Isolated Neuropathic Bladder Associated With Human T-lymphotropic Virus Type 1 Infection Precipitating Acute Focal Bacterial Nephritis: A Case Report.

A 71-year-old woman with previously undiagnosed human T-lymphotropic virus type 1 (HTLV-1) infection presented with fever, urinary difficulties, acute abdominal pain, and severe acute kidney injury (AKI). Diagnostic evaluations confirmed acute pyelonephritis (APN) and acute focal bacterial nephritis (AFBN). Despite antibiotic treatment, she required temporary hemodialysis and eventually developed chronic kidney disease. This case highlights the importance of considering APN and AFBN as differential diagnoses in patients presenting with acute abdominal pain and severe AKI. It also underscores t

Triggering role of focal infection in the induction of extra-palmoplantar lesions and pustulotic arthro-osteitis associated with palmoplantar pustulosis.

Palmoplantar pustulosis (PPP) is a chronic inflammatory disorder characterized by sterile pustules predominantly involving the palms and soles of middle-aged women. Whether PPP is the acral type of pustular psoriasis or a distinct entity has long been discussed; however, the clinical features of PPP are really heterogeneous and different between Asians and Caucasians, which may depend on the race with backgrounds of different HLAs. PPP is closely related with psoriasis, but considered to be a distinct entity in Japan. Although the pathogenesis of PPP is still poorly understood, PPP is a repres

Antibody production to heat shock proteins with Mr 65 kD (HSP65) in cutaneous inflammation: a possible relation to focal infection.

In order to correlate the immunomodulatory roles of homologous heat shock proteins with Mr 65 kD (HSP65) to skin diseases, antibody level to recombinant-HSP65 of Mycobacterium leprae was quantified with enzyme-linked immunosorbent assay (ELISA) in the sera of patients. In psoriasis, an insignificant increase was observed in anti-HSP65 IgG (0.111 +/- 0.053, mean +/- SD in 0D492 nm, n = 22), compared with a normal group (0.080 +/- 0.032, n = 9). However, psoriasis of acute guttate-type (PGA), which is often induced after tonsillar infection, showed a significant increase (0.178 +/- 0.032 n = 4,

Orthopedic symptoms in pustular bacterid (pustulosis palmaris et plantaris): Tietze's syndrome and arthritis of manubriosternal joint due to focal infection.

Painful orthopedic symptoms in the sternal area, such as arthritis of the manubriosternal and sternoclavicular joint or Tietze's syndrome, were found in 14 of 132 cases of pustulosis palmaris et plantaris. The majority of the pustulosis palmaris et plantaris patients with orthopedic symptoms had skin eruptions not only on the palms and soles, but also on the backs of the hands and feet, the arms, the legs and the trunk. Such cases were usually accompanied by elevation of the erythrocyte sedimentation rate, leucocytosis, changes in serum globulin, and also pyrexia at the time of exacerbation. T

Evaluation of provocation test monitoring palmoplantar temperature with the use of thermography for diagnosis of focal tonsillar infection in palmoplantar pustulosis.

BACKGROUND: Since focal tonsillar infections are often associated with palmoplantar pustulosis (PPP), provocation tests have been performed for preoperative evaluation of tonsillectomy. However, these tests have not been fully established. OBJECTIVES: To introduce a more sensitive operative indication for tonsillectomy to the patients with PPP, we have monitored the temperature after provocation tests at palmoplantar sites, as measured by thermography, and we hypothesized that this methodology may lead to a more sensitive marker for tonsillectomy. METHODS: Twenty-two PPP patients with/withou

[Improvement of methods for indication of the causative agents of natural focal infections under conditions of emergency situations with epidemiologic consequences].

The improvement of the laboratory diagnostics of quarantine infections (QI) is aimed at obtaining the result at a shorter time, the simplification of making serological reactions, enhancing the specificity and sensitivity of the results due to the introduction of new = generation diagnostic preparations into practical use. The wide practical use of erythrocytic and immunofluorescent diagnostic preparations has shown that they cannot completely meet all requirements simultaneously. The optimized scheme of the indication of the causative agents of QI must ensure the combined use, at different st

Pustulosis palmaris et plantaris due to tonsillar focal infections. From the analysis of long-term observed cases.

We studied the effects of tonsillectomy in patients with pustulosis palmaris et plantaris (PPP) and suspected of having focal tonsillar infections which could still be observed more than one year after tonsillectomy. The rate of healing with tonsillectomy, including 6 cases with remarkable effect, was 76%. The clinical findings from which we expect an effect of tonsillectomy are (a) late onset PPP and early tonsillectomy, (b) tonsils with grade I hypertrophy, oozing of pus, and localized dark-redness on the anterior palatine arches, (c) anamnestic tonsillitis, and (d) positive results both at

Collapsing Focal Segmental Glomerulosclerosis With Anti-nephrin Antibody Following Minimal Change Disease Possibly Triggered by COVID-19 Infection: A Case Report.

