About 25% of operated clubfeet will develop a recurrency or show a marked residual deformity. As main factor the failure of concentric reduction at the time of initial surgery has to be considered. Residual forefoot adduction and supination are the most common persistent deformities. Based on the experience with 94 recurrent/residual clubfeet (patients < 10 years) the surgical treatment at different age-groups is presented. As a general rule soft tissue release is applicable as a repeated procedure until the age of about eight to ten years. For revision in patients between two and eight years …

The pathology of the foot is frequent in the inflammatory rheumatisms. It can finally contribute to the diagnosis. In rheumatoid arthritis, the metatarso-phalangeal joint involvement appears earlier and more obvious. The lesion of the midrearfoot is later and the beginning is more indisious, they often evolve in pes plano valgus deformity. In spondylarthropathies the involvement of the heel and of the toes predominates, but there are more specific symptoms for each disease particularly for the psoriasic rheumatism. Local cares, plantar orthoses, casts, corticosteroid injections, synoviortheses…

Neuropathic and vascular changes in patients with diabetes mellitus put them at risk for developing chronic foot wounds after minor trauma or after pressure has caused a breakdown in the integrity of the skin. Accurate diagnosis of the underlying cause is the first step toward a successful treatment plan, and in patients with severe ischemia, vascular reconstruction may be needed. Neuropathic ulcers respond well to less-invasive procedures, particularly when combined with reducing the pressure that caused the ulcer. When pressure is relieved by means of total contact casting, necrotic material…

262 respondents from an organization for homosexual foot fetishists provide information from a broader sample than clinical cases and allow examination of the effects of sexual preference on fetishism. Data show a wide range of feet/footwear objects to be arousing. Such interests were often associated with particular types of men, yet interests were subject to change over time. Fetishistic arousal rested on both sensual and symbolic aspects of the fetish. Symbolically, it was the theme of "masculinity" that made male feet/footwear arousing, showing parallels to "femininity" evoked by female fe…

In rheumatoid arthritis (RA) we find a high rate of foot involvements, especially in progressive cases when we see patients submitted for hospital treatment. In about 50% of these patients the ankle joints are involved (25), in juvenile RA even in two thirds of all cases. More than half of all progressive cases show also joint affections of the forefoot. Mainly the metatarsophalangeal joints are involved, especially the first MTP joint. We also can find a high involvement rate of the interphalangeal joint of the big toe. In RA the peripheral toe joints II-V are less frequently involved. Change…

The most common conditions seen in a children's foot clinic are those of flatfeet and pigeon toe. Both respond to conservative treatment and few require surgical correction. However, one sees other types of flatfeet such as congenital vertical talus and tarsal coalition that frequently require surgical intervention. In general, many of the foot problems seen in children lend themselves to conservative measures, and surgical treatment can be delayed until a later age.

BACKGROUND: Prosthetic feet are available in a range of stiffness categories, however, there is limited evidence to guide optimal selection during prosthetic foot prescription. The aim of this study was to determine the effect of commercial prosthetic foot stiffness category on foot-ankle biomechanics, gait symmetry, community ambulation, and relative foot stiffness perception.
METHODS: Participants were fit in randomized order with three consecutive stiffness categories of a commonly-prescribed prosthetic foot. Prosthetic foot roll-over shape and ankle push-off power and work were determined…

OBJECTIVE: This study reports patient outcomes and predictors of outcomes after total contact casting (TCC) for diabetic stump ulceration in patients with partial-foot amputations.
METHODS: Retrospective review of patients treated with TCC for diabetic stump ulcerations with ipsilateral partial-foot amputation at a tertiary center from 2015 to 2022 was performed. Patients lost to follow-up, those unable to tolerate TCC, and those with partial amputations of digits 2 to 5 were excluded. Patient demographics, outcomes, and complication rates of TCC were collected and compared. Multivariable lin…

It is essential to master the techniques of tendon transfer and to know the indications, so as to be able to treat tendinopathy, tendon tear or static deformity in the foot and ankle. The type of transfer depends on the pathology, with the aim of reconstructing or supplementing the affected muscle. Complete clinical and paraclinical assessment is needed to determine feasibility and draw up the preoperative plan according to axial deformity and associated retraction. Success depends on several factors: rigorous technique, fixation method, tension adjustment, immobilization, and rehabilitation. …

Skin infections are an important issue among participants in expedition-length adventure races. Prolonged stress, scant sleep, and water exposure mean that competitors are at risk of uncommon manifestations of infections. Ulcerative tinea pedis is an example of this. We present a case with characteristic clinical manifestations, including the "sandpaper symptom." There is limited literature exploring infectious foot complaints in expedition adventure racers. Beyond this case report, more research is needed to better understand incidence rates, risk factors, diagnostic measures, treatment, and …

BACKGROUND: A self-contained, self-controlled, pneumatic power harvesting ankle-foot orthosis (PhAFO) to manage foot-drop was developed and tested. Foot-drop is due to a disruption of the motor control pathway and may occur in numerous pathologies such as stroke, spinal cord injury, multiple sclerosis, and cerebral palsy. The objectives for the prototype PhAFO are to provide toe clearance during swing, permit free ankle motion during stance, and harvest the needed power with an underfoot bellow pump pressurized during the stance phase of walking.
METHODS: The PhAFO was constructed from a two-…

Ankle-Foot Orthoses (AFOs) are common non-surgical treatments used to support foot and ankle joint when their normal functioning is compromised. AFOs have relevant impact on gait biomechanics, while scientific literature about effects on static balance is less strong and confusing. This study aims to assess the effectiveness of a plastic semi-rigid AFO in improving static balance on foot drop patients. Results underline that no significant effects on static balance is obtained on the study population when the AFO is used on the impaired foot.
