Historical roots of hospital-centred operational governance in China: a path dependence analysis and its implications for global health system reform.

BACKGROUND: Hospital-centred operational governance has been a persistent feature of China's health system. While this model has supported administrative control, rapid service expansion and organisational standardisation, it has also contributed to the continued concentration of resources in hospitals, constrained local flexibility and the relative marginalisation of primary healthcare. This study examines, from the perspective of path dependence, how this governance model gradually emerged and persisted from 1835 to the present. METHODS: Drawing on historical studies, policy documents, inst

Health financing, digital connectivity, and universal health coverage: a country-year panel study.

INTRODUCTION: Universal health coverage (UHC) requires broad service coverage and financial protection. Digital connectivity may enable digital public health, but general internet and telecommunications access does not capture the adoption, integration, quality, or equitable use of digital health services. We examined how health financing, connectivity, governance, and online information environments relate to UHC outcomes. METHODS: We assembled a multi-source country-year panel spanning 2000-2024. The principal two-way fixed-effects model included 3,522 observations from 166 countries in 200

How strong is the administrative backbone of ethical governance? A descriptive cross-sectional study of health research ethics committees in Tanzania.

BACKGROUND: Health research ethics committees (HRECs) provide the administrative backbone of ethical governance by safeguarding the rights, safety and well-being of research participants and ensuring the ethical conduct of health research. Effective ethical oversight depends not only on rigorous protocol review but also on robust governance, competent personnel, adequate infrastructure, sustainable institutional support and efficient administrative systems. Despite their critical role, evidence on the administrative functionality of HRECs in Tanzania remains limited. OBJECTIVE: To assess the

Social Participation for Health and Health Care Workers' Self-Management: Lessons from Socialist Yugoslavia for Contemporary Health Systems-A Systematic Review.

BACKGROUND/OBJECTIVES: Contemporary health systems continue to face persistent challenges in financing, equity, and governance, particularly under conditions of limited resources, workforce shortages, and demographic pressures. While the World Health Organization calls for strengthening social participation to achieve universal health coverage, evidence suggests that such participation often remains largely symbolic. Existing global examples of social participation in health are limited either by their short duration of implementation or by their limited scope. In contrast, the experience of f

An analysis of equity in Brazilian health system financing.

Health care in Brazil is financed from many sources--taxes on income, real property, sales of goods and services, and financial transactions; private insurance purchased by households and firms; and out-of-pocket payments by households. Data on household budgets and tax revenues allow the burden of each source except firms' insurance purchases for their employees to be allocated across deciles of adjusted per capita household income, indicating the progressivity or regressivity of each kind of payment. Overall, financing is approximately neutral, with progressive public finance offsetting regr

Bridging healthcare gaps through public-private partnerships in Nigeria: a scoping review.

Nigeria's healthcare system faces several persistent structural challenges. Public-private partnerships (PPPs) have emerged as a strategy to address these gaps, although evidence on their implementation and impact remains fragmented. This scoping review mapped the implementation models, outcomes, challenges and future directions of PPPs in Nigeria's healthcare system, following the Arksey and O'Malley framework and PRISMA-ScR guidelines. Literature searches were conducted across PubMed, Scopus, Web of Science, Google Scholar and grey literature sources. Studies were screened using Rayyan, whil

Should governments subsidize the use of insecticide-impregnated mosquito nets in Africa? Implications of a cost-effectiveness analysis.

Recent large-scale field trials show that mosquito nets impregnated with insecticide can substantially reduce all-cause mortality in children in malaria-endemic areas in Africa. This paper considers the cost-effectiveness of impregnated nets, initially from the perspective of a government programme which would distribute nets free of charge and organize and fund re-impregnation on an annual basis. The calculations show that with the reductions in all-cause mortality observed in the trials, complete government subsidy of nets through a vertical programme would represent an efficient use of scar

[Malaria in Tadjikistan: from control to elimination].

