Imaging of necrotizing fasciitis.

Necrotizing Fasciitis (NF) is a rare, but highly lethal, rapidly progressive necrotic infection of the soft-tissue fascia. More common in immunocompromised patients, NF typically affects the extremities, perineal area, genital area (Fournier gangrene), and less commonly the torso. Although classically a clinical diagnosis, imaging is a powerful adjunct to facilitate early diagnosis in equivocal cases. The key features of NF of radiography, ultrasound, CT, and MRI are reviewed, as are the inherent limitations of NF diagnosis for each modality. With a high level of sensitivity, specificity, and

Not just eosinophilic fasciitis.

This case report describes a rare case of progressive muscle weakness in a patient treated for eosinophilic fasciitis (EF) for many years before being diagnosed with a second autoimmune disease: dermatomyositis. Our case is a report of a 65-year-old male diagnosed with eosinophilic fasciitis 7 years before being evaluated in our service at Mayo Clinic in Jacksonville, Florida, due to progressive muscle weakness despite the chronic treatment with methotrexate. Contrast-enhanced magnetic resonance imaging of the lower extremity showed enhancement throughout the thigh musculature, which led us to

Nodular fasciitis of the tongue.

BACKGROUND: Nodular fasciitis is a non-neoplastic proliferation within the subcutaneous tissue and the deep fascia of the fibroblasts, probably of a reactive nature characterized by apparent infiltration of the connective tissues by a mitotically active spindle cell lesion. Nodular fasciitis in the head/neck region is rarely found and only 2 previous cases affecting the tongue have been reported. METHODS AND RESULTS: The purpose of this study was for us to report a very rare case of a 67-year-old man with a history of a 3-month subepithelial asymptomatic nodule of the tongue tip with an ulcer

[Necrotizing fasciitis of the neck].

Necrotizing fasciitis is a rare and rapidly progressive infection characterized by necrosis of the superficial fascia and spread on the surrounding skin or muscles, which can be fatal. It usually occurs in the limbs, abdominal wall and perineum. In this retrospective review, the authors present 15 patients with cervical necrotizing fasciitis. The patient mean age was 54.7 years and they had one or more comorbid health problems. Five of them had descending necrotizing mediastinitis and three had progressive sepsis with toxic shock syndrome. Broad-spectrum intravenous antibiotic therapy was admi

Necrotizing fasciitis of the perineum.

BACKGROUND: Necrotizing fasciitis is a severe soft tissue infection characterized by rapidly progressing necrosis, involving subcutaneous tissues. This rare condition carries a high mortality rate and requires prompt diagnosis and urgent treatment with radical debridement and antibiotics; but early diagnosis, which is essential to successful treatment, remains a challenge. METHODS: Physical examination findings, preoperative and operative findings, histopathological results of the structure, and follow-up results of the patient are discussed with related reports. RESULTS: A 15-month old girl

[Cranial fasciitis of childhood].

OBJECTIVE: To describe the clinical and pathological features of cranial fasciitis of childhood, and to review and compare this case with those of international literature. METHODS: We report a case of cranial fasciitis of childhood located on the face of a 13-month-old boy. A complete review was performed from the database "Pub Med", looking for the key words "head and neck inflammatory myofibroblastic tumour", "nodular fasciitis", "cranial fasciitis of childhood". RESULTS: Cranial fasciitis is benign rapidly growing fibroblastic tumour, belonging to the group of nodular fasciitis, and has

[Necroticing fasciitis in children].

Necrotizing fasciitis (NF) is a serious condition with infection of the muscular fascia causing rapidly spreading necrosis. NF rarely affects children, and we here report two paediatric cases of NF. The first developed NF after a joint puncture, the other after surgical extirpation of a lymph node. Both were treated with antibiotics, human immunoglobulin, hyperbaric oxygen and repeated surgery and survived. In contrast to adults, children with NF often have no underlying chronic illness. The most frequent access point is varicella zoster ulcers.

Necrotizing Fasciitis of the Extremities.

Necrotizing fasciitis (NF) describes a life threatening soft tissue infection characterized by a rapid spreading infection of the subcutaneous tissue and in particular the fascia. Various synonyms for this type of infection are used, often due to the difficult diagnosis. Necrotizing fasciits of the extremities is found after simple skin lacerations and often in rural, farming or garden setting environments. Many of the infections are found in immunologically healthy people, but persons revealing a compromised wound healing are endangered additionally, e.g., diabetes. In the majority of the mic

Mortality in necrotizing fasciitis.

OBJECTIVE: The objective of this study was to determine the mortality rate in patients presenting with Necrotizing Fasciitis. This prospective study was conducted at ward 26, JPMC Karachi over a period of two years from March 2001 to Feb 2003. METHODS: All patients above the age of 12 years diagnosed to be having Necrotizing Fasciitis and admitted through the Accident and emergency department were included in this study. After resuscitation, the patients underwent the emergency exploration and aggressive surgical debridement. Post-operatively, the patients were managed in isolated section of

Fasciitis ossificans of the breast.

We report a rare case of fasciitis ossificans of the breast. A nodule of the breast was incidentally found in a 77-year-old woman without a history of trauma. The painless lesion was located 3 cm from the nipple in the lower outer quadrant of the left breast. Excisional biopsy was performed, and there has been no evidence of recurrence or metastasis for 2 years. The 1.8 cm diameter nodule was well demarcated from the adjacent tissue and located 2.5 cm beneath the skin. Histologically, the lesion was composed of uniform woven bone trabeculae with rimming of osteoblasts and fibrous stroma. At th

Dermatomyositis with tuberculous fasciitis.

A case of dermatomyositis with tuberculous fasciitis is described. A 69-year-old Japanese man treated with prednisolone for dermatomyositis developed erythema and bilateral swelling of arms and forearms. A diagnosis of tuberculous fasciitis was confirmed by the presence of acid-fast bacilli in a biopsy specimen and the growth of Mycobacterium tuberculosis from tissue cultures. The unusual presenting clinical features resulted in delayed diagnosis. We emphasize that tuberculous fasciitis should be considered in an immunocompromised patient who is treated with steroids, especially if the results

Fasciitis in amyopathic dermatomyositis.

Dermatomyositis is a disease frequently treated by rheumatologists and dermatologists due to prominent systemic features of inflammatory myositis, less common arthritis, and rare systemic vasculitis, in addition to the characteristic cutaneous manifestations of Gottron's papules over extensor surfaces, and a heliotrope rash over the eyelids. Patients with amyopathic dermatomyositis, a subset of dermatomyositis, display skin disease but no apparent muscle disease. This report describes an adult patient with the typical dermatomyositis rash with no weakness, normal muscle enzymes, and the unique

Necrotising fasciitis of the neck.

Necrotising fasciitis is a fulminant soft tissue infection that causes necrosis of fascia and subcutaneous tissue while sparing skin and muscle initially. It is most commonly seen in adults involving the perineum, extremities and and minal wall. Immunncompromised patients are at an increased risk of developing this infection. These infections require early diagnosis, aggressive surgical debridaient and appropriate antibiotic therapy. Mortality rates have been reported to be as high as 52 and 73% in general surgery literature.(Freuschtag et al, 1985., Rouse et at 1982).Necrotising fasciitis of
