The role of positron emission tomography in the assessment of cardiac sarcoidosis.

The myocardium and the cardiovascular system are often involved in patients with sarcoidosis. As therapy should be started as early as possible to avoid complications such as left ventricular dysfunction, a prompt and reliable diagnosis by means of non-invasive tests would be highly warranted. Among other techniques, 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) has emerged as a high sensitive tool to detect sites of inflammation before morphological changes are visible to conventional imaging techniques. We therefore aim at summarizing the most relevant findings in the liter

FDG-PET/CT Assessment of Pulmonary Sarcoidosis: A Guide for Internists.

BACKGROUND: 18F-fluorodeoxyglucose positron emission tomography integrated with computed tomography (18-F-FDG-PET/CT) is getting wide consensus in the diagnosis and staging of neoplastic disorders and represents a useful tool in the assessment of various inflammatory conditions. DISCUSSION: Sarcoidosis is an uncommon disease characterized by the systemic formation of noncaseating granulomas. Lungs are the sites most often affected, and investigation with high resolution computed tomography and biopsy is essential to achieve a correct diagnosis. 18-F-FDGPET/ CT is effective in the assessment o

Interim FDG-PET Imaging in Lymphoma.

In the present article, the authors reviewed the rationale of FDG-PET/CT performed at an interim time point during therapy (iPET), focusing on the transition from standard, anatomical assessment of tumor shrinkage with CT to document a chemotherapy response, to the use of functional imaging with PET to assess chemosensitivity of the individual tumor. The prognostic or predictive role of iPET in different lymphoma subsets has been reviewed, with particular emphasis on early and advanced-stage Hodgkin lymphoma, diffuse large B-cell lymphoma, peripheral T-cell lymphoma, extranodal natural killer/

18F-fluorodeoxyglucose positron emission tomography in the diagnosis of malignancy in patients with paraneoplastic neurological syndrome: a systematic review and meta-analysis.

PURPOSE: The detection of occult cancer in patients suspected of having a paraneoplastic neurological syndrome (PNS) poses a diagnostic challenge. The aim of our study was to perform a systematic review and meta-analysis to assess the diagnostic performance of FDG PET for the detection of occult malignant disease responsible for PNS. METHODS: A systematic review of the literature (MEDLINE, EMBASE, Cochrane, and DARE) was undertaken to identify studies published in any language. The search strategy was structured after addressing clinical questions regarding the validity or usefulness of the t

Quantification of 18FDG in the Normal Colon-A First Step in Investigating Whether Its Presence Is a Marker of a Physiological Process.

The visibility of the colon in positron emission tomography (PET) scans of patients without gastrointestinal disease indicating the presence of 18F Fluorodeoxyglucose (18FDG) is well recognised, but unquantified and unexplained. In this paper a qualitative scoring system was applied to PET scans from 30 randomly selected patients without gastrointestinal disease to detect the presence of 18FDG in 4 different sections of the colon and then both the total pixel value and the pixel value per unit length of each section of the colon were determined to quantify the amount of 18FDG from a randomly s

PET approaches for diagnosis of dementia.

There is increasing use of neuroimaging modalities, including PET, for diagnosing dementia. For example, FDG-PET demonstrates hypometabolic regions in the posterior cingulate gyri, precuneus, and parietotemporal association cortices, while amyloid PET indicates amyloid deposition in Alzheimer disease and mild cognitive impairment due to Alzheimer disease. Furthermore, the use of combination PET with structural MR imaging can improve the diagnostic accuracy of dementia. In other neurodegenerative dementias, each disease exhibits a specific metabolic reduction pattern. In dementia with Lewy bodi

Malignant pleural mesothelioma: utility of 18 F-FDG PET.

