Spontaneous stress fractures of the femoral neck.

The diagnosis of spontaneous stress fractures of the femoral neck, a form of insufficiency stress fracture, can be missed easily. Patients present with unremitting hip pain without a history of significant trauma or unusual increase in daily activity. The initial radiographic features include osteoporosis, minor alterations of trabecular alignment, minimal extracortical or endosteal reaction, and lucent fracture lines. Initial scintigraphic examinations performed in three of four patients showed focal increased radionuclide uptake in two and no focal abnormality in one. Emphasis is placed on t

Ten years' experience with femoral shaft fractures.

One hundred one patients with femoral shaft fractures treated by open reduction and intramedullary fixation have been presented. The results demonstrated no nonunions in nonpathologic bone; less than 2% infection; clinical union in 4 to 6 months; radiologic union at 6 to 8 months; excellent range of knee motion in more than 90% of patients; unsupported full weight-bearing usually present by the eighth postoperative week, and an average hospital stay of 6 to 7 weeks. The advantages and disadvantages of the most common methods of treatment of femoral shaft fractures have been reviewed. The non-o

Femoral neck shaft angle in relation to the location of femoral stress fracture in young military recruits: femoral head versus femoral neck stress fracture.

OBJECTIVE: To evaluate the influences of the femoral neck shaft angle (FNSA) on the location of the femoral stress fracture and to assess the potential differences in FNSA between fractured and normal femurs. MATERIALS AND METHODS: Thirty-seven patients with femoral stress fractures who underwent both plain hip radiographs and MRI, from January 2016 to September 2019, were retrospectively included. Patients were classified as having either femoral head stress fracture (group A, n = 26) or femoral neck stress fracture (group B, n = 11). The FNSA was measured in anteroposterior (AP) hip radiogr

Femoral head fractures: anatomy, diagnosis and management.

Femoral head fractures are complex and severe injuries, usually associated with hip dislocation. They typically result from high-energy trauma. Therefore, a low index of suspicion is required for diagnosis in these contexts. Initial presentation can vary depending on coexisting injuries but is typically an emergency and requires immediate reduction of the joint. Delays result in worse outcomes for patients. Pelvic radiographs are recommended before and after joint reduction, with Judet, inlet and outlet views to identify any associated acetabular fracture and pelvic ring injury. Computed tomog

[Surgical treatment strategies for interprosthetic femoral fractures].

Due to demographic changes, a further increase in the number of arthroplasty procedures can be anticipated. As has been observed in recent years, a further increase in periprosthetic fractures is to be expected. An interprosthetic femoral fracture is a serious complication in cases of ipsilateral hip and knee endoprostheses. Although the incidence of interprosthetic fractures is low, fracture management can be highly demanding. In addition to the remaining implant stability, bone quality and comorbidities must also be taken into account in the decision-making process of fracture treatment. The

[Bilateral femoral fractures due to bisphosphonate use].

The risk of an atypical femoral fracture increases with the use of bisphosphonates for longer than 5 years. The typical presentation of an atypical femoral fracture consists of a transverse subtrochanteric femoral fracture originating from the lateral cortex with a medial spike, occurring after minimal or no trauma. For patients presenting with a spontaneous femoral fracture who are using bisphosphonates, it is recommended to discontinue bisphosphonate use and to determine the future fracture risk. Imaging of the contralateral femur should always be performed in patients with an AFF. If radiol

Double fixation for complex distal femoral fractures.

For complex distal femoral fractures, a single lateral locking compression plate or retrograde intramedullary nail may not achieve a stable environment for fracture healing. Various types of double fixation constructs have been featured in the current literature. Double-plate construct and nail-and-plate construct are two common double fixation constructs for distal femoral fractures. Double fixation constructs have been featured in studies on comminuted distal femoral fractures, distal femoral fracture with medial bone defects, periprosthetic fractures, and distal femoral non-union. A number

Femoral Neck Stress Fractures: An Updated Review.

Femoral neck stress fractures represent a relatively rare spectrum of injuries that most commonly affect military recruits and endurance athletes. If unrecognized and if proper treatment is not initiated, this condition carries potentially devastating consequences. Patients will typically present with an insidious onset, atraumatic hip, and groin pain that is relieved with rest. The condition may be initially misdiagnosed because radiographs are often normal. Magnetic resonance imaging has demonstrated superior specificity, sensitivity, and accuracy compared with other diagnostic modalities in

Management of low periprosthetic distal femoral fractures.

