Computed tomographic features of renal osteodystrophy.

The spectrum of musculoskeletal abnormalities seen on routine computed tomographic (CT) examinations of five patients with renal osteodystrophy are described. CT findings included multiple brown tumors, osteitis fibrosa cystica, abnormal sacroiliac joints, periarticular tumoral calcifications, prominent Schmorl's nodes, and slipped capital femoral epiphyses. In this small group of patients, the musculoskeletal abnormalities ranged from the subtle to the dramatic. Although CT examination is rarely used as a screening test for renal osteodystrophy, it is important to be familiar with its many ap

[Complications of slipped capital femoral epiphysis].

Slipped capital femoral epiphysis is well known disorder of the hip in adolescents, which is characterized by displacement of the capital femoral epiphysis from the metaphysis through the physeal plate. The incidence of slipped capital femoral epiphysis is about 5-8 cases per 100,000 adolescents. Etiology of slipped capital femoral epiphysis is still unknown, but this disorder is probably combination of genetic, hormonal and mechanical factors. On the basis of patient's history, physical examination, and radiographs, slipped capital femoral epiphysis can be classified as acute or chronic. Two

Slipped epiphysis associated with hypothyroidism.

Seven cases of slipped capital femoral epiphysis appearing concurrently with juvenile hypothyroidism are reported. This association seems to occur more commonly than the general incidence of slipped capital femoral epiphysis or of juvenile hypothyroidism would suggest, and the thyroid hormone deficiency and a diminished strength of the epiphyseal growth plate of the upper femur appear to be associated. Although concrete scientific data are as yet not available to define clearly the effect of thyroid hormone on the growth plate, the possible pathophysiology is discussed in light of available da

The ultrastructure of the growth plate in slipped capital femoral epiphysis.

Core biopsy specimens of the proximal femoral growth plate from three patients with slipped capital femoral epiphysis were studied by light and electron microscopy. In the resting zone, the cartilage matrix was composed of large, densely packed collagen fibrils. The cartilage matrix in the zones of chondrocyte hypertrophy where the slippage occurred contained only scattered fine collagen fibrils in a homogeneous ground substance. The change in composition of cartilage matrix in the distal region of the epiphyseal plate may predispose that region to slippage.

The torsional basis for slipped capital femoral epiphysis.

Fifteen moderate to severe acute slipped capital femoral epiphyses were manipulated in 15 patients to correct significant deformities; in each case, torsion played a critical role in the reduction. Nine of 15 patients with acute slips underwent controlled, open reduction from the anterolateral approach, stabilizing the epiphysis while the metaphysis was internally rotated, flexed, and abducted; none had osteonecrosis or chondrolysis. The other 6 acute slips were treated with traction using internal rotation to achieve closed reduction; 2 of these 6 closed reductions progressed to osteonecrosis

Anterior minimally invasive subcapital osteotomy without hip dislocation for slipped capital femoral epiphysis.

PURPOSE: A minimally invasive anterior approach appears to be an attractive alternative to achieve capital realignment without violating femoral head vascular supply and avoiding hip dislocation in slipped capital femoral epiphysis. The aim of this study was to detail the technical steps of subcapital realignment through a minimally invasive anterior approach and to report the preliminary results of this procedure in a prospective cohort of patients with stable slips. METHODS: Nine patients underwent subcapital cuneiform wedge resection through a minimally invasive anterior approach without h

Slipped capital femoral epiphysis. Incidence and clinical assessment of physeal instability.

In an unselected series of 55 cases of slipped capital femoral epiphysis (SCFE) we observed an incidence of 25% of epiphyseal reduction, mostly unintentional. Reduction indicated physeal instability and was associated with an effusion, detected by sonography on admission, and inability to bear weight. The true prevalence of instability may be higher since an effusion was noted in 33 cases (60%) on the initial sonographic assessment. Serial radiographs showed reduction in 12 (22%), with an average change of 15.1 degrees in the head-neck angle. Serial sonography showed reduction in 7 out of 20 c

Intraobserver and Interobserver Reliability of the Peritubercle Lucency Sign in Slipped Capital Femoral Epiphysis.