Malignant Pleural Mesothelioma is a rare tumour that arise from the mesothelial cells of the pleura and in recent time the incidence of this disease is rising. Because of the implications for management and therapy, it is important to assess the accurate staging. 18F-fluorodeoxiglucose Positron Emission Tomography (18F-FDG PET) is become a useful tool in the diagnosis of many neoplasms, such as Malignant Pleural Mesothelioma. In particular it has been shown to be useful in the evaluation of the extent of pleural disease, in the establishment of lymph node involvement, in the evaluation of tumo

Future Directions for [18F]FDG PET in Central Nervous System Diseases.

[18F]FDG PET is entering a new phase shaped by changes in representation, validation, and clinical integration. Beyond regional interpretation, network-based and computational approaches aim to provide more explicit and individualized descriptions of brain dysfunction. At the same time, dynamic [18F]FDG PET and total-body PET expand the field toward temporal physiology and systemic brain-body interactions. The clinical impact of these advances will depend on standardized, reproducible, and scalable frameworks that support multicenter deployment and clinically meaningful interpretation. In this

[Management of Antidiabetic Therapy in Patients Undergoing [18F]FDG PET].

Positron emission tomography (PET) with fluorodeoxyglucose {[18F]FDG} is a technique used to identify areas of increased glycolytic metabolism, aiding in the diagnosis, staging, and follow-up of neoplastic, infectious, or inflammatory disease. In people with diabetes mellitus (DM), hyperglycemia and hyperinsulinemia alter the radiotracer biodistribution, compromising the diagnostic accuracy of this test. The absence of clear guidelines may result in inadequate patient preparation and the need to postpone or repeat the test, causing harm to the patient and the institution. This clinical practic

Normal Patterns and Variants in PET Brain Imaging from Childhood to Aging.

Precise interpretation of brain PET imaging in both clinical and research settings depends on a well-defined understanding of normal variation, which is complicated by the evolving metabolic pattern of brain activity over the life span as well as the dynamic nature of human mental states. This article reviews important technical and neurophysiological factors that influence what constitutes a normal brain PET scan. It highlights the role of different radiopharmaceuticals in measuring cerebral blood flow, glucose metabolism, and neurotransmitter activity, while emphasizing that baseline cogniti

[18F]FDG PET/CT imaging in Polyarteritis Nodosa-spectrum vasculitis.

Polyarteritis nodosa (PAN) is a medium-vessel vasculitis that may lead to severe complications and whose diagnosis remains challenging. While [18F]FDG PET/CT (FDG PET/CT) has become a key imaging modality in giant cell arteritis, it also appears promising for diagnosing PAN. This review summarizes current evidence on the use of FDG PET/CT in PAN-spectrum vasculitis, focusing on nuclear imaging features that may support diagnosis. FDG uptake involving medium-sized arteries, especially in the extremities, is a characteristic finding suggestive of PAN. Additionally, a distinct heterogeneous muscu

18F-fluorodeoxyglucose PET in Major Psychiatric Disorders.

18F-fluorodeoxyglucose (FDG)-PET offers a systems-level view of brain dysfunction in psychiatry, with findings more informative at the circuit and network level than as fixed disease-specific patterns. In schizophrenia, addictions, and mood disorders, reproducible abnormalities involve prefrontal, limbic, striatal, and multimodal associative networks, although their expression depends on clinical state, stage, treatment, and comorbidity. This review summarizes principal FDG-PET findings in major psychiatric disorders and integrates them with selected molecular imaging data. Although FDG-PET is

A Rare Case of Four-year Complete Metabolic Response in Multisystem Anti-Jo-1 Antisynthetase Syndrome Assessed by 18 F-FDG PET/CT.

Antisynthetase syndrome (ASyS) is a rare idiopathic inflammatory myopathy with systemic manifestations, defined by the presence of anti-tRNA synthetase antibodies. Interstitial lung disease, particularly pulmonary fibrosis, is associated with a poorer prognosis. Here, we report a case of anti-Jo-1 positive antisynthetase syndrome assessed by 18 F-FDG PET/CT, demonstrating muscular, articular, cutaneous, and pulmonary involvement, with a favorable outcome and complete resolution of abnormalities at 4-year follow-up, without evidence of associated malignancy. This case highlights the potential v
