Cutaneous disseminated histoplasmosis in AIDS patients in south Florida.

BACKGROUND: Histoplasma capsulatum is a dimorphic pathogenic fungus endemic to the Mississippi and Ohio river valleys. In the immunocompetent it causes a self-limited disease, but in the immunocompromised may lead to disseminated disease (disseminated histoplasmosis (DH)). It is one of the opportunistic infections which defines the acquired immunodeficiency syndrome (AIDS) and is rarely encountered outside endemic regions. METHODS: Clinical, laboratory, and histologic information concerning seven patients with DH and AIDS in South Florida was recorded. RESULTS: We report seven cases of DH wi…

Mercury distribution in American alligators (Alligator mississippiensis) in Florida.

Thirty American alligators (Alligator mississippiensis), including 24 wild-caught and six control captive farm-raised alligators, were analyzed for whole body mercury contamination. Wild-caught animals were collected from Water Conservation Area 3 in the Everglades ecosystem (n = 12) and from Alachua, Brevard, and Collier counties outside the Everglades (n = 12). Using cold-vapor atomic absorption spectrophotometry, samples of brain, cervical spinal cord, liver, paired kidneys, paired testes, paired ovaries, paired oviducts, heart, lungs, spleen, bile, tail and leg muscle, and tail and leg sca

Reproductive toxins and alligator abnormalities at Lake Apopka, Florida.

The alligator population at Lake Apopka in central Florida declined dramatically between 1980 and 1987. Endocrine-disrupting chemicals and specifically DDT metabolites have been implicated in the alligators' reproductive failure. The DDT metabolite hypothesis is based largely on the observation of elevated concentrations of p,p-DDE and p,p-DDD in alligator eggs obtained from Lake Apopka in 1984 and 1985. In the following commentary, we draw attention to two nematocides that are established reproductive toxins in humans, dibromochloropropane (DBCP) and ethylene dibromide (EDB), which could also…

Recognition and management of teething diarrhea among Florida pediatricians.

According to current medical opinion, teething diarrhea (TD) is a myth; yet cross-cultural data document a worldwide distribution of popular belief in the association of frequent, loose stools with tooth eruption. A mail survey in 1990 of 215 pediatricians practicing in Florida investigated beliefs and practices related to TD. Thirty-five percent of respondents believed there is a real association between diarrhea and tooth eruption. When compared with pediatricians who do not believe in TD, these respondents were more likely to be more recent graduates of medical school, to be in general pedi…

Crack cocaine use and sexual activity in Miami, Florida.

Data are analyzed from the Multicenter Study of Crack Cocaine and HIV Infection in Miami, Florida, examining interrelationships among use of crack cocaine, use of other drugs, sexual activity, and exchange of sex for money and drugs. This study was designed to recruit two groups of approximately equal size: persons who reported current use of crack cocaine three or more times per week, and those who had never used crack. Participants (N = 641) were recruited in Miami. Participants' median age for first use of crack cocaine was higher than for use of alcohol, marijuana or powdered cocaine. It w…

Rocky Mountain spotted fever acquired in Florida, 1973-83.

From 1973 to 1983, 49 Florida residents were reported with confirmed Rocky Mountain spotted fever (RMSF), 25 of whom were considered to have had Florida-acquired disease. Although there was no history of tick exposure for six of these 25 persons, all had contact with dogs or outdoor activities during the incubation period. The tick vectors of RMSF are widely distributed throughout Florida. We conclude that RMSF, although rare in Florida, can be acquired in the state.

Chlamydia psittaci infection in companion birds examined in Florida.

Over a two-year period, companion birds, submitted mostly by small-animal practitioners to the Animal Disease Diagnostic Laboratory, Kissimmee, Florida, were examined for chlamydia. Chick embryo yolk sac and mouse brain inoculations were used for isolation. Chlamydiae were isolated in 58 (20%) of 287 birds examined. Although twice as many birds were examined in 1980 as in 1979, the percentage of positive isolations was 20% for each year. This suggests a constant level of chlamydial infection in companion birds. Most of the isolations were made in psittacine birds; the only positive isolations …

Isolation and identification of pathogenic Naegleria from Florida lakes.

