that of the upper tibial point 1.8 +/- 0.9 mm2. There was a significant difference (P less than 0.01) for both epiphyses between children of the 2 groups. The use of mathematical formula excluding non specific factors showed significant correlation between the corrected values for lower femoral points, and T4 and T3 plasma levels.…

Contribution of the epiphyses of the greater trochanter to the growth of the femur.

We have demonstrated the interaction of the capital femoral epiphysis and the greater trochanteric epiphysis in rats to give length and shape to the proximal femur. These findings have practical applications in the treatment of congenital coxa vara and other disease entities of the femoral head.

A single stage osteotomy and epiphysiodesis for treatment of moderately displaced femoral capital epiphyses.

Epiphysiodesis and subtrochanteric osteotomy have long been recognized as valuable procedures in the treatment of the patient with a moderately or markedly displaced femoral capital epiphysis. Typically these have been performed individually or sequentially. The operation herein described combines the two in one single-stage procedure. Using a stainless steel Lorenzo Screw, the epiphyseal plate is transfixed and further capital displacement prevented. The subtrochanteric compensatory osteotomy is performed using simple visual clinical guidelines and is held rigidly immobilized with a variable

Apophyseal and epiphyseal knee injuries in the adolescent athlete.

OBJECTIVES: In the context of pediatric sports injuries, the epiphyseal and apophyseal knee fractures represent rather peculiar lesions. The most frequently involved anatomical area is the knee. The peculiar function of the physis and the need to preserve their integrity, makes choosing what treatment methods to employ very important. Objective of this study is to assess the kind and the effectiveness of the most suitable treatment in the apophyseal and epiphyseal knee lesions occurring in the adolescents. MATERIALS AND METHODS: From 2006 to 2011, were treated 41 patients (34 M-7 F) between t

Review of distal tibial epiphyseal transitional fractures.

The closure of the distal tibial physis occurs over an 18-month period between ages 12 and 15 years. During this time period, children are susceptible to several transitional fractures, so labeled because they are transitioning to skeletal maturity. In the setting of an external rotation force, triplane and Tillaux fractures can occur. These fractures, which present similarly to other Salter-Harris growth plate injuries, do not fit neatly into any 1 classification scheme and are not easily evaluated on plain radiographs. Computed tomography scans are required to optimally assess these fracture

Epiphyseal bar resection and fat interposition for clinodactyly.

Clinodactyly, the angulation of a digit in the anteroposterior plane, is often due to a longitudinal epiphyseal bracket on the radial side of the middle phalanx of the little finger. Treatment options include observation, osteotomy, and epiphyseal bar resection. Epiphyseal bar resection is a simple surgery that requires neither postoperative pin fixation nor immobilization. The most appropriate indications are in children 3 to 6 years old with radial deviation of at least 25 degrees. The procedure reliably diminishes the extent of deformity.

Multiple epiphyseal dysplasia in children: beware of overtreatment!

PURPOSE: To determine the various musculoskeletal manifestations of multiple epiphyseal dysplasia in children, and the course of this disease in childhood. METHODS: Ten children were diagnosed and treated at the Children's Hospital of Eastern Ontario (CHEO) for multiple epiphyseal dysplasia, 1976-2001. We reviewed the clinical, pathological and radiographic records for these cases to determine cause of presentation and progress of the musculoskeletal pathology during the course of this disease in this age group. RESULTS: Average age at time of first presentation was 6 years and 4 months (ran

Epiphyseal osteoblastoma of tibia with xanthomatous stromal reaction.

Osteoblastoma occurring in long bones has a distinctive predilection for the metaphysis and the diaphysis. Epiphyseal location is rare. Although variation in histologic patterns is a well-known feature of this tumor, xanthomatous stromal reaction has not yet been described. We report a case of a 34-year-old man who developed an osteoblastoma primarily located in the epiphysis of his left tibia with extension into the metaphysis. The striking histologic features included a prominent xanthoma-like stroma consisting of foamy histiocytes in addition to focal areas with classical configuration of a

Severely displaced proximal humeral epiphyseal fractures.

The purpose of this study was to document the late outcome of a group of patients with Neer grade III/IV proximal humeral physeal fractures who were treated with reduction of the fracture and maintenance of reduction until fracture consolidation. A total of 28 patients treated between 1984 and 1999 at a large children's hospital were included in this study. Nineteen of the 28 patients were 15 years or older (range 5-16 y). All patients were treated in the operating room with closed reduction followed by immobilization (n = 3), closed reduction and pin fixation (n = 20), open reduction and scre
