Management of young femoral neck fractures: is there a consensus?

BACKGROUND: Femoral neck fractures in young adults (ages <60) are high-energy injuries that are associated with major fracture healing complications such as avascular necrosis, nonunion, and significant shortening. Historically, evidence from small trials has suggested multiple cannulated screws were the optimal implant; however, newer studies and implant designs warrant reevaluation of screws as the gold standard among surgeons. In addition, controversies surrounding reduction technique and urgency of surgical fixation have been previously identified. We aimed to survey surgeon treatment pref…

[Bilateral femoral neck fracture after seizure].

Femoral neck fractures after seizure are rare. This injury can easily be underdiagnosed due to generalised, musculoskeletal pain after seizure. In case of persisting groin pain and limited range of motion X-rays are indicated. Within the first 6 hours after the trauma a joint-preserving therapy is possible. After a delayed diagnosis total hip arthroplasty is necessary. As the result of prolonged intra-articular haematoma the incidence of a femoral head necrosis increases. When choosing the implant, an elevated risk of joint dislocation should be considered.

Fractures of the femoral neck: a review and personal statement.

EPIDEMIOLOGY: The number of hip fractures will increase enormously in the decades to come as will the cost of treatment of these patients do … ecrease the risk of suffering a hip fracture with 50%. CLASSIFICATION: The first classification of femoral neck fractures, proposed by Abraham Colles, in displaced and non-displaced (impacted) fractures appears to be still the most useful one. The Pauwels classification cannot be applied to the preoperative x-ray, because the fractured leg is always in external rotatio …

Femoral neck stress fracture in a sanitary worker.

Stress fractures of the femoral neck are uncommon, it is important to have a high index of suspicion in young active people who perform repetitive vigorous activity or have recently had an increase in frequency of such activity. We report a case of femoral neck stress fracture in a sanitary worker which was treated successfully with a dynamic hip screw.

[Medial femoral neck fracture. Controversies in treatment].

The treatment of the medial femoral neck fracture remains controversial until today. The goal of this paper is therefore, based on the literature, to show guidelines for optimal treatment: conservative treatment vs. operation, osteosynthesis vs. prosthesis, timing for osteosynthesis, open vs. closed reduction, choice of implant for osteosynthesis, postoperative treatment (weight bearing vs. non weight bearing), Pauwels-Osteotomy vs. prosthesis in cases op pseudarthrosis, femoral head prosthesis vs. total hip arthroplasty, bipolar vs. monopolar femoral head prosthesis, choice of classification.

[Management of ipsilateral femoral shaft and neck fracture].

From Oct. 1987 to Oct. 1993, 704 patients with femoral shaft fracture were managed at our hospital. Among these, 12 patients were with combined femoral shaft and neck fracture (2%). The age ranged from 17 years to 58 years (average 38.2 years). Eleven patients were victims of traffic accidents and another 1 fell from a height. Most of them had associated injuries, including fractures elsewhere, head injury, chest and abdominal trauma. Five of them were treated with open reduction and internal fixation with Huckstep intramedullary nail, 2 with Kuntscher nail and Knowles' pin, 2 with plate and c

Osteoclast cytomorphometry in patients with femoral neck fracture.

In patients with femoral neck fracture, nutritional deficiencies have been shown to be common. A low calcium diet and/or a reduced vitamin D intake have been suspected to cause secondary hyperparathyroidism responsible for increased bone turn over and bone loss. Parathyroid hormone (PTH) levels are increased in these patients, data which are in accordance with the pronounced changes observed on bone biopsies reflecting a true hyperparathyroidism. We have used a cytomorphometrical approach to characterize PTH-induced changes on the osteoclastic population. Osteoclasts were detected histochemica

[Elastic osteosynthesis in intracapsular fracture of the femoral neck].

The authors present a method of treatment for transcervical fractures of the femoral neck using implants with mechanical characteristics--curved shape, smooth surfaces and elastic resistance to deformity--which respect the biomechanical properties of the normal bone. These implants maintain a close contact between the bony fragments during convalescence and preserve the blood supply of the bone. In this way, bone healing is encouraged and the development of ischaemic necrosis of the femoral head is prevented. Between 1983 and 1985, 126 fractures, of the femoral neck, 120 recent and 6 old, were

Fracture of the femoral neck after internal fixation.