A man in his forties experienced foamy urine after COVID-19 infection and received a kidney biopsy. He received tacrolimus for myasthenia gravis; no kidney disease was noted before COVID-19 infection, and he had no family history of kidney disease. The biopsy showed minimal change disease, characterized by IgG deposition in the podocyte and mesangial areas. Prednisone was started at 70 mg, but he required hemodialysis three months after COVID-19 infection. A second biopsy 15 months after the COVID-19 infection showed collapsing focal segmental glomerulosclerosis (FSGS) with IgG deposition in t

Acute Focal Bacterial Nephritis and Bacteremia Due to Staphylococcus simulans Following an Upper Respiratory Infection in a Child: A Case Report.

Coagulase-negative Staphylococcus (CoNS) is a rare cause of UTIs in children and is often regarded as a contaminant in urine samples. We report a case of acute focal bacterial nephritis (AFBN) and bacteremia caused by Staphylococcus simulans following an upper respiratory infection in a pediatric patient. The patient, a four-year-old girl, presented with fever, cough, and a runny nose two days before being referred to our hospital due to persistent fever and poor oral intake. A multiplex nested PCR test of a nasopharyngeal swab sample detected human rhinovirus/enterovirus. Urinalysis showed no

Focal and Segmental Glomerulosclerosis in a Multiple Myeloma Patient After Belantamab Mafodotin Therapy and Severe COVID-19 Infection: A Case Report.

BACKGROUND Focal segmental glomerulosclerosis (FSGS) very rarely occurs in patients with multiple myeloma. Much more common are renal impairments secondary to monoclonal light-chain tubulopathy, AL amyloidosis, light-chain deposition disease, and the so-called monoclonal gammopathy of renal significance. CASE REPORT We report the case of a 79-year-old myeloma patient without noticeable medical problems but with a long history of myeloma treatment beginning 13 years ago. In the ninth line of therapy, he was successfully treated with belamaf mafodotin, an anti-BCMA monoclonal antibody coupled to

A Rare Case of Dilated Cardiomyopathy, Focal Segmental Glomerulosclerosis, and Bell's Palsy in a 29-Year-Old Male After Coxsackievirus Infection.

Dilated cardiomyopathy (DCM) is a severe myocardial disease with diversified etiologies. Coxsackievirus serotype B (CV-B) is a known cause of infectious myocarditis that leads to DCM. The pathogenesis of CV-B myocarditis is complex and involves a combination of tissue destruction from viral proliferation and host immune response. Diagnosis is based on clinical findings and the presence of post-infection elevated titers of IgM antibodies to CV-B. Echocardiography is an important imaging modality that plays a key role in diagnosing DCM. Rare complications of coxsackievirus infection may include

FTIR spectroscopy of peripheral blood mononuclear cells and machine learning: Spectral biomarkers for bacteremia, focal bacterial, and viral infections.

The rapid and accurate differentiation of bacteremia, focal bacterial infections (FBI), and viral illnesses in febrile immunocompromised patients is a critical unmet clinical need that directly impacts antibiotic stewardship. While host-response diagnostics are increasingly applied in clinical practice, most current approaches cannot reliably differentiate systemic from localized bacterial infections and do not offer mechanistic, biochemically interpretable signatures underlying their classifications. This study introduces a unified platform for Fourier-transform infrared (FTIR) spectroscopy a

[Acute Q Fever in Patients with Risk Factors for a Focalized Persistent Infection - Can Prolonged Antimicrobial Therapy Prevent this Complication?].

Q fever is a zoonotic disease, which is endemic in Israel. Most patients have an acute infection, but some cases manifest as a persistent focalized infection. Risk factors for a persistent focalized infection include significant valvular diseases, and aortic aneurism or vascular grafts, immunosuppression or malignancy, pregnancy, elevated anticardiolipin IgG. Recommendations for these high-risk patients after acute Q fever vary from clinical and serological follow-up to combined prolonged antimicrobial therapy, mostly with doxycycline and hydroxychloroquine. Retrospective studies involving a f

Role of brain biopsy in the management of focal brain lesions in HIV-infected patients. Gruppo Italiano Cooperativo AIDS & Tumori.

In this multicenter, retrospective study of 160 brain biopsies in the assessment of HIV-related focal brain lesions, diagnostic sensitivity was acceptable (87%), but the procedure carried considerable morbidity (7.5%) and mortality (3.1%). Moreover, it is not always possible to initiate the changes in therapy indicated by the results, and overall survival remains poor, with a median of 2 months. Criteria for brain biopsy for the diagnosis of focal brain lesions should be redefined to include selected patients for whom a less invasive approach does not yield a definitive diagnosis.

Should antiviral monotherapy with nucleotide analogs be the primary treatment option for focal segmental glomerulosclerosis-related nephrotic syndrome in chronic hepatitis B infection?

Renal involvement is the most common extrahepatic manifestation of chronic hepatitis B virus (HBV) infection. While membranous nephropathy is the most frequent, the association with focal segmental glomerulosclerosis (FSGS) is not as strong, and only a few cases have been described in the literature. In particular, the tip variant FSGS is extremely rare and to our knowledge has not previously been described in association with chronic HBV infection. The management of such cases can be challenging. Immunosuppression may lead to enhanced viral replication and flare-up of the hepatic disease. Ant