BACKGROUND: An area of enlargement of the metaphyseal socket around the epiphyseal tubercle, termed the peritubercle lucency sign, has recently been introduced as a possible predictor of contralateral slipped capital femoral epiphysis in patients with previous unilateral slipped capital femoral epiphysis. This study aimed to assess intraobserver and interobserver reliability for detecting the presence or absence of the peritubercle lucency sign. METHODS: Thirty-five radiographs were presented to 6 fellowship-trained pediatric orthopaedic surgeons on 2 separate occasions 30 days apart, ensurin

Treatment of slipped capital femoral epiphysis with a spica cast.

Thirteen patients who had seventeen slipped capital femoral epiphyses were managed with a spica cast between 1984 and 1986. The average time in the plaster cast was three months. Complications were noted in fourteen of the hips. Three pressure sores developed in two patients. Further slipping developed in three hips once the cast had been removed, and chondrolysis developed in one of these hips. Chondrolysis developed in eight additional hips, and the lesion was transient in four of them. Degenerative changes developed in all nine hips with chondrolysis, regardless of whether the chondrolysis

Corrective Imhäuser intertrochanteric osteotomy.

OBJECTIVE: Surgical correction of deformities of the proximal femur caused by epiphyseal displacement by restorative (usually inflection and internal rotation) osteotomy at the intertrochanteric level with the aim of reducing both the offset disorder of the coxal end of the femur that is causing impingement and the prevalence of secondary coxarthrosis. INDICATIONS: Chronic and subacute manifestations of slipped capital femoral epiphysis with an epiphyseal dislocation in the radiographic axial view of 30-60 degrees (ET angle) and DeltaED > 20 degrees (CCD angle minus ED angle) in the anteropos

Biomechanical analysis of in situ single versus double screw fixation in a nonreduced slipped capital femoral epiphysis model.

Slipped capital femoral epiphyses (SCFE) were created in 24 pairs of immature bovine femurs. In 17 pairs of femurs, the slip was left nonreduced (one-third diameter of physis), and in 7 pairs, the slip was reduced. Stabilization of the slips was with either 1 or 2 threaded 6.5-mm screws in a compression mode. The specimens were subjected to shear or torsional loading forces to failure, with the goal of trying to reproduce clinical conditions of in situ screw fixation for acute or unstable SCFE. In the nonreduced model, double-screw fixation was 312% stiffer than single-screw fixation under tor

The removal of metalwork in children.

The post-operative complications in 57 children under the age of 16 years requiring removal of metalwork during a 12-month period are presented. The grade of surgeon performing the operation is also discussed. The overall complication-rate was 14 per cent. Minor complications such as transiently painful scars and superficial infection occurred in 7 per cent of cases. There was one intermediate complication following removal of Knowles pins of an unacceptable scar requiring revision by a plastic surgeon. The major complication (7 per cent of all cases) involved fracture with retention of the di

In situ fixation of the slipped capital femoral epiphysis with a single screw.

We made a retrospective analysis of a consecutive series of patients with slipped capital femoral epiphyses (SCFE) surgically treated at the Shriners Hospitals for Crippled Children at Lexington, Kentucky. The clinical outcome was correlated to the number of pins and the incidence of chondrolysis, ischemic necrosis, and progressive slipping after the initial fixation. An evaluation made of the Walters-Simon theoretic pin penetration formula was of no value in this series. We compiled results in the medical literature and compared them with ours; all information indicated that a single pin thro

Slipped capital femoral epiphysis: rationale for the technique of percutaneous in situ fixation.

In pinning a slipped capital femoral epiphysis (SCFE), the position of the pin within the center of the femoral head is important for two reasons. The pin may disrupt the lateral epiphyseal artery and cause avascular necrosis (AVN), or pin penetration in certain locations may go unrecognized on radiographs. To place the pin accurately, the surgeon must be aware of where the femoral head lies in relation to the femoral neck and the shaft. Radiographs of models and of patients in various positions support the view that the femoral head in most cases of chronic SCFE rotates around the axis of the

Slipped capital femoral epiphysis during treatment with recombinant human growth hormone for Turner syndrome.

Recombinant human growth hormone has recently been used to treat the short stature of Turner syndrome. However, this therapy may induce slipped capital femoral epiphysis, since the epiphyseal plate widens and becomes weak through either growth hormone or hypogonadism. This report shows that a slipped capital femoral epiphysis occurred during this treatment for an 11-year-old girl who was suffering from Turner syndrome. Thus, these patients who are being treated with growth hormone should be carefully observed.