A case of femoral neck fracture despite the presence of a previously inserted sliding screw and sideplate is reported. Internal fixation in itself did not prevent subsequent fracture. A critical factor in the mechanism of this fracture was the patient's underlying disease, felt to be alcoholic osteomalacia.

Relevance of osteoporosis in women with fracture of the femoral neck.

A prospective study of fractures of the femoral neck was conducted over 12 months in order to ascertain the relevance of generalised osteoporosis as determined by metacarpal morphometry. A series of some 200 women sustaining a fracture of the femoral neck after minor trauma had bone mass measurements similar to those of a control population of normal women, and 16% were not osteoporotic. A history of previous fractures was documented in one third of the women, but this was unrelated to the presence or severity of osteoporosis, although over half of the fractures had occurred within the previou

Early outcomes of the synthes femoral neck system for valgus-impacted femoral neck fractures: a multicenter retrospective study.

INTRODUCTION: Nondisplaced and valgus-impacted femoral neck fractures have traditionally been treated with screw fixation, but reoperation rates remain high. This study reports clinical outcomes following internal fixation of low-energy femoral neck fractures (AO/OTA 31B1.1 and 31B1.2) using a new, fixed-angle sliding device. METHODS: This was a multi-institutional retrospective review at four academic Level I Trauma Centers, one Level II Trauma Center, and one Level III Trauma Center. Patients with AO/OTA 31B1.1 and 31B1.2 fractures who underwent internal fixation using the Femoral Neck Syst…

Biomechanical Stability of Femoral Neck System for Pauwels Type III Femoral Neck Fractures Based on Different Reduction Quality.

To further investigate the biomechanics of a femoral neck system (FNS) for Pauwels type III femoral fractures based on three different reductions. We constructed three different reduction (anatomical reduction, negative buttress reduction, and positive buttress reduction) models of Pauwels type III femoral neck fractures. Then, three cannulated screws (3CS), dynamic hip screws (DHS), dynamic hip screws combined with an anti-rotation screw (DHS + ARS), one-hole femoral neck system (1HFNS), and two-hole femoral neck system (2HFNS) were assembled with the reduction models, respectively, to simulat…

Nonanatomical reduction of femoral neck fractures in young patients treated with femoral neck system: a retrospective cohort study.

PURPOSE: Negative buttress reduction should be avoided in the treatment of femoral neck fractures (FNFs) using conventional fixation. As the femoral neck system (FNS) has been recently developed and utilized widely to treat FNFs, the association of reduction quality with postoperative complications and clinical function has not been clarified. The purpose of this study was to evaluate the clinical effect of nonanatomical reduction in young patients with FNFs treated with FNS. METHODS: This multicenter, retrospective cohort study included 58 patients with FNFs treated with FNS between Septembe…

Biomechanical study of femoral neck system for young patients with nonanatomically reduced femoral neck fractures: a finite element.

BACKGROUND: A consensus regarding the optimal approach for treating femoral neck fractures is lacking. We aimed to investigate the biomechanical outcomes of Femoral Neck System (FNS) internal fixation components in the treatment of nonanatomically reduced femoral neck fractures. METHOD: We constructed two types of femoral neck fractures of the Pauwels classification with angles of 30° and 50°, and three models of anatomic reduction, positive buttress reduction and negative buttress reduction were constructed. Subgroups of 1 to 4 mm were divided according to the distance of displacement in the…

Avascular necrosis of the femoral head after osteosynthesis of femoral neck fracture.

The reported incidence of avascular necrosis after femoral neck fracture fixation varies widely, and there is no consensus regarding its risk factors. We evaluated the incidence of avascular necrosis of the femoral head with the use of contemporary techniques for femoral neck fracture fixation. We then sought to determine what potential risk factors influenced the development of avascular necrosis.Between 1990 and 2005, one hundred sixty-three intracapsular femoral neck fractures in 163 patients were treated with internal fixation at our level-I trauma center. All patients were monitored until…
