Diagnosis of pleural effusion is difficult in children. The etiologies are numerous; however, infectious agents are more frequent. Thoracocentesis proves to be the first-line diagnostic tool. Light's criteria are the best for distinguishing whether the effusion is a transudate or an exudate. If the patient has an exudative pleural effusion, other tests are indicated to determine the etiology and in some cases the treatment: macroscopic appearance, cytology and differential white cell count (level of glucose, lactate dehydrogenase, adenosine deaminase, pH, bacterial cultures). Others investigat

There is a lot of literature about selecting a primary dressing for a wound. However, not much is made available on the selection of a secondary dressing, which is just as important, as it allows the primary dressing to remain in place. This article looks at selecting a secondary dressing that is appropriate for a patient, considering the patient's mobility or immobility, where the wound is located, how exudate can cause dressings to become detached, controlling a wound's odour, selecting a secondary dressing that will not damage a patient's skin and how nurses can keep themselves up-to-date w

In order to determine the exact significance and origin of lactate dehydrogenases (LDH) present in pleural effusions, LDH and their enzymes were assayed in the serum and pleural fluid of 60 patients with wet pleurisy and in the main blood cells (haematocytes, polymorphonuclears mononuclears) found in pleural fluids. Our results showed that: a pleural fluid/serum LDH ratio above 0.7 indicates that the fluid is an exudate; however, in about 10% of inflammatory pleural effusions due to infection or cancer this ratio is comprised between 0.5 and 0.7; the enzyme profile of transudates only differs

The detection of lymph nodes and a careful interpretation of dignity is one of the main indications for chest wall sonography. Palpable unclear masses of the chest wall can undergo US guided puctures very easily. Either rib and sternum fractures are detectable very sensitively. In addition to the more sensitive detection than in normal X-ray also accompanying soft parts processes, pleural effusions and haematomas can be visualised. Despite the physical laws of ultrasound, approximately 70% of the pleural surface can be accessed by sonography. The normal parietal pleural can be visualized and d

Plant root exudates serve pivotal roles in supporting plant development and interactions with the physicochemical and biological factors in the rhizosphere. Under stress conditions, root exudation is involved in enhancing plant resource-use efficiency and facilitating the crosstalk between plant and soil microbes to ameliorate stress. Although there are a large number of root exudates that remain to be characterized, recent technological advancements have allowed for the function of many exudate compounds to be elucidated. In this review, we discuss current knowledge about the key root exudate

BACKGROUND: Light's criteria remain the standard for distinguishing exudative from transudative pleural effusions, but require serum sampling and lack specificity. We assessed whether a pleural fluid-only approach could match the diagnostic accuracy.
METHODS: We analysed 7280 diagnostic thoracenteses from a single centre, divided into derivation (n=5000) and validation (n=2280) cohorts. We compared Light's criteria with a triple (protein >3 g·dL-1, lactate dehydrogenase (LDH) >250 IU·L-1 or cholesterol >55 mg·dL-1) and a double (LDH >250 IU·L-1 or cholesterol >55 mg·dL-1) combination using se

Pleural effusion affects 1.5 million patients in the United States each year. New effusions require expedited investigation because treatments range from common medical therapies to invasive surgical procedures. The leading causes of pleural effusion in adults are heart failure, infection, malignancy, and pulmonary embolism. The patient's history and physical examination should guide evaluation. Small bilateral effusions in patients with decompensated heart failure, cirrhosis, or kidney failure are likely transudative and do not require diagnostic thoracentesis. In contrast, pleural effusion i

Wound care dressing selection requires careful consideration of both exudate management capabilities and effectiveness in protecting adjacent skin. The ability to sustain a therapeutic, moisture-balanced wound environment that minimises pain and safeguards periwound integrity is essential for healing. A product evaluation was conducted to measure the effectiveness of the Sol-Care™ Dressings (Sol-Millennium Medical) in managing various wound presentations. Outcomes were evaluated based on: exudate management; protection of the periwound skin; and improvement in outcomes. The Sol-Care™ Dressings

AIMS: The aim of the study was to determine the performance of Xpert MTB/RIF assay in diagnosing tuberculosis on thoracoscopic pleural biopsies in exudative pleural effusion.
METHODS: Patients who underwent thoracoscopic pleural biopsy in the defined period were included in the study. Histopathology was done for all and Xpert MTB RIF assay and AFB culture of pleural biopsy specimen and pleural fluid were done as per the clinician's discretion.
RESULTS: Total of 110 patients underwent pleural biopsy and tissue Xpert MTB/RIF and MTB culture were done in 29 patients. XpertMTB/RIF assay and MTB

A sarcoidosis associated pleural effusion (SAPE) is a pleural effusion caused by active granulomatous inflammation from sarcoidosis. We describe the epidemiology, clinical features, diagnostic approach, treatment strategies and outcome of this condition. SAPE occurs in approximately 1% of sarcoidosis patients. The condition most commonly occurs at the initial presentation of sarcoidosis or within the first year. Dyspnea is the most common presenting symptom. Although a definitive diagnosis of SAPE requires a pleural biopsy, the diagnosis may be established on the basis of clinical features alo