Five cases of primary amoebic meningoencephalitis associated with swimming in freshwater lakes have been recorded in Florida over the past 14 years. The present study demonstrated that pathogenic Naegleria, the causative agent, is relatively widespread. Twelve of 26 lakes sampled only once yielded the amoeba. Populations in three of five lakes sampled routinely reached levels of one amoeba per 25 ml of water tested during the hot summer months. Overwintering in freshwater lake bottom sediments was demonstrated, showing that thermal-discharge pollution of waters plays a miniscule, if any, role …

Ticks in the Tropics: Challenging the Myth of a Lyme-Less Florida.

OBJECTIVES: Few analyses have differentiated locally acquired Lyme disease infection from travel-associated infection, limiting the ability to interpret whether case counts reflect true in-state transmission or increased detection among mobile populations. We analyzed and compared trends in Lyme disease incidence at the national level and in the state of Florida. METHODS: We primarily used Lyme disease surveillance data from the Centers for Disease Control and Prevention from 2010 through 2023. The data included total case numbers, demographic characteristics, and county-level distribution ma…

Patients' Perceptions of Cannabis Contamination in Florida's Medical Program.

INTRODUCTION: As medical cannabis becomes more accessible, product quality must be evaluated. Researchers identified a social media community where Florida medical cannabis patients discuss personal experiences. The objective of this study was to analyze posts in this community to understand patient experience with products purchased from Florida medical dispensaries they believed to be contamination, perceptions of product quality, and desired changes to the Florida program. METHODS: Social networking theory acted as the underlying framework, and we employed a qualitative case study design t…

Local Transmission of Dengue in South Florida: A Case Report.

Dengue is a significant global health concern, primarily prevalent in tropical and subtropical regions, affecting approximately 400 million people annually worldwide. This case report highlights the emergence of dengue in South Florida and raises concerns about the possibility of a local transmission. Unlike most cases, the patients discussed had not recently traveled, prompting the need for further investigation into the local transmission of dengue. In October 2023, two adult brothers presented to the Emergency Department (ED) with symptoms including fever, chills, nausea, vomiting, rash, se…

Geographic disparities in COVID-19 testing and outcomes in Florida.

BACKGROUND: Understanding geographic disparities in Coronavirus Disease 2019 (COVID-19) testing and outcomes at the local level during the early stages of the pandemic can guide policies, inform allocation of control and prevention resources, and provide valuable baseline data to evaluate the effectiveness of interventions for mitigating health, economic and social impacts. Therefore, the objective of this study was to identify geographic disparities in COVID-19 testing, incidence, hospitalizations, and deaths during the first five months of the pandemic in Florida.  METHODS: Florida county-le…

The Dermatologic Care Needs of a Rural Community in South Florida.

For patients in rural areas, primary care is often their only access to healthcare services, and skin concerns are among the most common diseases seen in these settings. This study aims to investigate the most common skin conditions, management trends and patterns of referral to dermatology in a rural and underserved community in South Florida. A retrospective chart review was conducted using medical records from the C.L. Brumback Primary Care Clinic in Belle Glade, FL. The most common skin conditions were fungal infections, unspecified dermatitis, pruritus, skin cancer concern, alopecia, and …

Ecology and public health burden of Keystone virus in Florida.

Keystone virus (KEYV) is an under-studied orthobunyavirus that is transmitted via both horizontal and vertical cycles involving various mosquito species and vertebrate hosts. Historical evidence indicates that KEYV causes sub-clinical infections in humans, and some case studies draw links between this virus and encephalitis. Given KEYV's potential to cause human infections, it is plausible that it causes an under-appreciated proportion of both generic viral infections and unidentified viral encephalitis cases. This review summarizes current knowledge of KEYV and its disease dynamics in order t

The Statewide Epidemiology of Mucormycosis Among Trauma Patients in Florida.

OBJECTIVES: Mucormycosis is a rare angioinvasive infection caused by filamentous fungi with a high lethality among the immunocompromised. In healthy people, the innate immune system is sufficient to prevent infection. The exception to this is deep tissue exposure seen during trauma. The purpose of this study is to evaluate the epidemiology of mucormycosis using a statewide population-based data set. METHODS: This is a retrospective cohort study of all hospital admissions for mucormycosis within the state of Florida from 1997 through the beginning of 2020. A distribution map was created to eva…
