Absence of metabolic bone disease in the proximal femur in patients with fracture of the femoral neck.

A clinical, radiological and histopathological study of femoral heads from 125 patients with fracture of the neck of the femur and from 30 cadavers was carried out to identify various risk factors. The findings showed that the Singh index was unreliable as a radiological indicator of the bone content of the femoral heads; that the bone content of the femoral head in patients sustaining a fracture of the femoral neck did not differ from that of the controls; that osteomalacia was not found in any of the heads examined; and that the distribution of trabecular microfractures did not support the h

A Rare Case of Masquerading Chondrosarcoma with Pathological Fracture of Neck of Femur - A Case Report.

INTRODUCTION: Chondrosarcoma is the second most common primary malignant tumor accounting for 20-27% of these. These are malignant cartilaginous tumor found most commonly in the pelvis, followed by the proximal femur. Perivascular epitheloid cell tumor (PEComa) is a rare mesenchymal neoplasm. These tumors all share a distinctive cell type, the perivascular epithelioid cell (which has no known normal tissue counter - part). These are post commonly found in visceral sites such as gastrointestinal and uterus, in retroperitoneum and abdominal pelvic sites, rarely affecting skeletal system. An Investigation Into the Risk Factors for Transfusion Need After Neck of Femur Fracture Surgery.

BACKGROUND: A common surgical procedure in the elderly is the operation on neck of femur fractures, with a primary complication being the need for a postoperative blood transfusion. Consequently, current standard practice involves cross-matching two units of red blood cells for each patient preoperatively. This incurs significant costs and is associated with blood product complications for transfused patients who are at low risk. This study aims to identify factors that could increase the risk of requiring a blood transfusion, thereby facilitating the adaptation of preoperative cross-matching

Biomechanical changes in the proximal femur before and after removal of femoral neck system.

BACKGROUND: As an innovative internal fixation system, FNS (femoral neck system) is increasingly being utilized by surgeons for the treatment of femoral neck fractures. At present, there have been numerous finite element analysis experiments studying the immediate stability of FNS and CSS in treating femoral neck fractures. However, there is scarce mechanical analysis available regarding the effects post internal fixation removal. This study aimed to investigate the alterations in mechanical parameters of the proximal femur before and after the removal of FNS (femoral neck system), and to asse

Outcome of surgery in neck of femur fracture patients with poor pre-fracture mobility.

BACKGROUND: Hip fractures are common with a UK incidence of over 70,000 cases and total healthcare costs of over £2 billion per year. Mortality rates of 10% at 30 days and up to 30% at 1-year have been reported. We wanted to assess the outcome of hip fracture surgery in patients with reduced pre-fracture mobility as this has not been exclusively studied previously. METHODS: We retrospectively reviewed 168 hip fracture patients with reduced pre-fracture mobility (wheelchair bound, bed bound, walking with 2 aids or a frame) who underwent hip fracture surgery at our institution between 2008 and

Outcomes of Combined Neck and Trochanter Fractures of the Femur Treated with Cephallomedullary Nail in Elderly.

PURPOSE: Proximal femur fractures are classified into intracapsular neck fractures and extracapsular trochanteric fractures, and several related treatment recommendations in elderly patients have already been introduced. Importantly, we have observed cases of combined intra and extracapsular fractures (i.e., ipsilateral neck and trochanter fractures). The purpose of this study is to report the outcomes of combined neck and trochanter fractures of the femur treated with cephalomedullary nail (CMN) in elderly patients. MATERIALS AND METHODS: From January 2010 to December 2014, 410 patients with

Pathological Neck of Femur Fracture with Failed Osteosynthesis in Adolescent: A Report of Two Cases.

INTRODUCTION: Non-union of the neck of femur fractures in adolescents is a complex problem, especially if there is an underlying pathology such as fibrous dysplasia or unicameral bone cyst. Treatment is challenging and needs careful selection of a combination of strategies. CASE REPORT: We present two such cases with a history of failed osteosynthesis with a sliding hip screw. Our strategy consisted of implant removal, open freshening and cancellous bone grafting of the fracture site, fibular strut grafting, subtrochanteric valgus osteotomy, and fixation with contoured 4.5 dynamic compression

Bilateral Neck of Femur Fractures in a Bilateral Below-Knee Amputee: A Unique Case.