In the past three years, a notable progress has been made in the control of malaria, as suggested by a drastic reduction in malaria morbidity and in the number of active foci. To fulfill the set tasks, there is a need for continuous financing of antimalaria measures by the country's Government, the Global Fund, WHO, and other sponsors. In the Republic of Tadjikistan, there are prerequisites for achieving the elimination of tropical malaria by 2010; however, it is necessary to collaborate with the Islamic State of Afghanistan for prophylactic measures to be implemented to prevent importation of

Strategic analysis of financing communicable diseases in Afghanistan.

BACKGROUND: Afghanistan as a low-income country suffers from the heavy burden of Communicable Diseases (CDs) and their significant economic consequences. Therefore, this research is aimed to strategically analyze the financing system of CDs management in Afghanistan and provide effective solutions. METHODS: We conducted the qualitative research using interpretative phenomenological analysis (IPA). An interview guide was used to conduct the semi- structured interviews with 49 experts from the Afghanistan health system. We used the framework analysis method to analyze the results. RESULTS: In

Chilaiditi's sign or Chilaiditi's syndrome in the emergency department.

This paper sets out a theoretical framework for analyzing board governance, and describes an empirical study of corporate governance practices in a subset of non-profit organizations (hospices in the UK). It examines how practices in hospice governance compare with what is known about effective board working. We found that key strengths of hospice boards included a strong focus on the mission and the finances of the organizations, and common weaknesses included a lack of involvement in strategic matters and a lack of confidence, and some nervousness about challenging the organization on the qu

Mapping policy gaps and governance challenges in Kenya's hand hygiene environment: a health policy triangle analysis.

Hand hygiene is an effective public health measure for preventing infectious diseases. Sustained hand hygiene practice requires an enabling environment that constitutes access to hand hygiene products and services, clear and effective policy provisions, financing, monitoring, and coordination mechanisms. This paper analyses Kenya's national hand hygiene policy landscape and identifies the underlying governance barriers and systemic opportunities for strengthening hand hygiene policy and practice in Kenya. Guided by the Health Policy Triangle (HPT) framework, this study systematically examined

Governance and implementation of solar-powered digital health systems in low- and middle-income countries: a mixed-methods study from Kenya and Ethiopia.

BACKGROUND: Digital health systems are increasingly recognised as critical components of health system strengthening in low- and middle-income countries (LMICs). However, their effectiveness is often constrained by unreliable energy infrastructure, weak governance, and fragmented implementation approaches. Solar-powered digital health systems offer a promising solution, but their scalability depends on integrated governance and sustainable implementation models. OBJECTIVE: This study aimed to develop a governance and implementation framework for scaling solar-powered digital health systems in

From emergency adaptation to institutional resilience: a policy and practice review of Somalia's health system, 1991-2026.

Health-system resilience is a major policy concern in fragile and conflict-affected settings, where recurrent shocks intersect with chronic weaknesses in governance, financing, workforce, service delivery, information systems, and supply chains. This policy and practice review synthesizes 51 peer-reviewed and institutional sources published or issued between 1990 and 3 July 2026 to examine how Somalia's health system has responded to three decades of conflict, climatic emergencies, displacement, outbreaks, and humanitarian intervention. Sources were identified through targeted searches of PubM

Consensus development on eligibility of government subsidisation of biologic disease modifying anti-rheumatic agents for treatment of ankylosing spondylitis: The Singapore experience.

INTRODUCTION: The beneficial effects of biologic disease-modifying anti-rheumatic drugs (bDMARDs), such as tumour necrosis factor inhibitors (anti-TNF) in active ankylosing spondylitis (AS) are well established. The significant costs on patients in the absence of financial subsidization can limit their use. The objective was to describe a consensus development process on recommendations for government-assisted funding of biologic therapy for AS patients in Singapore. METHODS: Evidence synthesis followed by a modified RAND/UCLA Appropriateness Method (RAM) was used. Eleven rheumatologists rate
