A 16-year-old cryptorchid Quarter House with colic had a large, lobulated soft-tissue mass to the left of the pelvic inlet. At surgery, 2 large multilobulated pedunculated masses were removed. A large blood vessel enveloped by one of the masses was damaged, and the horse exsanguinated. Postmortem examination of the abdomen revealed the masses to be malignant seminoma, with multiple sites of metastasis.

Complex suicides represent only a small number of all suicides. The case of a 27-year-old man who drove his car against a tree is presented. The emergency physician diagnosed a stab to the thorax. A kitchen knife was found on the rear seats of the vehicle. Autopsy revealed death by exsanguination due to perforation of the left cardiac ventricle. Blunt force injuries caused by the collision were also found.

The authors report on the suicide of a 27-year-old woman with extreme self-inflicted injuries. The victim suffered from schizophrenic psychosis with several stays in mental institutions and one previous suicide attempt. Autopsy revealed multiple cut and stab injuries in various body regions (51 stabs to the chest, cutting off parts of ears and nose, stab to the eye and transection of the scalp). Death was caused by exsanguination.

Burned patients with deep venous thrombosis present a particularly perplexing challenge. They frequently require central venous catheters. Their altered skin integrity does not permit correlation with the typical changes described by the classic terminology for thrombophlebitis or its most severe forms, phlegmasia cerulea dolens or phlegmasia alba dolens. They are at risk of exsanguination or massive graft loss with lytic therapy or anticoagulation. Venous thrombectomy may be a necessary limb-saving surgical option.

Two cases are presented in which transcutaneous balloon catheter tamponade of exsanguinating infraclavicular injuries was performed in the trauma admitting area. Angiography and venography demonstrated isolated injuries of the subclavian vein tamponaded by the balloon catheter. Balloon catheter tamponade provided emergency control of bleeding in the admitting area and ultimately definitive treatment of the venous injury.

The objective of the study is to describe the causes of cardiac arrest and complications of cardiopulmonary resuscitation (CPR) on thoraco-abdominal CT examinations for resuscitated patients in our institution. We evaluated the causes of cardiac arrest on thoraco-abdominal CT scans, which was compared with the final diagnosis (determined by consensus of two emergency physicians based on the clinical, imaging, and laboratory findings). Additionally, we evaluated the complications of CPR on thoraco-abdominal CT scans. From March 2005 to August 2011, 82 patients underwent CT of the thorax (n=77)

INTRODUCTION: Noncompressible torso hemorrhage is the leading cause of potentially survivable battlefield death. In far-forward environments, rapid localization of aortic injury is limited by lack of fluoroscopy and advanced imaging. Intravascular ultrasound (IVUS) may enable identification of arterial injury and support targeted endovascular hemorrhage control, but quantitative physiologic data are needed to guide its use and inform development of realistic hemorrhage simulation models.
METHODS: Twenty Yorkshire-Landrace swine (70-90 kg) underwent supraceliac aortic arteriotomy to induce let

The utilization of extracorporeal membrane oxygenation (ECMO) in trauma mirrors wider trends toward increased utilization of ECMO throughout various forms of critical illness. ECMO can safely be performed on trauma patients with or without anticoagulation. Most of the trauma ECMO cases are for the management of post-traumatic respiratory failure, but they can be used for certain cases of circulatory failure as well. Cannulation of patients for ECMO is technically feasible in the hands of surgeons and intensivists involved in the care of trauma patients. A sound understanding of the ECMO circui

A 76-year-old woman was attacked by a domestic rooster on her rural property while collecting eggs. The bird pecked her lower left leg causing significant hemorrhage with collapse and death. The decedent's past medical history included treated hypertension, hyperlipidemia, non-insulin dependent diabetes mellitus and varicose veins. At autopsy the major findings were limited to the lower left leg which was covered with adherent dried blood. Two small bleeding lacerations were present, one of which was located immediately over a perforated large varix. Death was therefore due to exsanguination f

We report the case of a 65-year-old man who was attacked and killed by a female elephant that had previously escaped from a nearby circus. According to a witness the man was lifted up by the tusks, flung into the air and stomped upon. At autopsy the body showed signs of severe blunt force trauma and impalement, including multiple lacerations of the skin, a crushed thorax, a spinal fracture and a fractured humerus. The spinal cord and the major abdominal vessels were severed. There have been very few elephant attacks in Europe; this is the first reported case in Germany.