According to the National Hip Fracture Database, over 64,000 patients were admitted with a hip fracture across England, Wales, and Northern Ireland in 2013, but very few are bilateral, and there are no current cases in the literature of bilateral neck of femur fractures in a patient with bilateral below-knee amputations. We present a case of a 69-year-old bilateral below-knee amputee male admitted to the emergency department with bilateral hip pain and radiological evidence of bilateral displaced neck of femur fractures. The patient subsequently underwent synchronous bilateral total hip replac

Technique for gentle accurate reproducible closed reduction of intracapsular fracture of neck of femur.

Non-anatomic reduction with either inferior offset or varus angulation was the strongest predictor of re-displacement of the fracture and poor outcome after fixation of intracapsular fracture of neck of femur. There are many techniques for closed reduction and can be broadly divided into non-invasive and invasive techniques. We describe a minimally invasive technique, which we believe is easy, reproducible and successful in achieving good anatomical reduction.

Subtrochanteric femur fracture after removal of screws for femoral neck fracture in a child.

Displaced femoral neck fractures are rare in children and are associated with a high rate of complications. Subtrochanteric fractures after cannulated screw fixation of femoral neck fractures in adults are well recognized, and there are several reports on the topic. However, there are no reports on complications related to hardware or subtrochanteric fractures after removal of the screws in the treatment of femoral neck fractures in children. Here we report the case of a 10-year-old boy who sustained a subtrochanteric fracture after the screw removal and healing that followed a femoral neck fr

Diffusion MRI of the neck of the femur in Legg-Calve-Perthes disease: a preliminary study.

PURPOSE: To evaluate diffusion MR imaging of the neck of the femur in Legg-Calve-Perthes disease (LCPD). MATERIAL AND METHODS: This is a prospective study in 27 children followed for unilateral LCPD. Forty-six MRIs were carried out with bilateral quantification of the apparent diffusion coefficient (ADC) of the neck of the femur. The intra- and inter-observer variability of the ADC measurements was evaluated. The association between the ADC and age as well as the healthy or pathological status of the neck of the femur and the Catterall classification were looked for. RESULTS: Intra- and inte

Septic hip after a fractured neck of femur in a rheumatoid patient: A case report.

A 63-year-old rheumatoid patient presented with severe pain in her left hip without a preceding history of trauma. An initial clinical and radiological examination revealed no "obvious" abnormality and the patient was symptomatically treated. The patient presented again five weeks later as weightbearing became increasingly difficult due to worsening pain. Inflammatory markers were found to be elevated and a repeat radiological examination revealed a displaced ununited fracture of the neck of femur. Isotope imaging suggested increased activity around the hip. A formal arthrotomy revealed pus in

Complications of fracture of the neck of the femur in children. A long-term follow-up study.

This is a long-term study of 53 children with fracture of the proximal femur. The age of these children at the time of injury ranged from 3 to 16 years (average, 10.2 years). The follow-up period averaged 9.4 years (range, 5-20 years). There was a single transepiphyseal fracture, 28 transcervical fractures, 21 basicervical fractures and three intertrochanteric fractures. The overall results were good in 49, fair in 25 and poor in 26% of patients. Better results were obtained in undisplaced fractures and in those displaced fractures where anatomical reduction was achieved and maintained through

[Posterior dislocation of the hip associated with fracture of the neck of the femur. Apropos of 3 cases].

Three patients with posterior dislocation of hip associated with fracture of neck of femur were treated by early sanguineous reduction and osteosynthesis. This therapy was very effective since necrosis did not develop either in the short or long-term follow up (1, 3 and 4 years). These findings combined with documented data are in favor of anatomic reconstruction of upper end of femur by stable and solid osteosynthesis, at least in young adults. Early operation, an irreproachable operating tactic (orthopedic table, ventral decubitus, posterior approach), an immediate stable and solid synthesis
