Risk factors for lower extremity vascular complications in adult patients on veno-arterial extracorporeal membrane oxygenation.

OBJECTIVE: Utilization of veno-arterial extracorporeal membrane oxygenation (VA-ECMO) has increased significantly over the last decade. Prior studies have reported worse mortality for patients with vascular complications on VA-ECMO; however, these were limited by small sample size. The purpose of this study is to investigate predictive risk factors for vascular complications in VA-ECMO patients and their potential impact on mortality. METHODS: Patients who underwent peripheral VA-ECMO from January 2011 to December 2021 were identified. Primary outcomes were lower extremity vascular complicati

Joint effects of visual acuity impairment and visual field loss on reading performance: a low-vision simulation in Chinese readers.

BACKGROUND: This study aimed to evaluate the individual and joint effects of reduced visual acuity (VA) and restricted visual field (VF) on the reading performance of Chinese readers under standardized low-vision simulation. METHODS: Eighty-one adults were randomized to three simulated distance VA levels using Bangerter foils: normal VA (≤ 0.30 logMAR), mild VA impairment (VAI) (0.30 logMAR< VA ≤ 0.52 logMAR), and moderate VAI (> 0.52 logMAR). VF was simulated as normal, moderate VF impairment (VFI) (20° radius), or severe VFI (10° radius) using specially designed goggles. Reading speed (RS)

Baseline factors predictive of visual prognosis in acute postoperative bacterial endophthalmitis in patients undergoing cataract surgery.

IMPORTANCE: Although rare, postoperative endophthalmitis in patients undergoing cataract surgery can lead to anatomical or functional loss of the eye. Therapeutic strategies such as antibiotic prophylaxis and microbiological diagnosis are more effective with a target patient population. New prospective data are needed to identify prognostic factors. OBJECTIVE: To identify baseline factors of visual prognosis in patients with acute bacterial endophthalmitis after cataract surgery. DESIGN: Prospective study of consecutive patients undergoing cataract surgery, enrolled from March 1, 2004, throu

Efficacy and Safety of Viscum album L. in Patients With Triple-Negative Breast Cancer Receiving Immune Checkpoint Inhibitors: A Protocol for an Open-Label Randomized Clinical Trial.

Although pembrolizumab has shown clinical benefit in triple-negative breast cancer (TNBC), effective long-term responses remain limited. Viscum album (mistletoe extracts, VA) may have immunomodulatory, pro-apoptotic, and immune-stimulating effects in preclinical and clinical studies. This trial was designed to evaluate the immunologic effects and safety of combining pembrolizumab with VA in the adjuvant setting. This open-label, randomized, clinical trial will include 40 adults (≥19 years) with histologically confirmed TNBC who have undergone surgery and started adjuvant pembrolizumab but rece

Comparison of Conventional Methods with Pump-Controlled Retrograde Trial off for Weaning Adults with Cardiogenic Shock from Veno-Arterial Extracorporeal Membrane Oxygenation.

BACKGROUND: Pump-controlled retrograde trial off (PCRTO) is a safe, simple, and reversible method for weaning patients from veno-arterial extracorporeal membrane oxygenation (VA-ECMO). However, few studies have compared PCRTO to conventional weaning methods. This retrospective study aimed to compare PCRTO to non-PCRTO methods. METHODS: This study included patients who were weaned from VA-ECMO from January 2016 to December 2022 at our medical center. Demographic data, ECMO management, ECMO complications, survival to discharge, and cardiogenic shock after VA-ECMO weaning were compared between t

Statin therapy reduces vertebral artery atherosclerotic plaques: a case report with insight from vessel wall magnetic resonance imaging.

BACKGROUND: Posterior circulation stroke accounts for a substantial proportion of ischemic cerebrovascular events, with vertebral artery (VA) atherosclerosis representing an important etiological factor. Although statin therapy is a well-established modality for stabilizing atherosclerotic plaques and reducing stroke risk, its direct effects on VA plaque regression have not been sufficiently explored, particularly through high-resolution vessel wall magnetic resonance imaging (VW-MRI). We present a case demonstrating VA atherosclerotic plaque regression on serial VW-MRI following statin therap

Ventriculoatrial Shunts in Adults, Incidence of Infection, and Significant Risk Factors: A Single-Center Experience.

BACKGROUND: In recent years, the number of ventriculoatrial (VA) shunt insertions has decreased worldwide, the major cause being the risk of shunt infection. VA shunts remain as an alternative option to ventriculoperitoneal shunts. We describe our 10-year experience with VA shunts by analyzing the incidence of shunt infections and predisposing cofactors. METHODS: During a median follow-up of 15.3 months, 259 shunt insertions, performed on 255 patients, were analyzed. The infection rate was calculated and the predisposing cofactors age, gender, cause of the hydrocephalus, previous external ven

Vitamin A deficiency exacerbates autism-like behaviors and abnormalities of the enteric nervous system in a valproic acid-induced rat model of autism.

The manifestations of autism spectrum disorder (ASD) are highly heterogeneous. As many individuals with ASD have gastrointestinal (GI) comorbidities, ASD with GI problems is considered to be a subtype of ASD. Vitamin A (VA) plays an important role in the development of both the central and peripheral nervous system. However, the relationship between VA deficiency (VAD) and ASD with GI comorbidities is still unclear. We established rat models with different VA levels based on the valproic acid-induced autism model. Compared to autism model rats with VA normal (VAN), autism model rats with gesta

PVN-centered multi-omics and single-cell analyses suggest shared inflammatory and immune features in acute myocardial infarction-associated ventricular arrhythmia and atrial fibrillation.

OBJECTIVE: Acute myocardial infarction (AMI) increases the risk of arrhythmias, particularly ventricular arrhythmias (VA), and atrial fibrillation (AF) is also common after AMI. The paraventricular nucleus (PVN) is a key regulator of sympathetic outflow. This study used a PVN-centered strategy to investigate molecular changes after AMI, prioritize candidate molecules associated with VA, and assess whether related inflammatory and immune features are also present in AF cohorts. METHODS: An AMI model was established in C57BL/6 mice, and PVN tissues were collected at 48 h and 7 d for transcripto

Baseline Predictors of Visual Acuity Outcome in Patients with Wet Age-Related Macular Degeneration.

Age-related macular degeneration (AMD) is one of the leading causes of severe vision loss in people over 60 years. Wet AMD (wAMD) causes more severe visual acuity (VA) loss compared with the dry form due to formation of choroidal neovascularization (CNV). Antivascular endothelial growth factor (anti-VEGF) agents such as ranibizumab and aflibercept are now the standard of care treatment for wAMD. Unfortunately, up to a quarter of anti-VEGF-treated wAMD patients might not fully benefit from intravitreal injections and CNV activity may not respond to the treatment and these patients are called an