The potential complications of cardiac surgery with sternotomy include mediastinitis and major bleeding, events that are infrequent but carry high mortality. We report a unique complication of median sternotomy. A 71-year-old man underwent coronary artery bypass grafting complicated by sternal dehiscence on postoperative day 7. One week after discharge, he presented with purulent drainage from the sternal wound bed and was diagnosed as having mediastinitis. Irrigation, debridement, and sternal reconstruction were performed. Two days later, bleeding was observed in the wound, and during surgical

BACKGROUND: The use of massive transfusion protocols (MTPs) in the resuscitation of hemorrhaging trauma patients ensures rapid delivery of blood products to improve outcomes, where the decision to trigger MTPs early is important. Scores and tools to predict the need for MTP activation have been developed for use to aid with clinical judgment. We performed a systematic review to assess (1) the scores and tools available to predict MTP in trauma patients, (2) their clinical value and diagnostic accuracies, and (3) additional predictors of MTP.
METHODS: MEDLINE, EMBASE, and CENTRAL were searched

The case of a fatal, unprovoked shark attack is reported and analyzed. The incident took place on the 30th of September 2007, in the lagoon of Luengoni Bay, Lifou Island (Loyalty Islands, New Caledonia). A young French woman who was snorkeling was severely bitten on the right thigh and died of hemorrhage. An analysis based in particular on the size and color of the shark, the characteristics of the wounds, and the behavior of the shark before and after the bite suggests that the aggressor was a great white shark, Carcharodon carcharias.

We report a rare case of solitary peripheral pulmonary artery aneurysm in a patient who was evaluated for haemoptysis. Incidentally, his total antibodies were positive for Coronavirus 2019 infection. Patient underwent right lower lobectomy uneventfully. Peripheral pulmonary artery aneurysms arising from segmental or intrapulmonary branches are extremely rare. Untreated, the majority end fatally due to sudden rupture and exsanguination. The purpose of this article is to report our rare case and review the pertinent literature.

Foreign body aspiration is frequently encountered in young children, in the majority of cases it will not lead to an adverse outcome. However, in case of coin battery ingestion more serious adverse outcomes, including death, have been reported. We present a case with fatal outcome due to exsanguination, of a 2-year-old child with an aberrant right subclavian artery (or arteria lusoria) and coin battery ingestion. Radiological and autopsy findings and relevant literature are discussed.

Innominate artery rupture is a rare, but usually fatal, complication of tracheal stenosis. Although prevention is key, prompt, appropriate intervention can be life saving. Hemorrhage and airway must simultaneously be controlled. Most deaths occur from exsanguination with adequate ventilation before surgical repair can be effected. In the ideal situation, the event would occur in the operating room. We report on just such a case with concomitant respiratory failure requiring cardiopulmonary support in order to accomplish definitive therapy.

A 4-year-old dog of the Appenzeller strain with experimentally induced renal hypertension presented with apparently longstanding retinal changes, including hemorrhage, detachment, tears/holes, and anemia, in one eye and intravitreal hemorrhage in the other. The retinal tears/holes probably occurred following constant vitreous traction due to retinal hemorrhage and subsequent exsanguination of most of the retinal vasculature. This animal was reexamined six months later and most of these changes had persisted. Photodocumentation is presented.

A newborn boy had exsanguinating gastrointestinal hemorrhage in in the first week of life secondary to an unusual form of infradiaphragmatic pulmonary venous drainage. Autopsy and postmortem angiograms demonstrated huge esophageal varices as the course of the uncontrolled bleeding. The occurrence of massive gastrointestinal hemorrhage should be considered to be a rare but possibly lethal complication in patients with total anomalous pulmonary venous drainage (TAPVD) type III and IV.