AIMS: Debate continues regarding the optimum management of periprosthetic distal femoral fractures (PDFFs). This study aims to determine which operative treatment is associated with the lowest perioperative morbidity and mortality when treating low (Su type II and III) PDFFs comparing lateral locking plate fixation (LLP-ORIF) or distal femoral arthroplasty (DFA). METHODS: This was a retrospective cohort study of 60 consecutive unilateral (PDFFs) of Su types II (40/60) and III (20/60) in patients aged ≥ 60 years: 33 underwent LLP-ORIF (mean age 81.3 years (SD 10.5), BMI 26.7 (SD 5.5); 29/33 fe

Management of two-level proximal femoral fractures.

We present the case of an 82-year-old female, who experienced a ground-level fall on the trochanter of the right femur. X-rays showed a proximal femoral fracture (PFF) with an unclear and unusual fracture pattern. Three-dimensional CT images were obtained and showed a displaced femoral neck fracture and ipsilateral fracture of the greater trochanter. Our patient underwent unipolar hemiarthroplasty and fixation of the greater trochanter with a hook plate and cable grip. At 11 months, functional outcomes, patient satisfaction and quality of life were excellent. Primary osteoporosis was diagnosed

Outcomes of operatively treated interprosthetic femoral fractures.

AIMS: The prevalence of ipsilateral total hip arthroplasty (THA) and total knee arthroplasty (TKA) is rising in concert with life expectancy, putting more patients at risk for interprosthetic femur fractures (IPFFs). Our study aimed to assess treatment methodologies, implant survivorship, and IPFF clinical outcomes. METHODS: A total of 76 patients treated for an IPFF from February 1985 to April 2018 were reviewed. Prior to fracture, at the hip/knee sites respectively, 46 femora had primary/primary, 21 had revision/primary, three had primary/revision, and six had revision/revision components.

Revisiting Pauwels' classification of femoral neck fractures.

Pauwels' femoral neck fracture classification is based on the biomechanical principle that shear stress and varus force increase along more vertically oriented fractures, resulting in higher risk of fracture displacement and ultimately nonunion. This principle continues to guide construct selection for femoral neck fracture internal fixation and is the foundation for treating non-union with valgus osteotomy. However, with poor inter- and intra-rater reliability, dated treatment recommendations, and unreliable prognostic value, the Pauwels classification cannot be directly applied in its entire

Mechanical analysis of femoral stress-riser fractures.

BACKGROUND: A stress-riser fracture develops when stress in an object is higher than that in the surrounding material. The aim of this retrospective study was to evaluate all consecutive femoral stress-riser fractures during a period of >10 years and to verify high-risk factors by using mechanical analysis. METHODS: Forty clinical fractures caused by local stress concentration in the femoral cortical bone were included. Risk factors were analyzed, and Sawbones models were used to verify their effect. One hundred thirty-six models were divided into 6 groups to simulate clinical cases of stress

Femoral Neck Fractures in Children: A Review.

Paediatric femoral neck fractures are uncommon injuries and are usually caused by high-energy trauma. Low-energy trauma can result in pathologic neck fractures and stress fractures of the neck, due to repetitive activity. Surgical options can vary based on age, Delbet classification and displacement of the fracture. Treatment for displaced fractures is by closed or open reduction and smooth/cancellous screw fixation. Compression screw and side plate fixation is indicated for basal fractures. Fixation should be supplemented by spica cast immobilization in younger children. The high rate of comp

Identifying atypical femoral fractures--a retrospective review.

Subtrochanteric atypical femoral fractures (AFFs) have been reported in patients on osteoporosis therapy (bisphosphonates and denosumab). In 2010, and again in 2013, the ASBMR AFF Task Force developed strict diagnostic criteria for AFFs. This is the first study using these criteria to define the prevalence of AFFs in Canada. This study is a retrospective review of all adult patients (April 2002-March 2013) with an ICD 10 code for hip, femoral or subtrochanteric fracture, from two referral hospitals in Alberta, Canada. All identified as isolated subtrochanteric fractures were further evaluated

Periprosthetic femoral fractures in total hip arthroplasty.

Periprosthetic fractures in total hip arthroplasty (THA) are a significant problem facing hip surgeons, and were responsible for revision surgery in 9% of single stage revision THAs recorded in the National Joint Registry of England and Wales (NJR) in 2012; the 5th most common cause after aseptic loosening, osteolysis, pain and dislocation. The incidence has increased along with the number of THAs performed. Implants and techniques of THA continue to evolve, surgical indications are expanding and the number performed annually continues to rise. Furthermore, patients are undergoing THA earlier
